Well was opened up and observed to be static for 30 min. When the BHA was picked and lowered into the well for makeup, there was a burp of diesel that came out of the well. BOPE Used and Reason: Blind rams and choke HCR. Current BOPE test time/date Initial BOP test on well was completed on 4/28/15. Is a BOPE re-test required after use: Yes/No and reason Yes - BOPE Retest time/date and result (WSL to complete as applicable) BOPs were re-tested prior to the following run. Plan Forward: Freeze protect fluid was circulated out from bottom with a nozzle with no other issues. Additional Comments: ACTIONS REQUIRED: 1) Well Site Leader: Timely email notification of BOPE re-test time estimate to AOGCC North Slope Inspectors is required following any WC incident to allow the State to witness re-test as required. (This is a judgment call given WC is an unplanned event during rig operations and factors such as wellbore condition and trip timing will influence timing of any retest) 2) Superintendents: Email the completed BOPE Usage Report form to AOGCC North Slope Inspectors within 24 hours of a WC incident: doa.aogcc.prudhoe.bay@alaska.gov 3) Well Site Leader: Complete AOGCC BOPE Test report form 10-424 and send to three AOGCC addresses (noted in form) within 5 days following the test/retest. History of F-23A: F-23A is a CTD ST drilled in 1999 targeting Zone 2/1 sands. It was completed with 184' of perforations. A tubing leak was repaired with a 30' x 5-1/2" patch. ' Proposed plan for F-23B: Drill a through tubing sidetrack with Coiled Tubing Drilling rig on or around April 15th, 2015 with Nordic 2. The F-23B sidetrack will exit the existing 5-1/2" liner and kick off in the Shublik and target Zone 2 reserves. The proposed sidetrack is a -2640' horizontal Ivishak Zone 2 producer and will be completed with a 3-1/2" x 3-1/4" L-80 solid liner, cemented in place. This Sundry covers plugging the existing F-23B perforations via the F-23B liner cement job or liner top packer, if the liner lap pressure test fails (alternate plug placement per 20 AAC 25.112(i)). The following describes the work planned and is submitted as an attachment to Form 10-403. A schematic diagram of the current well and proposed sidetrack is attached for reference. I 1 I I f Sundry Application Plug for Re-drill Wells Work Flow This process is used to identify wells that are suspended for a very short time prior to being re-drilled. Those wells that are not re-drilled immediately are identified every 6 months and assigned a current status of"Suspended." Step Task Responsible 1 The initial reviewer will check to ensure that the "Plug for Redrill" box in the Patricia or Steve upper left corner of Form 10-403 is checked. If the "Abandon" or "Suspend" boxes are also checked, cross out that erroneous entry and initial it on both copies of the Form 10-403 (AOGCC's and operator's copies). 2 If the "Abandon" box is checked in Box 15 (Well Status after proposed Patricia or Steve work)the initial reviewer will cross out that erroneous entry and initial it on both copies of the Form 10-403 (AOGCC's and operator's copies). Instead, then check the "Suspended" box and initial that change. Guy or Victoria The drilling engineers will serve as quality control for steps 1 and 2. (QC) 3 When the Stat Tech receives a Form 10-403 with a check in the "Plug for Jenni Redrill" box, she will enter the Typ_Work code "IPBRD" into the History tab for the well in RBDMS. This code automatically generates a comment in the well history that states"Intent: Plug for Redrill." 4 When the Stat Tech receives any Form 10-407, she will check the History tab Jenni in RBDMS for the "Intent: Plug for Redrill" code (IPBRD). If present and there is no evidence that a subsequent re-drill has been COMPLETED, the Stat Tech will assign a status of SUSPENDED to the well bore in RBDMS no matter what status is listed on the 10-407. The Stat Tech will also change the status on the 10-407 form to SUSPENDED, and date and initial this change. If the Stat Tech does not see the "Intent: Plug for Redrill" comment or code, she will enter the status listed on the Form 10-407 into RBDMS. 5 When the Form 10-407 for the redrill is received, the Stat Tech will change Jenni the original well's status from SUSPENDED to ABANDONED. 6 The first week of every January and July, the Stat Tech and one of the Jenni Geologists will check the "Well by Type Work Outstanding" user query in Patricia or Steve RBDMS to ensure that all of the Plug for Redrill wells have been handled properly. At this same time, they will also review the list of suspended wells for accuracy. OF 6,411/%/ s THE STATE A 'a:31 a C , Etillr . It was completed with 184' of Z1 B perfs and came online at over 1500 bopd and 3000 GOR. The well developed TxIA communication in the summer of 2005 after the operator called it in for high IA pressure. The subsequent July 2005 caliper showed the tubing to be in good condition. The caliper however shows the tubing tail and 5-1/2" liner to be in poor condition (> 50% penetration from 9779' and and deeper). The 12/02/06 CMITxIA failed at 3000 psi with a LLR of 0.25 bpm. The January 2007 LDL found a leak at 8567' md. A passing MITIA to 2500 psi on 5/18/07 restored F-23A's integrity after the leak had been patched with a 30' x 