Enclosed please find a spreadsheet with a list of wells from Northstar that were treated
with corrosion inhibitor in the surface casing by conductor annulus. The corrosion
inhibitor is engineered to prevent water from entering the annular space and causing
external corrosion that could result in a surface casing leak to atmosphere. The
attached spreadsheet represents the well name, top of cement depth prior to filling and
volumes of corrosion inhibitor used in each conductor.
As per previous agreement with the AOGCC, this letter and spreadsheet serve as
notification that the treatments took place and meet the requirements of form 10-404,
Report of Sundry Operations.

IreM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
IreM 4b: TPI (Top of Producing Interval).
IreM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in
other spaces on this form and in any attachments.
IreM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).

Make up cement head and TIW on DP pup.
Continue running liner on DP, 10,687' @ 00:00 hrs.

250 psi and 5000 psi for 5 mins., OK -Perform Accumulator

AOGCC wavied right to witness of BOP test.
LD 5" test jt. PU 4" test jt. and test upper and lower variable
pipe rams on 4' size, OK. Test 4" TIW and 4" dart floor valves;
all to 250 psi and 5000 psi for 5 mins. OK.
Install wear bushing.
PU Schlumberger E-line logging tools. RU to pr. tst csg/liner.
Replace breaker in #3 SCR.
Test casing/liner to 3000 psi for 30 min. OK. Bleed off and
blow do lines.
PJSM. RU E-line logging tools. Run GR-CCL PDC log from
12,240' to 9180'. Tie in with original PDC log dated 2/17/2002.
Clear floor. Chg elevators. BD test jt and floor vlvs.
PU 4", 14#, S-135, HT-40 DP. 53 jts picked up at 00:00 hrs.
Noted slack in drilling line on the drum near the locking wedge.

This permit to drill does not exempt you from obtaining additional permits or
an approval required by law from other governmental agencies and does not
authorize conducting drilling operations until all other required permits and
approvals have been issued. In addition, the Commission reserves the right to
withdraw the permit in the event it was erroneously issued.
Operations must be conducted in accordance with AS 31.05 and Title 20,
Chapter 25 of the Alaska Administrative Code unless the Commission
specifically authorizes a variance. Failure to comply with an applicable
provision of AS 31.05, Title 20, Chapter 25 of the Alaska Administrative Code,
or a Commission order, or the terms and conditions of this permit may result

Subject: NS14 Permit Application for Sidetracking NS14
Dear Mr. Hubert,
NS14 experienced high GOR in the Ivishak production from either gas coning or
communication with nearby gas injectors. As a result, NS14 has been shut-in. It is
planned to abandon the current wellbore in the Ivishak and sidetrack NS14 with a
horizontal lateral at a new location. A Sundry Notice has also been submitted for
abandoning the current wellbore.
Maximum anticipated surface pressure (full column of gas from TD to surface) is 4619
psi. We will be utilizing Nabor's 33e 5000 psi four preventer stack (3 rams and an
annular). The drilling operation will require running 2381' feet of 7° intermediate liner. We
are requesting a variance to the requirement to have two ram preventers to cover the 7"
liner and 2 ram preventers for the 5" DP. We would like to use a variable bore ram on top
that can cover both DP and casing. This would provide one ram and the annular for the 7"
liner and two rams for the DP and does not necessitate changing rams after picking up
the 7" liner. In the event of a well control incident, our standard practice is to immediately
cross-over to DP to allow ram to ram stripping (if required) and we would prefer not to
have to change out rams during ashut-in.
The only close approach for the sidetrack is with the original NS 14 wellbore which will be
abandoned.
NS14 sidetrack will utilize gas detection equipment but is not planned for mudloggers.
Please find attached the NS14 sidetrack Well Plan Summary, directional plan and

