ConocoPhillips would like to rescind the PTD for the following wells with their corresponding approved PTD numbers:

Wet sand were found while drilling the 1H-108 injector, a subsequent pilot hole was drilled that gave further
information to move the landing point for the 1H-104 producer. We will be repurposing the 11-1-108 surface casing string
to drill the 1H-104 targets at a future time and will send new permit packs to the AOGCC for that project when finalized.

Enclosed is the approved application for the permit to drill the above referenced development well.
Per Statute AS 31.05.030(d)(2)(B) and Regulation 20 AAC 25.071, composite curves for well
logs run must be submitted to the AOGCC within 90 days after completion, suspension or
abandonment of this well.
This permit to drill does not exempt you from obtaining additional permits or an approval required
by law from other governmental agencies and does not authorize conducting drilling operations
until all other required permits and approvals have been issued. In addition, the AOGCC reserves
the right to withdraw the permit in the event it was erroneously issued.
Operations must be conducted in accordance with AS 31.05 and Title 20, Chapter 25 of the Alaska
Administrative Code unless the AOGCC specifically authorizes a variance. Failure to comply
with an applicable provision of AS 31.05, Title 20, Chapter 25 of the Alaska Administrative Code,
or an AOGCC order, or the terms and conditions of this permit may result in the revocation or
suspension of the permit.

Application for Permit to Drill, Well 1 H-104

ConocoPhillips Alaska, Inc. hereby applies for a permit to drill a grass roots onshore producer, a penta-lateral horizontal
well in the West Sak A2, A3, A4, B and D sands. The expected spud date for this well is July 4, 2018. It is planned to
use rig Doyon 142.
A 13-1/2" surface hole will be drilled on diverter & 10-3/4" surface casing will be run to total depth and cemented to
surface. The remaining intervals will utilize Doyon 142's 13-5/8" x 5000 psi BOP stack with required ram installations
and tested on a 14 day cycle in accordance with COP and AOGCC policy. The intermediate hole section will drill a 9-
7/8" hole with 7-5/8" casing run to total depth & cemented in place. The first 1H-104 6-3/4" production lateral will be
geo-steered in the West Sak A2 sand and lined with 4-1/2" slotted liner. The subsequent production laterals will again be
drilled with 6-3/4" hole and lined with 4-1/2" slotted liner in the following order: West Sak A3, A4, B and D sand. A
separate PTD application will be submitted for each lateral: 1H-104, 1H-104L1, 1H-104L2, 1H-104L3, and 1H-104L4
laterals.

Please reference BOP schematics on file for Doyon 142 & within this PTD application. Due to lower
reservoir pressure in the West Sak a fixed stack ram placement will be utilized to fulfill all barrier
requirements during well construction operations, graphic attached.
Drilling Hazards Information
Requirements of 20 AAC 25.005 (c)(4)
An application for a Permit to Drill must be accompanied by each of the following items, except for an item already on file with the commission and
identified in the application:

(A) the maximum downhole pressure that may be encountered, criteria used to determine it, and inaxinnmr potential suiface pressure based on a
urethane gradient;
The maximum potential surface pressure (MPSP) while dr' ing 1H-104 is calculated using an estimated
maximum reservoir pressure of 1724 psi (based on JR-05A pressure at a 4005.2' TVD reference datum), ✓
a gas gradient of 0.10 psi/ft, and the estimated depth of the West Sak A2 sand at 3855' TVD:

Requirements of 20 AAC 25.005 (c)(5)
An application for a Permit to Drill must be accompanied by each of the following items, except for an item already on file with the commission and
identified in the application:
(5) a description of the procedure for conductingfarmation integrity tests, as required tinder 20 AAC 25.03069r
A procedure is on file with the commission for performing FIT/LOT's. The FIT/LOT will be performed
on the motherbore of the 1H-104 penta-lateral well (11-1-104). ConocoPhillips requests a waiver on -
subsequent laterals FIT/LOT (1H-104L1, 1H-104L2, 1H-104L3, & 1H-104L4). Waiving the upper
lateral FIT/LOT may be undertaken if the following criteria are met:
1. Primary cement job pumped as per design with no issues.
2. WellView cement report submitted to AOGCC for review of job before 2nd lateral is drilled
(1H-104L1).
3. No abnormal pressure is discovered while drilling the motherbore well (lowest lateral).
4. Kill weight fluid is confirmed using a static flow check at the shoe for 30 minutes post TD of
the 1 H-104 motherbore.
If any of the above -mentioned criteria are not met, performing an FIT/LOT of the upper laterals will be
undertaken.

