As provided in AS 31.05.080, within 20 days after written notice of this
decision, or such further time as the Commission grants for good cause
shown, a person affected by it may file with the Commission an application
for rehearing. A request for rehearing is considered timely if it is received by
4:30 PM on tYie 23rd day following the date of this letter, or the next working
day if the 23rd day falls on a holiday or weekend. A person may not appeal
a Commission decision to Superior Court unless rehearing has been
requested.

Re: G-31 A Sundry Approval

Sundry approval is requested for well G-31A. G-31A was sidetracked in May 1998 as a Zone
1 horizontal well. Approximately 1000' was drilled into Zone 1B with the last 500' being very
poor quality. The well was kicked off with N2 after the rig perforated 300' of poor quality
Z1B rock, then kinley buttons were set uphole to provide gas lift. Shortly after, two schmoo-
be-gone (SBG) treatments were pumped into the well followed by a diesel/xylene wash to
clean up suspected Moveable Heavy Oil. In 1999, 60 feet of adperfs were shot in clean sand
which made a well out of it. Another 75' of adperfs were shot in Mar-2002, making the well
more competitive.
The well produced until Dec 2002, when it was shut-in for more then two years until a patch
was placed over the leaking kinley buttons. The well came back making high GOR oil and
started cycling about a year later.
In September 2008, 20 feet of end-of-life adperfs were shot in the heel. The well showed to have
improved slightly in the first cycle, but did not make a drastic improvement. There are no further
options for this well.
REASON FOR SIDETRACK
Given the high GOR G-31 A is a good candidate for a coil sidetrack.

The proposed plan for G-31A is to drill a through tubing sidetrack with the SLB/Nordic 1 Coiled ~~~~
Tubing Drilling rig on or around May 1, 2009. The existing perforations will be isolated by a CIBP, as ~„ oL
well as the primary cement job. The sidetrack, G-31B, will exit the existing 7" liner after the 2-7/8" is
cut and pulled. The following describes work planned and is submitted as an attachment to Form 10-
403. Aschematic diagram of the proposed sidetrack is attached for reference.

Tue.sd.ay, April 24, 2001: I traveled to BPXs G Pad and witnessed the one-month
retest of the safety valve systems.
As the attached AOGCC Safety Valve System Test Report indicates ! witnessed the
testing of 16 wells and 32 components with no failures. The surface safety valve, on
well G-03A closed rather slowly but passed its pressure test. The wing valve on
wells G-16 and G-18A had very slow leaks but did not prohibit a valid test~of the
surface safety valve. Well G-21 which~.failed the. previous months test. due.~o-a
frozen pilot, has the new style insulation box in place and functioned properly.
Merv Liddelow performed the testing today; he demonstrated good test procedures
and was a pleasure to work with.
A walk through inspection of the well houses on this pad was performed during the
SVS testing with no problems noted. Well G-t4A which was shut in at this time and
not tested was found to have the new style insulation box on the pilot. This should
fix the problems found on previous tests.
Summary: I witnessed the one-month retest of the safety valve systems at BPXs
G-Pad.

March .26, 2.0.01; I traveled to BPX's Prudhoe Bay Field, G Pad, to witness
Surface Safety Valve System tests and perform well house inspections.
Merv Liddelow was the BPX representative in charge of testing and he
performed the tests in a safe and proficient manner. Fifteen wells were tested
with nine failures witnessed. The pilots on wells G-02A, G-11^, G-14A and G-
21A failed to operate properly. The SSV's on wells G-01A, G-02A, G-03A, G-09
and G-13 failed to close or failed to hold D/P. Wells G-03 and G-09 were shut in
pending repairs. Mr. Liddelow indicated to me that ali other failures had been
repaired and passed a retest within 24 hours.
Fifteen well houses were inspected and slight crude oil leaks were noted on
wells G-14 and G-16. Mr. Liddelow and his crew repaired these leaks
immediately. The AOGCC test reports are attached for reference.
SUMMARY: I witnessed SVS tests on 15 oil producing wells on BPX's G Pad in
the Prudhoe Bay Field.
15 wells, 30 components tested. 9 failures: 30% failure rate.

