Re: Location Clearances David River Wells 1 and 1-A.
Dear Mr. Gardner:
You requested location clearances on the two David River wells drilled
by USA Pan American (now AMOCO). David River #1 (PTD 68-084) and David
River #1-A (PTD 68-106) wells were drilled and suspended in 1968; all
downhole work was completed on the #1 well.
In 1993, surface abandonment of the #1-A well was accomplished; this
included surface plugs, casing cut-off, and marker post installation.
Location Clearance could not be granted at that time because on the
well, same pad, the casing needed to be cut off below ground level and
marker installed.
Our inspector John Spaulding visited the David River location with Jesse
Mohrbacher and others on June 2, 1994. He observed the casing in the #1
well being cut off 2' below original grade and the marker plate being
installed (see his report "David River #l.doc" dated June 10, 1994 for
additional details). John noted the pad was clean and graded and there
was no trash or equipment left on the pad. John recommended the David
River #1 for location clearance.
Well Abandonment and Location Clearance is hereby approved for USA Pan
American (now AMOCO) David River well #1 (PTD 68-084) and David River
well #1 A (PTD 68-106)

Clearance
Dear Mr. Wondzell:
Please find the enclosed Well Completion or Recompletion Report and Log for the
USA Pan American David River No. 1 well. As part of the final abandonment
procedure for the David River location, a 3'x3'x3/4" subsurface steel dry hole marker
plate was welded on the David River No. 1 well on June 2, 1994. This operation
completed the AOGCC required tasks to obtain final location clearance for the site.
Based on this activity and previous well abandonment operations at the site, AMOCO
Production Company hereby requests that location clearance be issued for the David
River location.
The attached drawing shows the location of the subsurface plate in relation to the
David River No. I-A dry hole marker post.

Thursday June 2, 1994: I traveled to Nelson Lagoon AK. to witness the surfac~
location abandonment of David River well #1, an AMOCO Production Co. well
drilled in 1968,
The casing was cut off approximately 2' below original grade and a 3i4" plate was
welded on the 36" casing with the apprOpriate data bead welded on the plate. The
area was cleaned last year and all debris was hauled out and disposed of. The pad
area was in very good shape with all reserve pits closed out. The area is graded
back to the shape of the surrounding terrain,
I recommend the David River 01 location for location clearance.
Summary: I witnessed the welding on of the surface location plate and location
abandonment of David River #1, AMOCO Prod. Co.

Dear Mr. Gardner:
On May 20,1993 you submitted a 10-403 to perform final abandonment and location
clearance on Amoco's David River well no. I-A, PTD 68-0106. This well was drilled on
federal lease A-062169 which has since been TA'd to the State of Alaska.
Your 10-403 (Sundry Notice) explained the proposed procedure which we examined
carefully; once satisfied that the procedure was the best available considering when the
well was drilled and its location, we approved it on May 18, 1993.
On or about June 1, 1993 Jesse Mohrbacher of your office asked me, as field inspections
supervisor, which operations the AOGCC wanted to witness. After some discussion, I
stated that in consideration of the uncertain timing and the logistics and cost, if you would
keep accurate daily records of the operations and take usable pictures of cementing the
annuli, the installed marker post, and the location after clean-up, we would be able to
provide location clearance with-out personally inspecting the location.
I have reviewed the "Daily Operation Reports" and the photos covering the cementing,
marker post, and cleaned location of David River well no. 1-A. As I requested on July 1,
1993, you supplied additional photos to show the lettering on the marker post. At this
point I was convinced that the David River well no. 1-A was ready for "Location
Clearance".
Bob, we can not give location clearance until all wells on a pad are properly abandoned.
Therefore, while preparing a Location Clearance letter for David Johnston's signature, I
checked the files of the no. 1 well (PTD 69-84) and found this statement in the last Sundry
Notice (12-3-68) filed on the no.1 well:
"The well will be capped and a marker set when Well No. 1-A has been drilled
and equipment is removed."

I carefully checked both well files and talked to Jesse Mohrbacher of your office. The best
determination is that the no. 1 well is properly abandoned except there is no marker post
or plate.
Your August 18, 1993 letter to me states that you plan a visit to the location in June 1994
and at that time you would install the dry hole marker on the no. 1 well. Please let me
know when you plan the site visit so we have the opportunity of having an inspector
accompany you.
Once the appropriate marker has been installed on the no. 1 well, we will be able to
approve Location Clearance on both wells.
Bob, we appreciate your efforts in abandoning these old wells.

of David River Well No. 1
Dear Mr. Wondzell:
This letter is in response to your inqUiry of the current status of the USA Pan American David
River No. 1 well t. hat was uncovered during abandonment operations for the David River No.
1-A well. During final site grading activities, the No. 1 well was discovered approximately
30 feet north of No. 1-A with a 20-inch casing stub sticking up inside a 30-inch conductor
(see attached photo). No leakage of Wellbore fluids was evident from the 20-inch or 30-inch
casing in the No. 1 well. The 20-inch casing was cemented to surface and extended
aPproximately 2 feet above the 30-inch casing. The 20-inch casing was cut off level with the
30-inch conductor and covered with backfill. Final elevation of the No. 1 well is
approximately 2 feet below original grade.
Previous cOrrespondence from Pan American Petroleum Corporation (see attached Sundry
Notice) dated December 3, 1968, indicated that a dry hole marker would be installed on the
No. 1 well when the David River No. 1-A well was completed. This, however, was not done
as stated, and in order to complete the abandonment of the No. 1 well, we propose that a
dry hole marker be installed on the No. 1 well during 'the summer of 1994. A site visit is
currently planned in June 1994 to inspect the site and the dry hole marker could be installed
at that time. A letter report, documenting the procedure would be provided to the
Commission after installation of the dry hole marker. ~ ,,.

USA Pan American David River Well N6. 1 was plugged and abandoned on
11-19-68 as follows:
A 100 sx cement plug was set in 20" casing from 113' to approximately
10' below the top of 20" casing. The well will be capped and a marker set when
Well No. 1-A has been drilled and equipment ig removed.
Well No'. 1-A will be drilled approximate2y 30' South of Well No. 1. The
new location will be surveyed and plat furnished when available.

In reference to the subject well, the Alaska Department of Fish and Game requests
the following wildlife stipulations be attached to the permit to drill.
1 Prior to drilling, a ring dike will be constructed around the drilling
area to contain any spills that might occur. The dike will be lined
so as to make it impervious to petroleum products.
2. Prior to the beginning of drilling, a sump of sufficient size to contain
all waste flui'ds will be constructed. This sump will also be made
impervious to petroleum production.
3. Prior to the abandonment of the site, the sump must be cleared of all
oil, and be covered. ,
4. The operator must dispose of all refuse, including, but not limited to'
pallet board,, garbage, visqueen, sacks' barrels, etc., in an acceptable
manner.
