John, Dan:
In the haze of late Friday night, I forgot to forward this to you all. At 4:40 PM on Friday, I got a call
at home from Tom saying Forest needed immediate approval (the rig was waiting on orders) to test a
sand interval that lies below the 9400-foot sand in the West Foreland 2 well. Forest emailed the logs
for West Foreland 1 and West Foreland 2 to my house for purposes of discussion. I independently
verified Forest's correlations and depth picks using the logs provided by Forest.
The 9400-foot sand is not developed in West Foreland 2, but Forest saw potential for gas in the next
sand lobe below the 9400-foot sand. As you recall, Forest owns 100% working interest in the state
lease within which West Foreland 2 was drilled. According to BLM's land administration case files,
Forest owns 100% interest in this stratigraphic interval in the adjacent Federal Lease A-035017 to the
stratigraphic equivalent of 9,624' measured depth on the West Foreland 1 Induction Electric log.
Below that depth, Forest owns 60% interest, and ConocoPhillps owns 40%. The lowest perforation in
Forest's potential West Foreland 2 test is 10,760' MD, which is 60 vertical feet above the base of
Forest's 100% working and operating interest in the federal lease, and it will be separated and sealed
from the base of their working interest in the federal lease by 25 vertical feet of mudstone and
siltstone and 10 vertical feet of coal. ConocoPhillips correlative rights in the deeper portion of the
federal lease will be protected.
I gave Forest approval to test the zone. I also required Forest to notify ConocoPhillips of the well
location prior to initiating regular production.
My apologies for not forwarding this more quickly,
Steve Davies

Jim,
Based on information presented to me via email and telephone on the evening
of November 19, 2004, I concur with Forest's opinion that the deepest
desired test interval in the West Foreland #2 well ("WF #2"), from 10,725'
to 10,760' measured depth ("MD"), -9,354' to -9,374' true vertical feet
subsea ("tvdss"), lies within the stratigraphic interval in which Forest has
a 100% working and operating interest in adjacent Federal Lease A-035017.
BLM's land administration case file for that lease states that Forest has
100% working and operating interest ownership from the surface to the
stratigraphic equivalent of 9,624' MD on the Induction Electrical log
("IEL") from the West Foreland No.1 well ("WF #1") . I also concur with
Forest's opinion that 9,624' MD on the WF #1 IEL correlates with 10,862' MD
(-9,434' tvdss) in WF #2.
Forest's proposed bottom perforation at 10,760' MD in WF #2 will be 60
vertical feet above the base of Forest's 100% working and operating interest
in adjacent Federal Lease A-035017. An interval of 25 vertical feet of
mudstone and siltstone and 10 vertical feet of coal will lie between the
bottom perforation and the base of Forest's 100% working and operating
interest.
Forest may perforate and test the interval from 10,725' to 10,760' MD in WF
#2. However, pursuant to the Commission's request sent to you via email
approximately four days ago, Forest must also notify ConocoPhillips Alaska,
Inc. of the location of the well prior to initiating regular production.
Please call me at 349-1191 or 793-1224 if you have any questions.
Sincerely,
Steve Davies

Amoco Production Company - Drilling Waste Reserve Pits
Final Closure Approval of 25 Inactive Facilities
The Alaska Deeartment of Environmentai Conse~'ation (ADEC) has evaluated your
co~,.~once.,c~, dated Februap,' 19. , 99~. requesting closure of 29 .~-noco Production Company
(.-' ,oco l inactive rese~:e pit disposal facilities. Surface evaluations were performed at the sites
in !989 and !990. Txis corresvondence describes our review procedures, puts forward the
Devar=ment's findings, and takes action upon the closure request.
The 29 facilities were assessed by .Zanoco prior to the issuance of the ADEC Interim Site
.-Xssessrnent Guidance .for Inactive Drilling Waste Sites. June 1o01. The new Alaska Solid Waste
Reguiations. 18 AAC 60, became effective January. 28. 1996 and contains a chapter on inactive
reserve pit closures ( t 8 AAC 60.440). The closure criteria in the new regulations parallel the
closure criteria in the 1991 guidance document, and the review procedures require a discussion of
the risk screening assessment as required in the 1991 document. Briefly, the significance of risk
to local receptors is evaluated through srudv of contaminant release, exposure criteria, and
conse=uence criteria.
The 1989-1990 Amoco assessment program included acquisition of numerous (I 0-30)
shallow cores within and adjacent to each drill pad. sampling of the cored material at five-foot
inter,,'ais, and instailation of monitorin_o wells on several facilities from which shallow
ground~vater ,,vas encountered while coring, Total petroleum hydrocarbon I,.TPH). BTEX. and

Napat~ Creek No. 1
Concern is expressed by ~e U,S. Fish and Wildlife Service over surface conditions at
Napamk Creek No. 1. which appear to be impac:ed by abandoned drilling-related trash and
equipment. Of particular concern are referinces in the original assessment document to
drams, batteries, and sacks of dr/llin~ mud andJor mud additives. Recent documentation