5-1/2"Weatherford Eclipse Patch that was set between 8562'-8592' SLM. The Pad Operator called in the well for high IAP on 6/14/08. The well then failed an MITIA to 2500 psi on 6/23/08 with a LLR of 0.75 gal/min. A 8/8/08 Tecwel log detected possible casing leaks at these locations: 6636' MD, 6790' MD, and 7210' MD. To remediate the production casing leaks, a RWO is required. Scope of Work: Pull 5-1/2" L-80 completion from cut, RIH with RTTS and test 9-5/8" casing to 3,000 psi from 3,300' MD to surface, Run short 7" scab liner, Run and fully cement long 7"scab liner, Drill out shoe track, test casing, perform cleanout runs, pull 7" RBP, Run new 4-1/2" 13Cr-85 gas lift completion. • Pre Rig Prep Work: S 1 Pull 2-7/8" WRP from 10,200'. Set composite bridge plug at-10,157' (3' below the top of the first full joint below the deployment sub). Pull 3-1/2" nipple reducer at 9,732'. S 2 Pull DWP patch from 8560'. Do not spend longer than 3 days fishing patch. If patch does not pull with Slickline do not leave patch baited. We will not rig up coil tubing to pull patch. If patch does not pull call RWO engineer for plan forward. Drift for RCT cut. F 3 CMIT-TxIA to 3,000 psi, MIT-OA to 2,000 psi. ✓ 4 Check the tubing hanger lock down screws for free movement. Completely back out and check each lock down screw for breakage (check one at a time). PPPOT-T& IC. E 5 Use RCT to cut tubing at-9,584' MD (-2' below the top of the first full joint above the TIW packer). F 6 Circulate well through the cut with 9.8 ppg brine and diesel freeze protect. ✓ 7 Prep annular valves, set a BPV ✓ 8 Remove the wellhouse&flow line. Level the pad as needed for the rig. Proposed RWO Procedure: 1. MIRU Doyon 16. ND tree. NU and test BOPE with four preventers configured top down (annular, 7" fixed, blind shears, mud cross and 3-1/2"x 6"VBR's). Test the VBR's and annular to 3,500 psi. 2. Pull and lay down 5-1/2"tubing string to the jet cut. 3. RIH with RTTS and test 9-5/8" casing to 3,000 psi from 3,300' MD to surface. 4. Run short 7" scab liner(install RBP w/liner set in rig slips, stack 50' of sand on RBP) and set on 5-1/2" stub. 5. Run and fully cement 7", 26#, L-80 VamTop HC scab liner with liner top @ - 3,200' MD. 6. Drill out shoe track and test casing to 3,000 psi. 7. Perform cleanout run, pull 7" RBP. 8. Run new 4-1/2" 13Cr-85 gas lift completion with 7"x 4-1/2" HES TNT packer. MIT-T& MIT-IA to 3000 psi. Freeze protect to 2,200' MD. 9. Set a BPV. ND BOPE. NU and test tree. RDMO. Post rig Work ✓ 1 Pull BPV S 2 Pull ball and Rod, Pull RHC seat, Run GL Design. C 3 Drill out composite bridge plug. O 4 Hook up flowline and AL lines, set wellhouse, etc. and POP well. I. Supervisor 'b~-~ ,~ THRU: December 7, 2000 FROM: Jeff Jones, SUBJECT: Petroleum Inspector Safety Valve Tests Prudhoe Bay Unit BPX / F-Pad Thursday, December 7, 2000; I traveled to BPX's F-Pad in the Prudhoe Bay Field to witness SafetY Valve System tests. Merv Liddelow, BPX representative in charge of testing, performed the tests in a safe and proficient manner. Of the 28 wells tested, 10 failures were witnessed. For the most part, frozen contaminants (.w. ater) in the block valves upstream of the pilots and in the SSV hydraulic control lines caused the failures. On 12/8/00 Mr. Liddelow indicated tO me th~at ail SVS failUres had been correcte~d a..nd successfully re-tested. The 17.8% SVS failure rate requires a 90-day test cycle for this pad. The AOGCC test report is attached for referenCe. I conducted 28 well house inspections concurrent With the SVS tests.. Due to recent high winds, several well houses were drifted with snow inside. In addition, SVS hydraulic fluid leaks were noted on wells F-12 and F-13A. Mr. Liddelow . indicated that these items would be addressed. No other exceptions were noted. SUMMARY: I witnessed SVS tests and conducted well house inspections on BPX's F-Pad in the Prudhoe Bay Field. 17.8% failure rate = 90 day test cycle. 28 wells, 56 components tested. 10 failures. 17.8% failure rate. 28 well house inspections cond[~cted. A tubing integrity test was performed, the tubing was calipered and the well was drifted. Service coil then rigged up and set a cast iron bridge plug at 11,050' MD, 50 feet above the top perforation, per the prep plans described in the Sundry notice (399-133). After setting the bridge plug the well failed a pressure test. Service coil was then used to mn production logs to identify the leak. Spinner and Temperature logs have identified two leaks in the 5 Va" liner at 10,294~ and 10,372'. Service coil was then used to set a whipstock at 11,000.' MD. /. These leaks are not considered to be a problem with respect to drilling the proposed sidetrack. The sidetrack completion will be a fully cemented 2 7/8" liner that will be e,_xtended_t_o_at least 200 ft above the u~.l_iner leak. F- 23 is a horizontal well with the sid~ at approx. 75 degree hole inclination. The target for F-23a, the 14P, is the same zone originally completed in F-23. We ate requesting approv,-tl to proceed with the sidetrack of F-23 without a pre_ssure test of the CI~_~P as originally planned. The extensive cemented 2 7/8" hner lap, along with the CIBP, will effect the abandonment of original perforations in F-23. A schematic diagram of the proposed F23a sidetrack completion is attached.
