Box 100360 Anchorage, Alaska 99510-0360 Reference: Issuance of Permanent Closure Approval for drilling waste reserve pits at production pads DS IC and DS IB and exploration drilling sites East Ugnu #1, West Sak 9, West Sak 16, and Gwydyr Bay State 2/2A (includes Kuparuk Delta 51-1) all located in the Kuparuk River Unit Dear Mr. Kanady: The Alaska Department of Environmental Conservation (ADEC) has completed review and public notice for permanent closure approval of the inactive reserve pits at the above referenced production pads and exploration drilling sites located in the Kuparuk River Unit. The closure applications and corrective action plans provided the required information describing the history and conditions of the reserve pits, results of required water sampling, and an assessment of the potential risks posed by the drilling waste to human health and the environment. All sites were inspected by ADEC after completion of the corrective actions to confirm the work was completed in accordance with the approved plans. At production sites, all drilling waste was excavated from the pits for disposal by deep underground injection. The pits were tested for the presence of hydrocarbon sheening and trace metals before and after waste excavation. At these sites, the pits are closed as open impoundments. Trace amounts of residual hydrocarbon sheen may remain in some pits. Trace amounts of residual sheening is not considered a risk. The reserve pits at exploration sites West Sak 9, Gwydyr Bay State 2/2A and East Ugnu #1 were capped with the drilling waste in place. At West Sak 16, the drilling waste was excavated from the shallow pit and the pit backfilled. ADEC has determined that excavated or capped pits at these sites do not present a material or probable risk to the public health or the environment. Public notice requesting comments on closure approval was published September 29 and 30, 2004 in the Anchorage Daily News and posted on the ADEC web site September 29th 2004. No public comments objecting to closure approval were received in response to this public notice. ADEC has determined that the reserve pits at these sites meet the closure requirements of 18 AAC 60.440. Closure for the pad as a whole (if required) must be coordinated between the owner/operator and the appropriate land owner/manager. Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18 AAC 15.195- 18 AAC 15.340 or an informal review by the Division Director in accordance with 18 AAC 15.185. Supervisor 7 ( DATE: April 14, 2004 FROM: Chuck Scheve, Petroleum Inspector SUBJECT: Surface Abandonment East Ugnu #1 PTD#169-118 Wednesday, April 14,2004: I traveled to the Conoco Phillips East Ugnu #1 exploratory location to inspect the casing cut off and abandonment of this well. All casing strings were cut off at approximately 4' below original ground level and all were found to be full of cement. A %" thick marker plate was welded to the conductor casing with the required information bead welded to it. Gravel reclamation was ongoing at this time and good deal of equipment remained on location. A summer visit to ensure location clearance is recommended. SUMMARY: I inspected the casing cut off depth, and the P&A marker plate on the East Ugnu #1 well and found all to be in order. Four sites were inadvertently omitted bv ADEC ~om the list of closure approvals, and two additional omissions were identified by tine Department whiie reviewing Mr. Lowe's letter. Furthermore, four sites received conditional closure status contingent ur~on surface rehabiiitation: ~,_ree other locations were mentioned as candidates for further remedial action. Surface work is compiete or in ?ro~ess at these latter sires and their status is now sui~ject to further review by the Department. This correspondence describes our review procedures, puts forward the Department's f'mdings, and takes action upon the closure reauests. The sites have been re¥iewed in accordance with tSe ADEC Interim Site Assessment Guidance for Inactive Drilling Waste Sites. June t991. The third draft of Alaska's new Solid Waste Re~lations. i8 AAC 60. is currentiv under review, and th/s third draft contains a char~ter on Inactive Reserve Pit closures. The closure criteria in th/s proposed re_m~lation parallels the closure criteria in the 1991 Guidance Document. Closure under the 1991 Guidance Document should constitute closure under the proposed Solid Waste Regulations. The review procedures include an evaluation of the risk screening assessment required in the 1991 Guidance Document, and is summarized in our previous correspondence (Januarx' !2. 