Working to get bit past window. No Luck. Wait on Vac trucks to swap mud. Dropped ball to
CS and circulate FIo-Pro around. POOH. LD Mill BHA. Wait on Baker to dress formation Mill.
PU Milling BHA and TIH. Dry tagged at 8889' Started milling again at 8888'. Some motor
work at 8890' Milled to 8893.8' Last Stalls. Milled from 8893.8' to 8895.3', slow, formation mill
is worn below 8893.8'. POOH to check BHA. LD string reamer assy. MU drilling assy. RIH
with BHA #5 (2 deg motor). Tie in to BHCS at 8790', subtract 9.0'. RIH and tag corrected TD
at 8884'. Drill build section (slow P rate) just below Zone 3 from 8884-8949'.

Drilling build Section from 8949; to 9363' (414') Landed at 9363' = 89° and a TVDss of 8969'
POOH for bit and motor change. Change bit, motor and MWD probe. RIH with BHA #.6 (1 deg
motor). Tie-in, subtract 15.0'. Orient to get thru window at 30L. Precautionary ream to TD.
Ream and backream thru 9090-9150'-sticky. Drill horizontal from 9363-9500' and swap out
mud to new FIo Pro. Backream to window. Set down at 9135' on RIH. Ream until clean. Drill
from 9500-9587' at 8965' TVD-no good sand. Need to come up to 8950' TVD per geo and
need more left turn. POOH for motor.
4/24/98 5 10086
POOH for motor. Tie in and corrected (+2'). POOH and PU 1.3 Deg Motor. TIH and
corrected to flag
(-16') RIH. Oriented TF to 80 L. Drilling and building angle. 100' wiper trip at 9686'. Drilling.
Drilled to 9759' and having problem drilling shale. Wiper trip to window at 9759' (Hole angle =
95° TVDss = 8955). Found good drill-able sand at 9808'. ROP inc to 60 FPH and some
differential sticking taking place with bit stacking. Drill to 9909'. MP #1 had packing leak.
Backream to window with MP#2 (can't drill with it). MP #1 back on line. Discuss lack of
productive sand with geology. There's not enough polygon left to make a well so will plug back
after sand is found. Drill from 9909' (8950' TVD) building angle to look for a good sand. Begin
having stacking problems at 9976'. Backream to window at 10003'. Drill ahead thru mostly
shales with associated stacking requiring frequent reaming to 10062' (8941' TVD). Backream
to 9200'. Drill ahead to 10086' (8937' TVD). Penetration rate started increasing.
4/25/98 6 10213
Drill from 10086-10203' with stacking, reaming and drilling with negative string wt. Located
sand at estimated MD of 10130' & at 8923' TVDss. Pump up survey to determine the TVDss
location. TD well at 10213' MD, 121°, 8887.8' 'rVDss (after tie-in (+10). POOH and Tie-in.
POOH to plug back well. OOH changed out BHA. RIH with cementing BHA #8 (nozzle), tie-in
to flag. TIH and taqged the shale at 9122' which is cap rock on zone of interest. Unable to
jet through shale ledge. PUH and wait on cement trucks (on other jobs). Got plan from town to
KO at 9000' and land at 90° at 8952' TVD. RU DS cementers (6.5 hr wait). Pump 6 bbls 17.0
ppg Class G cement followed by mud with nozzle at 9110'. Lay cement in open hole and 7"
liner to 8810'. CIP 2200 hrs. RIH and jet liner and open hole to 8980'. Chase cement to
surface while POOH. LD cementing BHA. Perform weekly BOP test. MU drilling BHA. RIH
with BHA #9 (2.3 deg motor).
4/26/98 7 899O
Finish RIH with drilling BHA. Tie-in to GR marker. Tag at min rate at 8987'. Orient TF and
RIH reaming and didn't tag any cement (UT at DS Lab at 4500 psi.) RIH and tagged hard at
9144' 20' below the cap rock. POOH to lay plug # 2. OOH change BHA. RIH and didn't tag
at 9144'. TIH to 9600' and batched mix up 14 bbls cement. Layed in12 bbl cement from 9500'
to 8800' at 1:1 ratio. Pulled to safety and WOC 15 min to allow setting. TIH at 0.5 bpm to
8950' to contaminate cement. POOH from 8950' at 2.1 bbl/min chasing contaminated cement
to surface. Lost 100 bbls to contamination. POOH. WOC to 4500 psi. MU drilling BHA. RIH
with BHA #11 (2.3 deg motor). RIH thru window at 51R. Tie-in to GR marker. Tag at rain rate
at 8935' corr. Orient to drill. RIH reaming. No cement to 8990'.