Proposed Drilling Program

The proposed drilling program follows. Please refer to attachment for proposed well schematic.
1. Move in / rig up Doyon Rig 142. Install 21-1/4" Annular with 16" diverter line and function test.
2. Spud and directionally drill 13-1/2" hole to casing point as per directional plan. Survey & log with MWD, GWD,
GR/RES.
3. Run and cement 10-3/4", 45.5#, L80, Hyd-563 casing to surface. Displace cement with mud & check floats upon
bumping plug. Perform top job if required after notifying AOGCC. Evacuate top 2 feet of cement from annulus
for sealant job (post rig).
4. Remove diverter system, install and test 13-5/8" x 5,000 psi BOP's to 3500 psi (annular preventer to 2500 psi)
high and 250 psi low. Notify AOGCC 24 hrs before test.
5. PU 9-7/8" PDC bit and directional drilling assembly, with MWD, Gamma, Resistivity. RIH and clean out to 100'
above shoe. Test surface casing to 3,000 psi for 30 minutes.
6. Drill out shoe track and 20' of new formation. Perform FIT to a minimum of 11.5 ppg, maximum of 14 ppg <-
EMW LOT/FIT, record results.
7. Directionally drill to intermediate hole TD landing in the West Sak A2 sand package.
8. BROOH to surface shoe, circulate well clean then POOH on elevators.
9. Laydown intermediate BHA.
10. Run 7-5/8", 29.7 L80, Hydril 563 casing from surface to TD.
11. Pump cement job as per Schlumberger proposal. Top cement to be placed @ +/- 2,883' MD (250' TVD
minimum above Ugnu C formation).
12. Install 7-5/8" packoff & test to 5000 psi.
13. Flush 10-3/4" x 7-5/8" annulus with water, followed by diesel to freeze protect.
14. PU 6-3/4" bit, and drilling assembly with MWD, Gamma, Resistivity, and Periscope.
15. RIH on 4" DP to top of float equipment. Clean out cement to top of float equipment & test casing to 3,500 psi for
30 minutes, testing against the annular to accommodate the West Sak fixed stack BOP configuration.
16. Drill out shoe track and 20' of new formation. Perform LOT to a minimum of 11.5 ppg EMW maximum FIT of
13.0 ppg, recording results.
17. Directionally drill 6-3/4" hole geo-steering through the West Sak A2 sand to TD per directional plan.
18. Circulate 1 BU after TD and BROOH to 7-5/8" shoe CBU to clean casing & POOH on elevators.
19. Lay down Production BHA.
20. PU and run 4-1/2", 12.6# slotted liner w/ ZXP liner hanger & packer. The top of liner will be placed at -6415'
MD, allowing for overlap in the 7-5/8" casing & proper spacing for the whipstock assembly.
21. Set liner hanger & packer using MWD to orient hanger.
22. PT liner top packer to 2,500 psi for 10 minutes, chart results.
23. Flow check well for 30 minutes.
24. POOH with liner running tools and lay down.
25. Run & set Baker whipstock system, placing the KOP for 1H-101L1 at +/-6,664' MD / 3,898' TVD, just below the
top of the A3 sand orienting with MWD.
26. Mill 6-3/4" window until top mill across face & clean up window.
27. POOH with milling BHA.
28. PU 6-3/4" bit, motor, MWD, and magnets to perform a clean out run & land the well. (GR, Res)
29. RIH, orient through window and tag bottom.
30. End 1H-104 PTD & begin 1H-104L1 PTD.
Discussion of Mud and Cuttings Disposal and Annular Disposal
Requirements of 20 AAC 25.005 (c)(14)
An application for a Permit to Drill music be accompanied by each of the folloiwutg items, except for an item already on file with the commission
and identified in the application:
(I 4) a general description of hoar the operator plans to dispose of drilling mud and cuttings and a statement of whether the operator intends to
request authori ation under 20 AAC 25.080 for an annular disposal operation in the well.:
Waste fluids and cuttings generated during the drilling process will be disposed of by hauling the fluids to a KRU Class II ''
disposal well at the 1B Facility. If needed, excess cuttings generated will be hauled to Milne Point or Prudhoe Bay Grind and
Inject Facility for temporary storage and eventual processing for injection down an approved disposal well, or stored, tested
for hazardous substances, and (if free of hazardous substances) used on pads and roads in the Kuparuk area in accordance
with a permit from the State of Alaska.
