Each of these wells was found to have a void in the conductor. These voids were filled with cement and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10 -404, Report of Sundry Operations. The cement was pumped on 5/23, 2011. The corrosion inhibitor /sealant was pumped 6/22, 6/24, 6/25, 6/26, and 6/27, 2011. Box 100360 Anchorage, AK 99510-0360 ,,~~""'r Dear Mr. Wheatall: The Alaska Oil and Gas Conservation Commission ("Commission") performed an inspection of your well safety valve system (SVS)at~!ì~~R.iver1..Jn.it2X ("KRU 2X") pad on October 15, 2004. Attached is a copy of the InspedionReport. Regulation at 20 AAC 25.265 requires all wells to be equipped with a Commission approved fail-safe automatic surface SVS capable of preventing uncontrolled flow by shutting off flow at the wellhead and shutting dQwn any artificial lift system where an overpressure of equipment may occur. If installed, a subsurface safety valve must be maintained in working order and is subject to performance testing as part of the SVS. An SVS component failure rate of 12.5 percent was identified during the above-noted inspection. Commission policy, as outlined in a letter dated November 14, 1995, states that the testing frequency will be increased when a failure rate exceeds 10 percent. The AOGCC-AOGA Safety Valve System Task Force report issued June 13, 2003, supported retention of the 10% failure thresho ld. Also noted during the Commission's inspection were unsupported flow piping and pilots on KRU 2X pad, specifically Wells 2X-03 and 2X-09. These were not included in the failure rate calculation but clearly are deficiencies in the SVS that must be corrected. Effective October 15, 2004, and until an acceptable level of performance is achieved, the safety valve systems associated with wells located on KRU 2X must be tested every 90 days. After the rig is released, an acid and/or frac job will be done. A 13-5/8", 5000 psi, SRRA BOP stack will be used on well work. BOP equipment will be tested weekly and operationally t~ested each trip. A non-freezing fluid will be left in all uncemented annuli within the permafrost interval. A 5000 psi WP wireline lubricator will be used and tested on all wireline work. The subsurface safety valve will be installed and tested upon conclusion of the job. The treatment consisted of 3150 gallons of 12% HCL w/MSR to clean up the perforations, followed by the frac. A 13-5/8", 5000 psi, SRRA BOP stack will be used on well work. The BOP equipment will be pressure tested weekly and operationally tested each trip.