NS 14 Permit Data
NS 14 Sidetrack Scope
The original NS 14 completion will be plugged and abandoned due to high GOR possibly from
coning and/or communication with a nearby gas injector. This will be achieved using a full bor~:
squeeze of 9 to10 bbl of Class G, 1s.8 ppg cement while displacing the tubing volume.
The existing s-1/2" X 4-1/2" tubing will be chemically cut at approximately 12,266' MD and pulled.
A bridge plug will be set at approximately 10,234' TMD and a whipstock and milling assembly run
to cut a window from 10,214' to 10231' MD (8,70s' to 8,718' TVD). The original wellbore will be
sidetracked in 8-1/2" hole (opened to 9-1/2") to 12426' MD (Sag Target) and a 7" drilling liner will
be set and cemented. After cementing the 7" liner, 6-1/8" hole will be landed at 90° and continue for
approximately 1,500' in the Ivishak. A 4-1/2" 13 Cr liner will be run and cemented and the liner will
be perforated with tubing conveyed guns. The well will be completed with 4-1/2", 13 Cr Vam Top /
tubing. The new horizontal section will minimize gas coning effects and minimize communication
with the injector -reducing GOR.
Pre-Riq Operations
1. Test 9-5/8" pack-off to 5000 psi.
2. MIT inner annulus to 4200 psi.
3. Through tubing cement squeeze perforations with 15.8 ppg cement slurry. Final
cement volume should be determined by infectivity test. Minimum cement volume
should be sufficient to cover perforations and leave TOC at +/- 12426' TMD. This
is 505' of cement above the PBTD of 12931' and requires a minimum volume of
7.7 BBLS to fill the liner. Desired minimum volume is 9.7 BBLS of cement slurry.
(7.7 BBLS to fill pipe and 2 BBLS into perfs. WOC
4. Run wireline drift tubing and tag TOC. Pressure test tubing to 2000 psi.
5. Run and set plug in XN nipple at 12318'.
6. Chemically cut tubing at 12266' TMD above packer.
7. Install BPV.
Drilling Riq Operation:
1. MIRU.
2. Circulate well clean with sea water.
3. NU BOPE and test to 5000 psi (3500 psi annular).
4. Pull 5-1/2" X 4-1/2" tubing string
5. PU DP. RIH w/ casing scraper to +/-10400'. Test casing to 3500 psi.
6. RIH and set 9-5/8" bridge plug at ~ 10234' TMD. Final setting depth will be
determined by CCL to space out bridge plug for whipstock. The desired top of
window is 10214'. Pressure test bridge plug.
7. Run and set whipstock on top of BP. Mill a window in 9-5/8" casing from +/- 10214
to 10230'.

8. Perform a FIT to a minimum of 14.0 ppg EMW, (FIT/LOT procedure on file with ~
AOGCC).
9. Pick up 8-1/2" directional assembly. Ensure the mud is in good condition prior to
drilling ahead. Drill the drilling liner hole section to the top of the Sag at
approximately 12368' MD (10709' TVD RKB). The Intermediate casing point has
been picked at 12426' MD. POOH.
10. Run and cement a 7" 26# Hydril 521 drilling liner. Displace cement with mud. Set
the liner hanger and liner top packer. Circulate excess cement from well bore.
11. POOH and lay down liner running tools. Test 9 5/8" x 7" casing to 2900 with 12.5
ppg fluid. This test pressure may change based on the actual mud weight in the
hole.
12. RIH w/ 6" directional assembly. Clean out the 7" 26# liner to the landing collar.
Displace the wellbore to 9.1 ppg mud.
13. Drill out remaining cement. Drill 20' of new formation below 7" shoe with 9.1 ppg ~.
MW. Perform a FIT to 11.5 ppg. EMW (FIT/LOT procedure on file with AOGCC).
14. Drill 6-1/8" horizontal production interval to the proposed TD of 14656' MD.
Condition mud weight to 9.3 ppg prior to POOH.
15. Run and cement the 4-1/2" 12.6# 13 CR-80 liner with 1 to 3 ECPs. Set the liner
hanger and liner top packer. Circulate a minimum of one bottoms-up to remove ~
excess cement with a 9.3 ppg mud.
16. Once a successful cement job is obtained, pressure test liner lap to 1500 psi with
9.3 ppg mud to confirm pressure integrity.
17. Change well over to 9.3 ppg completion fluid (less than 80 NTU's) and by using
spacers as per MI recommendation.
18. Shut down and monitor the well for a minimum of 30 minutes (negative test on
liner top).
19. POOH and lay down liner running tool.
20. Close blind rams and test 9 5/8" casing x 7" drilling liner x 4'/z" liner to 3700 psi
for 30 minutes. This liner lap test assumes a 15.5 ppg LOT at 7" shoe (10704' r"
TVDrkb) + 500 psi.
21. Run tubing conveyed perforating guns and perforate as determined from logs.
22. Run and land the 4-1/2" 12.6# 13-CR80 Vam Top tubing string with fiber optics' j
as per completion diagram. Pressure test the control lines and tubing hanger
seals to 5000 psi.
23. Set 7" X 4'/2" packer. Test tubing and annulus to 4200 psi. Open shear valve in
gas lift mandrel.
24. Freeze protect the tubing and annulus to 3000' TMD/TVD
25. Set the TWC valve in tubing hanger. Nipple down BOPs. Nipple up the 13-5/8"
5000 psi x 7-1/16" 5000 psi tubing head adapter and 7-1/16", 5000 psi tree. Install j
all flanges and needle valves.
26. Test the tubing head adapter and tree to 5000 psi. Pull TWC valve and install
BPV.
27. Ensure all tree and annulus valves are secured. RD and move off well.
