Box 100360 Anchorage, AK 99510-0360 Subject: Defeated SVS at KRU 2B-O1 ~~`~~~ ~``J'~ ~ ~ ?f~j'C Kuparuk River Oil Pool Dear Mr. Dubuisson: The Alaska Oil and Gas Conservation Commission (Commission) has received and reviewed the explanation from ConocoPhillips Alaska, Inc. (CPAI) regarding the defeated safety valve system on Kuparuk River Unit ZB-Ol. That explanation was in response to the Commission's letter dated May 21, 2010 and provides an overview of corrective actions that have been implemented to prevent this scenario from repeating, including reiterating flagging/tagging requirements from the Alaska Safety Handbook and instituting a procedure. CPAI's response specifically addresses corrective actions that are relevant to this violation. One area of concern from the findings presented by CPAI is the timing of preventive maintenance activities at KRU 2B (May 6, 2010) relating to the performance of safety valve testing (May 7). The scope of preventive maintenance was not fully described by CPAI, but we understand it to include activities such as actuating valves and pilots, and greasing components within the safety valve system. Such activities have the potential to influence test results, leading us to question the accuracy of the test and motive for performing preventive maintenance activities so close to the scheduled safety valve system tests. The Commission does not intend to pursue enforcement action regarding the KRU 2B-O1 defeated safety valve system. However, the Commission does not accept the proximity of safety valve system preventive maintenance to scheduled testing. CPAI is instructed to restrict scheduled preventive maintenance activities to no closer than 30 days to the time safety valve system tests will be performed on a pad. If available, a tape containing the digitized log information shall be submitted on all logs for copying except experimev, ta! logs, velocity surveys and dipmeter surveys. Many rivers in Alaska and their drainage systems have been classified as important for the spavming or migration of anadromous fish. Operations in these areas are subject to AS 16.05.870 and the regulations promulgated thereunder (Title 5, Alaska Administrative Code). Prior to commencing operations you may be contacted by the Habitat Coordinator's Office, Department of Fish and Game.