1995). Burrell, Director Gentlemen: Blowout Prevention Equipment We acknowledge receipt of your letter of February 24 concerning the check made by your representative of the blowout prevention equipment on our Rig #1 at Put River 23-11-13 and our Rig ~5 at East Ugnu #1. We assure you that corrective action on your criticisms is being taken. We welcome these inspections from the point of view that it keeps the industry "up to scratch" and trust that in the future your inspector will not have any cause to complain about our rigs. In particular we would like to comment as follows:- Put River 23-11-13~~ Rig #1 Blowout prevention equipment on our rigs is always tested on installation. On this particular well it had been tested approximately 8 days prior to your check and this was the first time that this particular hook-up had been in use on the well since the introduction of Conservation Order 83-B, so that there had been little opportunity for routine,.-regular tests to be instigated. Weekly tests will be carried out according to the regulations. A spring loaded type of inside blowout preventer was, we agree, not avail- able. but had been ordered prior to January 12 and was awaiting delivery. It is now on site. Blind rams could have been tested, with the rig delay time accepted, but this was not required by your inspector. The 5000 psi system approved on the drilling Permit was fully expected to be in use by the time this well was drilling but, in fact, delays in de]J~very prevented the comPlete installation of this equi~rment, which was 'being installed at the time of etlon of' tb.e new. " Blowout Prevention Equipment -2- March 6, 1970 East _Ugnu #1, Rig #5 Once again, the blind rams could have been tested and we would have accepted the down time; however, your inspector did not insist. The Swaco automatic adjustable choke was not working, but in fact a Swaco representative was on site attempting to correct the fault at the time. The hydraulic lines mentioned, which were leaking, on the BOP remote system, have been repaired but we would add that these leaks did not prevent the operation of the blowout prevention equipment. Since the end of last year we have been steadily improving and upgrading equipment on contractors' rigs working for us and are currently in the pro- tess of rewriting our blowout prevention and equipment testing procedures. Smith Petroleum Engineer State of Alaska DATE : February 23, 1970 ,SUBJECT, Inspection of British Petroleum' BOP equipment on North Slope On February 10 I traveled to Fairbanks via commercial aircraft on my way to the North Slope, but the flight was held at Fairbanks due to severe weather conditions on the North Slope. After holding in Fairbanks for 24 hours and in the face of worsening ":<::.",:her reports and an increasing buildup of personnel traveling to the North Slope, I .~:~ instructed to return to Anchorage and reschedule the tests when conditions were more favorable. I traveled to the. North Slope via commercial and charter aircraft On the morning of February 16, arriving at Prudhoe Bay at approximately 10:30 a.m. I was met by British Petroleum's senior drilling representative, Mr. Jim Gillespie, who informed me of BP's operations, and that no wells would be ready for testing until the next day. During the afternoon I visited with BP's camp manager, Mr. Charles Wark. BP has four wells drilling at the present time. One is the permafrost test hole, which presently has a fishing job at near total depth of around 2000'. This is on Put River Well No. 20-10-15 which they call Rig No. 4. Rig No. 3 is on Put River Well No. 31-11-14, and is currently drilling the surface hole but I expect they will be setting pipe within the next few days. Rig No. 1 is on PUT River Well No. 23-11-13. Mr. Gillespie and I traveled to this rig on the morning of February 17 about 9:45. In preparing for the BOP test they discovered that their drilling nipple, which was made of 13 3/8" pipe, was too small for the test plug and they requested that I allow them to use a cup tester, so that they wouldn't have to remove the drilling nipple and have an undue delay (since the. drill pipe wa's a. lmost out of the hole on both wells to be tested). This meant that we would not be able to test the blind rams with pressure, but I went ahead and o.k'd this