Tvonek State No. 1
Conce:-., is expressed by the Contaminated Sites Section of A.DEC over potential
groundwater contamination fi'om buried reserve pit wastes at the Tyonek State No. 1
location· esr~ecially with fre=uent summer habitation of the site by Tyonek village residents.
Evidence from the 1990 monitoring well data indicate substantial water quality, exceedances
with resr~ecr to c~omium, lead. barium. TPH. and the BTEX components. Subsequent
anaivses from samples collected in 1991 and 1993 indicate the BTEX concentrations are
diminishing: however, metal analyses were not reported and TPH concentrations appear to
increase significantlv in 1993. Final closure approval of the reserve pit may be granted
under l 8 AAC 60.4400)(5) 'if the owner or operator demonstrates that water pollution in
the affected water body is diminishing~. Therefore, it is requested that Amoco re-sample
the two monitoring wells to verifv that BTEX. DRO. and metal concentrations are
diminishing. Accurate analyses of the metal and hydrocarbon (DRO and BTEX)

::rom ponds on ',.he nor& and south ends of the reserve pit and at baclcground and down-
=.ame,,t locations should be sampled. The analytical data. an appropriate Qualitative risk
assessment, and analysis of feasible corrective action as outlined in 18 AAC 60. !.*..1,0(f)(7-10)
are required prior to closure.
Chuima River State
Further information is required to reach a ffmal decision on the closure status of the Chuitna
,River State location. Hizh concentrations of BTEX (Level A and B soil exceedances),
~, H. and several met. als in the rese,-we pit 4rilling waste exist at or within ~ i0 ff depths
and may contact the large pond of standing water. Groundwater from monitoring wells
around the nit identified variable ieveis of pe=oieum hydrocarbons. Large concentrations
,or' TPH at station MW28 (TPH=58 rog/t) and MW9 ~TPH= !0 rog,,15 were oboe,'ed in the
' 992 data. The down-gradient occurrence at 3,fW9 suggests that TPH-deriveci orgamcs may
~e migrating outside the pit area. Groundwater metal analyses were not included. Surface
· .rater malvses from the t993 program identified trace levels of the BTEX components:
elevated levels of barium and lead (Pb=25 ppb) were detected in a 1989 surface sample.
T'~e Denamment requests that further sampling and analyses of the resuired metal and
hydrocarbon components listed in Table D (18 AAC 60.440) be performed at the site,
utilizing analytical methodolo~ specified therein. At least one surt'ace water sample from
location SW4, and three groundwater samples from M-W9, MWl7, and MW28 are
resuested. The Department recommends that you bail/purge the monitoring wells prior to
sampling and use a field-filtration method to eliminate particulates from the groundwater
~i.e. measure only the dissolved metal concentrations). Final closure approval of the site
should ,then be possible under I8 .~4C 60.440(j)(5) if the contaminant levels are indeed
diminishing.
final closure approval will be granted after it is demonstrated to the ADEC that drilling
wastes within :he pits are ,".ot activetv contaminating Surface water or groundwater at the sites.

?!e~e r)rovide the supplementaQ' information to this office. Contamination at the Tyonek State
No. 1 site. and susr~ected pad contamination bv historic spills at the Foggy Island Bay State No. 1
,_.nd the Cathedral River sites have been retorted to the ADEC Contaminated Sites Section for
review, as required under Anicie 1 of the :bllowing "Terms and Conditions" section. In
addition. ADNR reserves the fight to furmer evaluate both pad and reserve pit of each site at the
:ime of full lease :
c,osure.
TERMS AND CONDITIONS
All of the above closure approvals are subject to the following terms and conditions:
1) Reserve Pit Facility. Closure: Cae approval =m-anted by ti'tis letter is for the inactive drilling
waste rese.we pit faciti~ only, as reauired under 18 .~.C 60.440. Closure for the pad as a
whole must be coordinated be~veen the owner/or~emtor and the appropriate state (ADNR.
.,DF&G. AOGCC), or federal (BLM') agency. Additionally, should any contamination from
hJstonc spills be found on the pad outside the cMiling waste site (reserve pit area),
notification needs to be made to the .4.DEC Contaminated Sites Section (269-7659).
23 New Information: Should additional information conce, mmg environmental conditions at
the thcilitv make further actions necessary, to protect human health or the environment, the
Deuarrment reset'es the fight :o reauire additional investigation, assessmenu monitoring, or
remediation at the facility.

SUBJECT: Pan American West Foreland
Unit ti2
On Thursday, August 18, 1966, I traveled to the subject location with Mr
VonderAhe and Mr. Marshall to inspect the general housekeeping arrangement and
blowout equipment. We traveled by Pan American charter helicopter from Anchorage
to location and return. ~
At the location, we found the housekeeping in good order with equipment
clean and painted. Mr. Shakleford, the Pan American Drilling Engineer, guided
us around the drilling location explaining various houkups and etc. During the
course of the tour, we requested that the BOP's be tested as the crew had just
completed pull&hq ~t a worn bit. The Hydril bag preventer was tested with a Joint
of pipe in the hole and the double ram preventers were tested with pipe out of the
hole. A flashlight was used to observe closing of the rams. All prevention
The
equipment and hookups operated satisfactorily, g double ra~reventers w~re of .
the ~gtt[ hydraulic closing type and closed in approximately 10 seconds.j The
only thing found wrong at thiS'location was the absence of the well sign and this
was called to the operatOr's attention.
