The wellbore's schematic was updated on 8/15/16 with the annotation of "Depth Correction" as seen on the third line of the schematic. After researching the reason for the correction, the original schematic that was sent to the AOGCC had not been correlated to the RKB of original completions tubing tally. The corrections were made, and the attached schematic is representative of the accurate depths. Please let me know if you need anything else. Each of these wells was found to have a void in the conductor. These voids were filled with cement if needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. The cement was pumped January 18th and January 24th, 2009. The corrosion inhibitor/sealant was pumped February 21St through February 23`d , 2009. The attached spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. Jared Break was the CPAI representative for the tests today. The pretest pressures on each well were monitored and found to be stable and the standard annulus pressure tests were then performed with the three wells passing their MITs. I inspected three well houses and no exceptions were noted. Summary: I witnessed successful MIT's on CPAI's water injection wells 2W-04, 2W-08 and 2W-14 in the Kuparuk River Field. This injection valve will act as a one-way check valve allowing downward injection of water and gas while preventing upward flow of these fluids. Individual subsequent sundry notices will be filed when the actual well conversions occur.- As stated in form 10-403, well conversion to water injection will occur in two primary stages. The first stage will convert approximately four wells per drillsite to achieve a nine-spot pattern. First stage' conversions are expected to be completed by approximately 10/15/85, prior to waterflood startup. The second stage of conversions will convert approximately four additional wells per drillsite to achieve a line drive pattern. Second stage conversions are expected to be completed mid-December to January 1986.
