General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and
leases in Alaska.
Item 1 a: Classification of Service wells: Gas injection, water injection, Water-Alternating-Gas Injection, salt water disposal, water
supply for injection, observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
Item 4b: TPI (Top of Producing Interval).
Item 8: the Kelly Bushing elevation in feet abour mean low low water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
Item 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
Item 20: True vertical thickness.
Item 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location
of the cementing tool.
Item 23: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item

an ave rate of 6 bpm at 1,000 psi at 285 bbls slowed rate to 4 bpm at
975 psi for 60 bbls then to 7.0 bpm at 1,400 psi until 305 bbls slowed
rate to 4 BPM at 1900 psi & bumped plug wI total of 331.5 bbls,
Bumped plug wI 2,400 psi, Checked Floats & Held, Bled back 2.75 bbls
for a total of 328.75 total bbls displaced (Calculated 323 bbls), CIP @
00:36 hrs 5/17/03, Displaced entire job with Dowell. Unable to
reciprocate Pipe During Cement Job. Check flow- Well static. Good
returns thooughout job.
Rigged down cement head and flushed stack.
Changed out bales and picked up tubing elevator.
Installed and tested packoff.
Rigged up to run completion. PJSM.
PU and RIH with completion on 251 Jts 3 1/2" 9.3# L 80 EUE tubing to
7943'.
PJSM, Rig & test 7" casing to 3500 psi fl 30 min. -OK
Perform injecttivity test on 7" x 9-5/8" annulus, EMW of 16.5 ppg using
11.8 ppg mud with 640 psi @ 9-5/8" shoe 3071' MD & 2606' TVD
Drop Baker 2" packer setting ball. Will set SAB-3 packer @ 7911.19',
Slowly pressure 3-1/2" tbg. to 2500 psi. Wait 5 min. (Noticed packer
stroke at 1500 psi- Noticed a 1500 psi drop in pressure at 2500 psi,
indication of PBR shear), Bleed pressure to zero. Slack off to 40K (20K
on pkr) to comfirm packer set, then pressure tbg string to 3000 psi and
hold 5 min. Bleed pressure to 1500 psi. PU to 65K (PBR sheared),
Slack back off to 40K. Check & tag locator wI 15K. Pressure tbg to
3600 psi to shear ball seat- Very good indication of ball sheared. Bleed
off all pressure.
Pull jt. #252 out of hole (pulling 12' out of PBR). M/U 3-1/2" FMC tubing
hanger on jt. # 251, Use jt. #252 as landing jt., RIH & land tubing wI

7" annulus. Installed 2 way check valve.
PJSM Nippled down BOP.
Nippled up and tested tree 250/5000 PSI. Pumped 60 Bbls. Diesel in 9
5/8" X 7" for freeze protection.
Pulled 2 way check valve and installed mousehole.
Pumped 120 Bbls. diesel freeze protection with Little Red while laying
down DP.
Laid down excess drill pipe from derrick.
Rigged down Little Red, installed back pressure valve and secured well.
Finish Laying down excess drill pipe from derrick.
Slip and cut 74' drilling line, serviced rig and top drive.

ConocoPhillips Alaska, Inc. hereby files this Application for Sundry Approval to plug back well 3S-06. A
permit application for the sidetrack, 3S-06A, will be submitted by 5/13/03.
Attached are the plug back procedure and a well bore schematic. ConocoPhillips hereby requests a
waiver of the tagging requirement on the lower abandonment plug. The plug will be set from the bottom
of the hole and an additional 100' of cement will be placed above the Kuparuk "C" sand to ensure
isolation.

38-06 Plug Back Procedure
1. Contact inspector to witness job.
2. PU +1- 947' cement stinger wI slotted flow diverting joint on bottom.
3. RIH with stinger and 5" DP to TD.
4. Circulate and condition mud to obtain consistent mud weight in and out.
Plug # 1
5. Spot balanced cement plug to cover the Kuparuk "c" sand interval. (see cement
calculations) Reciprocate pipe until cement reaches the end of the plugback string.
Spot plug at 8-10 BPM.
6. Pull stinger and DP out of plug no faster than 1 stand 12 minutes. Pull out of hole to
+1- 8200' MD and pump 25 bbls at maximum rate to clear the drill pipe.
7. POOH to 7600' MD and spot a weighted (1 ppg over mud weight) high viscosity pill
from 7600' to 7300' MD.
Plug # 2
8. POOH to 7300' MD I circulate and condition mud to spot sidetrack plug. Reduce the
Yp to 15-20. Balance mud weight in and out.
9. Spot balanced cement plug from 7300' MD to 6700' (see cement calculations).
Reciprocate pipe until cement reaches the end of the plugback string. Spot plug at 8-
10 BPM.
10. Pull stinger and DP out of plug to 6600' MD no faster than 1 stand 13 minutes while
pulling out of cement.
11. Circulate up spacers and contaminated mud/cement the long way at maximum rate.
12. POOH and UD cement stinger.
13. Test BOP equipment.
14. P/U directional assembly to dress off cement and sidetrack well. Dowell will run a
compressive strength test on the cement blend. Directional plan will be provided as
soon as possible.