RIH and looking for cement. Stalled at 8977' Stringer, RIH and stalled at 9052' Drilled through
(cement soft). Dry tagged at 9071' Confirms the cements soft. Conversed with town. Will
cement plug back to 8950'. POOH and changed out BHA. TIH. Tagged TOC at 9031 CTM.
PUH and hatched up 6 bbls cement (BHT=145°F and WL at 74 cc, TT = 2:24). Spotted
cement at nozzle and with 0.5 bbls out PUH laying 1:1 . Contaminated cement to 8910'.
POOH and chased cement to surface. Cement in Place at 1200 hrs. Lost 80 bbl FIo-Pro due
to cement contamination. POOH and WOC. MU drilling BHA. RIH with BHA #13 (2.3 deg
motor). Tie-in to GR marker. RIH at min rate and tagged semi-firm cement at 9012'. Drill
consistent semi-firm cement to 9028'. Circ, WOC.

WOC. RIH. Stopped at 9040' No hard cement (8910'). Found hard cement at 9071' Plug #2
top. Stalled motor. Called town. Begin sidetrack from 9071'. Town called and want to PB
with cement. POOH and LD drilling BHA. PU cement BHA and RIH. Tagged at 9071' (hard);
PUH and wait on Vac trucks and cementers. Phase II on the slope. Batched mix 10 bbl
cement. Cement at density at 1230 hrs. PT, Pumped 5 bbl water ahead, 11.3 bbl cement, 3
bbl water behind. Pumped cement to the nozzle at 4.0 BPM With cement at the nozzle,
dropped Qp to 1.5 bpm and let 2.3 bbls get around nozzle. Reduced rate to 0.5 BPM and
POOH laying 1:1 ratio Pulled to 8400' and allowed cement to settle. TOC at 8647'. CIP at
1430 hrs, RIH and contaminated cement to 8890' at 0.5 BPM. PUH at 8890' Qp = 2.5 BPM
chasing cement to surface. Stopped at 8600' and circulated wellbore to determine loss
circulation. Circulate 1:1 ratio. POOH, lost 80 bbls rio-pro due to cement contamination.
WOC. RIH with drilling BHA #15. Tie-in. Orient to get by top of whipstock. Tag at 8890'. Drill
med firm cement to 8910'. Begin kickoff trough while WOC

Cut sidetrack trough while WOC. Begin ST at 8910'. Depart from original hole at 150R. Begin
build and turn at 8946'. Drill from 8946-9001'. Having trouble orienting. Backream into liner to
check, RIH thru window and new hole at 8L at min rate-no bobbles. Drill from 9001- 9099 ft.

horizontal turning right from corrected TD of 9349-9402'. Penetration rate increased at 9375'.
Drill horizontal turning left from 9402 - 9666 ft. Unable to make required turn with 1.0 Deg.
motor in soft sand. POOH. Adjust motor to 1.75 Deg. RIH. Correct depth at flag. Orient TF.

9892'. Encountered nasty shale thru 9715-9800'. Backream to window, tie-in, add 1.0', RIH.
Drill from 9800-9923 ft. Shod trip to window. Tie-in, add 1.5 ft to CTD. RIH. Drill horizontal
hole from 9923 - 10056 ft. Shod trip to window. RIH. Change out packing and plunger on
Pump#1.

