Oil or gaB zones either previously productive or correlated with produc- tive zoneS must be protected by adequate cemont plugs. 4. That portion of the hole between cement pluga shall be filled with heavy mud fluid. Top of the hole shall not be cloHed until the District Engin- eer or the Supervisor has been given an opportunity to inspect it. 5. Abandoned wells shall be marked with markers consisting of a pipe not less t than 4 inches in diameter and not less than 10 feet in length, of which 4 feet shall be above the general ground level and the remainder embedded in concrete. The top of the pipe shall be closed with a cap or cement plug and the marker should show in a permanent manner the name of operator, well number and location. ' 6. Equipment and valuable material shall be removed from the well site or dis- mantled and arranged in an orderly manner; junk and debris shall be removed or destroyed; pits and cellars shall be filled and leveled and the well site, including sumps, shall be placed in a clean and orderly condition. 7. The hole should be converted to a fresh water well if practical. 8. A subsequent Report of Abandonment on Form 9-331a, giving a detailed ac- count of all work performed, including location of cement plugs, casing pulled, casing left in the well, date of placing surface plug or cap, shall be filed in triplicate, within 15 days after completion of'the work and the District Engineer or the Supervisor shall be notifi.ed_whenthe well site has been cleaned up and is ready for inspection. . Compliance with Federal Oil and Gas Operating Regulations. 2. The date actual drilling is started shall be reported promptly to the Regional Oil & Gas 'Supervisor. 3. Any change in the approved program must receive prior approval of the Regional Oil & G.£,s Supervisor. 4. Adequate surface casing shall be used to protect fresh water zones and it must be cemented with sufficient cement to circulate to the surface. 5. All pertinent data, including electrical logs,. directional surveys, core descriptions and analyses, stratigraphic and faunal determinations, formation tests, 2nd similar information shall be submitted in duplicate. Electrical logs and súnilar surveys and the results of formation tests should be submitted as soon ,as the records are available. Other data not previously submitted shall be included in or submitted with the log and well history. 6. All rigs or wells shall be permanently marked in a conspicuous place and should shew ~a~e of operator, lease serial number, number designation and location of the well. 7.