current BHL on 3S-06. The planned kick-off point will be approximately
7000' MD.

Each well must be Identified by a unique name designated by thi:? operator and a unique API number assigned by the commission
under 20 AAC 25.040(b). For a well with multiple well branche~ each branch must similarly be Identified by a unique name and API
number by adding a suffix to the name designated for the well by the operator and to the number assigned to the well by the
commission.

A pressure build-up test performed on Palm lA (35-26) in 4/01 indicated a reservoir pressure in the
Kuparuk C sand of 3597 psi at 5830' tvd, 0.62 psi/ft or 11.9 ppg EMW (equivalent mud weight).
The maximum potential surface pressure (MP5P) while drilling 35-06 is calculated using above maximum
reservoir pressure, a gas gradient of 0.11 psi/ft, and the Kuparuk C sand target at 5871' tvd:
MP5P =(5871 ft)*(0.62 - 0.11 psi/ft)
= 2994 psi

exception to 20 AAC 25.412 (b) is requested. The distance between seals at the top of the 3 1/2" liner
and the top perforation will exceed 200' MD. In the top-set case, the distance between the seals and the
top perforation will be ""400' MD. An adequate leak-off test of the 7" casing shoe, combined with a
cement bond log of the liner section below the 7" casing shoe will provide proof of zonal isolation
between the seals and the perforated interval.

13. Proposed Drilling Program

The proposed drilling program is listed below. Please refer as well to Attachment 3, Well Schematic.
1. MIRU Nabors 7ES.
2. Install riser (no diverter).
3. Drill 12-1/4" hole to 9-5/8" surface casing point according to directional plan. No LWD logging
equipment required.
4. Run and cement 9-5/8" 40#, L-80 BTC casing string to surface. Employ lightweight permafrost
cement lead slurry and LiteCRETE tail slurry. Pump adequate excess to ensure cement reaches
the surface. If cement does not circulate to surface, perform top job. Install and test BOPE to
5000 psi. Test casing to 2500 psi.
5. Drill out cement and 20' of new hole. Perform LOT.
6. Drill 8-1/2" hole to total depth according to directional plan. Run LWD logging equipment with
GR, resistivity, and density-neutron. Run 7" 26# L-80 BTCM casing to surface and cement.
Note: If significant hydrocarbon zones are present above the Kuparuk C sand, sufficient cement
will be pumped for isolation in accordance with 20 MC 25.030(d)(3)(C). A cement bond log will
be performed prior to perforating the well.
7. Run 3-1/2" 9.3#, L-80 8rd EUE 8rd-Mod completion assembly w/ SAB-3 permanent packer and
PBR. Set packer 350' above top planned perforation, shear out of PBR, P/U tubing hanger, and
land tubing. Pressure test tubing to 3500 psi. Pressure test 3 V2" x 7" annulus to 3500 psi.
Burst shear valve for freeze protect.
8. Set TWC and ND BOPE. Install and pressure test production tree.
9. Freeze protect. Secure well. Rig down and move out.
10. Handover well to Operations Personnel.
Note: If the 7" 26# L -80 BTCM casing must be top-set above the Kuparuk C sand, 6 1/8" hole will be
drilled to total depth according to the directional plan and a 3 112" 9.3# L-80 SLHT liner will be ran and
cemented across the productive interval. The cement bond log will be done after moving the rig off the
well.

operator intends to reque!:JT authorization under 20 AAC 25.080 for an annular disposal operation in the well.¡,
Waste fluids generated during the drilling process will be disposed of down a permitted annulus on 35
pad or hauled to a KRU Class II disposal well. All cuttings will be disposed of either down a permitted
annulus on 35 pad or hauled to the Prudhoe Bay Field for temporary storage and eventual processing for
injection down an approved disposal well.
At the end of drilling operations, an application may be submitted to permit 35-06 for annular pumping of
fluids occurring as a result of future drilling operations on 35 pad.