procedure to prevent being responsible (indirectly) for downtime on two wells. Mr. Gillespie ordered new 16" drilling nipples to be prepared and installed on each well so that future testing could properly be conducted using a plug tester. Neither Mr. Gillespie, the senior drilling representative for BP on the North Slope nor their drilling contractors were aware of, or had copies of, the new field rules. I furnished Mr. Gillespie with a copy of the field rules and we discussed the BOP points in detail. Since they weren't aware of these rules, they obviously had not been complying with testing procedures when equipment was installed or testing to working pressures each week, and due to the laxness of their procedures I doubt if they even had been checking operation of the equipment prior to each trip. Rig No. 1 had two emergency valves of the manually operated ball-type available on the rig floor. After further checking it was found that they also had another valve of this type but no internal "inside blowout preventor" (spring-loaded type ) valve was available. The choke manifold system on this we~ was only rated at 3000~ so that was the extent of the pressure test. They are now in ~,,e process of hooking up a $000 psi working pressure manifold, which I saw on the rig ' floor. Of course, the objective depth of this well is the Sadlerachit and should require f,$00 psi equipment. ' · . ~ 'Memo to: Homer L. Burrell Page 2 From- Lonnie C. Smith February 23, 19 70 We left Rig ~1 at 2:30 p.m. and traveled to Rig ~5, which is approximately 30 miles west. We traveled by truck and arrived about 3:30 p.M. This Rig 95 is on '~--~.~,r East Ugnu Well No. 1, which they are handling as a tight hole. I observed from ~::e sit record that they are drilling somewhere below $000'. The test on this well was almost identical to Rig No. 1. The wellhead manifold equipment was also 3000~ on this well, and plans do not call for increasing the test manifold to $000 psi because after this well this rig will be released. It was necessary to use the cup tester, so the blind rams were not pressured. Their Swaco automatic choke would not operate properly, due to ice in the hydraulic system, so the manifold was pressured to the manual valve, upstream of the choke. During the BOP test a hydraulic line failure occurred in the remote line to the accumulator system on the blowout preventors, leaving the BOP system inoperative for approximately 30 minutes until repairs were completed. No springloaded internal blowout preventor emergency valve was available on this well.. All 'testing was completed at approximately 8:00 p.m. and we returned to Base Camp about 9:30 p.m. Ground winds had increased to about 30 miles per hour with blowing snow and temperatures of about -160. 'On Wednesday morning, February 18, I called Anchorage by the BP telephone and. was instructed to check on the BOP equipment of the Mobil West Staines well, provided it was deep enough. Upon request, the BP personnel, carried me to Deadhorse, where I contacted Mobil's production foreman, Mr'; Jerry?Mathews, by radio and he informed me that they did not have their full drilling stack...on yet, indicating that they were still drill- ing on the surface hole. . Some Other items of current interest picked up on.this trip were as follows: 1. I questioned Mr. Wark concerning, his knowle'dge a..well being drilled near Umiat (the McCulloch Oil Co. well),~:' and'.:he 'referred i it'~as the "King Resources Well'', and affirmed that:it was drilling. Mr.';~.Wark..,also told me that Polaris Drilling was the same as Brinkerhoff Drilling. I questioned him further and he called the MUkluk truck pusher .at Mukluk operations, and' confirmed from the pusher that the well.was drilling, a s..:Muklu.k ha's two tr. ucks working at the rig. tn fact, the statement was made that'they were atmbs'~ithrough with the hole. Mr. Wark als° .tOldine that the acreage was ..... , . 2. Another noteworthy incident was tha't:"i~':i Fish''~''& Game's man came in to see the BP personnel and I chatted wi.th,him fOr:.a few minutes. It seems that the State Fish & Game have placed a man on. permanent duty on the North Slope. The man will rotate every two weeks and they:.:,ha'Ve room and board at Burgess Construction Camp and are furnished':a truck, through Burgess. 3. It was also learned that Mobil has not yet spudded their Mekkelsen Bay well and Gulf was to spud their well this week and is drilling a water supply well. 4.