Drill horizontal from 10056-10107' with lots of stacking and backreaming. Backream to
window. Tie-in, add 3.0'. RIH. Drill from 10107-10125' and had a good drilling break. Drill 50
APl sand from 10125-10285' at 8960-65' TVD. Backream to window. Tie-in, add 1.0'. RIH.
Drill from 10286 - 10290 FT BKB, Which is the final TD. POOH. Lay out drilling BHA. Held
Safety Meeting with all personnel. Ran 50 its of 2 7/8" STL liner with float equipment and 44
integral blade turbolizers. Fill liner. RIH with 20 more Jts of STL liner. M/U XO, Run 2 7/8"
FL4S liner.

Finish RIH with 103 jts 2-7/8" 6.16# L-80 STL/FL4S liner. MU Baker CTLRT. Install Baker
CTC. Pull test and pressure test. Replace bad blower motor on power pack. RIH with liner on
2-3/8" CT. RIH thru open hole with one set down at 9100'. Tag TD at correct depth. Displace
hole to 2% KCI water. Drop ball and displace. Unsting from liner. Held Safety Meeting. Mix
15.8 PPG CMT Slurry. Pump 18.0 BBLS CMT into CT. Load wiper dad and Displace with rig
pump. Wiper dad latched liner wiper at 60.5 BBLS. Sheared at 3100 PSI. Pumped plug with
2000 psi after 17.6 BBLS. POOH jetting. Layout Baker CTLRT. M/U nozzle and RIH on CSH.
Tag bottom or Cmt at 10176 ft. POOH. Held Safety meeting. RIH with SWS Memory log on
CSH workstring.

Finish RIH with SWS Memory GPJCNL. Log down at 30'/min from 8600' to tag at 10194' CTD.
Log 200' repeat, then log OOH at 30'/min to 8600'. POOH with CT. Standback workstring.
Determine that PBD is approx 50' higher than landing collar. RIH with 2.26" mill on 2-1/8'"
motor on workstring. RIH with CT. Drill firm cement from 10176 - 10228 ft BKB. Stall 3 times
at 10228 ft. Liner clean to top of landing collar at 10228 ft. Spot 20 BBL pill of high viscosity
FIoPro. POOH. Held Safety Meeting with all personnel, before picking up SWS perforating
guns.

MU nozzle assy. RIH and freeze protected tubing from 1900 ft to surface with 30 BBLS
MEOH. POOH. Closed master valve and swab valve. Displaced water from CT with Nitrogen.
Vac rig pits and green tank. N/D BOPE. Prepare rig for move to J-05a.

G-31A Sidetrack
Summary of Operations:
G-31 is currently shut-in due to high GOR. The sidetrack, G-31A, will target Zone 1.
This sidetrack will be conducted in two phases.
Phase 1: Lay abandonment cement and set whipstock: Planned for 4~09~98.
· A tubing integrity test will be performed. A dummy whipstock drift will be run prior to
perforation abandonment.
· Existing perforations will be abandoned with approx. 20 bbls cement. Abandonment cement
will be pressure tested to 1500 psi and tagged with AOGCC witness.
· A permanent whipstock will be set on ELine at approximately 8870' MD.
Phase 2: Mill Window and Drill Directional Hole: Planned for 4118/98.
Directional drilling coil tubing equipment will be rigged up and utilized to mill the
window and drill the directional hole as per attached plan to planned TD of 10,073'
MD (8,941' -I-VDss).
Mud Program: - Phase 1: Seawater
- Phase 2: FIo-Pro (8.8 - 8.9 ppg)
Casing Program:
- 2 7/8", 6.16#, L-80, ST-L liner will be run from TD to approx. 7194' MD and cemented with
approx. 20 bbls. cement. The well will then be perforated with coil tubing conveyed guns.
Well Control: - BOP diagram is attached.
- Pipe rams, blind rams and the CT pack off will be pressure tested to 400 psi and to 4000 psi.
- The annular preventer will be tested to 400 psi and 2000 psi.
Directional - See attachments.
- Kick Off: 8918' MD (8,764' TVD ss)
- TD: 10,073' MD (8,941' TVD ss)
- Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging · A Gamma Ray log will be run over all of the open hole section.
· Memory CNL will be run at TD.
