Alaska.
Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
TPI (Top of Producing Interval).
The Kelly Bushing elevation in feet above mean Iow Iow water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
The APl number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
True vertical thickness.
Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the
cementing tool.
If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in

Re: P&A operations for well F-26A in prep for a CTD sidetrack
Well F-26A has just come onto the CTD schedule and is needed to keep Nordic 1 operational. Problems have
been experienced preparing 18-14a & 2-37 and alternatives on Z & W pad are inaccessible because of the /
Kuparak river bridge outage.
F-26B will be a tubing sidetrack performed by SLB/Nordic 1 CTD rig. Off of a.mechanical whipstock the
sidetrack will exit 5½"liner into the Shublik formation. From here it will build & turn towards a Zone 2A landing f'
point. Therea~r a horizontal lateral secion will target the Z2A light oil column, (LOC) reserves. This is the 2na
visit by Nordic 1 to sidetrack this well. The existing perforations in the 27A'' liner, (run in Oct 98) will be P&A'd f
with a CIBP. The top uncemented section of this liner will then be cut and recovered to surface, exposing the
original 5½" liner in which the WS will be set. The following describes work planned and is submitted as an f
attachment to Form 1.0-403. A schematic diagram of the proposed sidetrack is attached for reference.
Pre CTD Summary 1) Slickline -Drift 2Y8" liner to 10,100' - below CIBP setting.
2) Electric line - Set CIBP at 10,000' md in the 27A'' liner. PT wellbore to 1000 psi. Wellbore f
abandonment is complete.
3) Electric line - Chemical cut 2V8" liner at 9910' which is free pipe - TOC from USIT = 9920'. f

Yesterday we found ourselves without a viable next well to go to with Nordic1. We have been unable to prepare wells 18-14a and
02-37 due to mechanical restrictions in the wellbore. We are also unable to go to Z and W wells because of the Kuparak bddge
outage.
Today we've been working hard on F-26A. This well is nearly identical to F-10A, the well Nordic 1 is currently on. F-26 was sidetracked
back in Oct 98 with Nordic 1 & in order to sidetrack it again we will be plugging & cutting the 2-7/8" liner and setting a whipstock in
the uncovered 5-1/2" liner section.
These operations require both a 10-403 sundry approval for the P&A and a 10-401 permit to drill for the sidetrack. Both of
these documents are close to being finalized and sent to you. The sundry document will be required asap, hopefully by Fri 13th in
order to plug and abandon the well. A verbal on the sundry would be ideal. The permit to drill will realistically not be needed until Mon
16th as we have at least 2 days of decomplete work to do on the well with the rig.
What I'm basically asking is that these 2 permits be reviewed as soon as you receive them in order to keep Nordic 1 operational. Your
assistance in this matter would be greatly appreciated.

Thursday, December 7~ 2000: I traveled to BPX's F-Pad in the Prudhoe Bay'
Field to witness Safety Valve System tests.
Merv Liddelow, BPX representative in charge of testing,'performed the teSts in a
safe and proficient manner. Of the 28 wells tested, 10 failureS were witnessed.
For the most part, frozen contaminants (water) in the bloCk valves upstream of.
the pilots and in the SSV hydraulic control lines caused the failureS. On 12/8/00
Mt: Liddelow indicated to me that ail SVS failures had been corrected and
successfully re-tested. The 17.~%' SVS failure rate requires a 90-da~;~test Cycle
for this pad. The AOGCC test report is attached for referenCe.
I conducted 28 well house inspections concurrent with the SVS tests.. Due to
recent high winds, several well houses were drifted with snow inside. In addition,
S¥S hydraulic fluid leaks were noted on wells F-12 and F-13A. Mr. Liddelow
indicated that these items would be addressed. No other exceptions were noted.
SUMMARY: I witnessed SVS tests and conducted well hOuse inspections on
BPX's F-Pad in the Prudhoe Bay Field. 17.8% failure rate = 90 daY test cycle.
28 wells, 56 components tested. 10 failures, 17,8% failure rate.

The permit to redrill does not exempt you from obtaining additional permits required by law from
other governmental agencies, and docs not authorize conducting drilling operations until all other
required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
bv contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

F-26A Sidetrack
Summary of Operations:
F-26 is a poor cycle performer and is currently shut in for high GOR. The sidetrack, F-26a, will
target Zone 2B reserves.
This sidetrack will be conducted in two phases.
Phase 1: Set whipstock and downsqueeze abandonment cement: Planned for
9~23~98. "
A mechanical whipstock will be set at 10,314'.
Existing perforations will be abandoned with approx. 20 bbls cement. Abandonment cement
will be downsqueezed past the mechanical whipstock. Abandonment cement will be
pressure tested to 1500 psi with AOGCC witness. This. procedure is described in more
detail in the Form 10-403.
Phase 2: Mill Window and Drill Directional Hole: Planned for 1012198.
Directional drilling coil tubing equipment will be rigged up and utilized to mill the
window and drill the directional hole as per attached plan. (In the event that service
coil, rather than drilling coil, is used to mill the window, a flowline will be in service
and AOGCC will be notified to witness a full test of the service coil BOP equipment.)
Mud Program: - Phase 1: Seawater
- Phase 2: FIo-Pro (8.8 - 8.9 ppg)
Casing Program:
- 2 7/8", 6.16#, L-80, ST-L liner will be run from TD to approx. 9,560' MD and cemented with
approx. 20 bbls. cement. The well will then be perforated with coil tubing conveyed guns.
Well Control: - BOP diagram is attached.
- Pipe rams, blind rams and the CT pack off will be pressure tested to 400 psi and to 4000 psi.
- The annular preventer will be tested to 400 psi and 2000 psi.
Directional - See attachments.
- Kick Off: 10,325' MD (8,857' TVD ss)
- TD: 12,160' MD (8,960' TVD ss)
- Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging · A Gamma Ray log will be run over all of the open hole section.
· Memory CNL will be run in the 2 7/8" liner.
