GeNew-~: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Submit a
well schematic diagram with each 10-407 well completion report and 10-404 well sundry report when the downhole well design is changed.
IreM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for Injection,
Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely
segregated. Each segregated pool is a completion.
IreM 4b: TPI (Top of Producing Interval).
IreM 8: The Kelfy Bushing and Ground Level elevation in feet above mean sea level. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
IreM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
IreM 20: Report true vertical thickness of permafrost in Box 20. Provide MD and TVD for the top and base of permafrost in Box 28.
IreM 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool.

PPPOT-T RU HP bleed tool onto THA test void, 50 psi to 0 psi. Cycle LDS "Standard" (1) blank in place of
LDS all moved easily with no damage on flats. Pressure to 5000 psi for 30 min, 5000/ 15 mins, 4950/ 30 mins.
Bleed void to 0 psi RD test equipment, re-installed Alemite cap.

14-18A 9/25/2007 T/I/0=1750/1725/20 Load IA (Rig ST Prep) -Loaded IA w/a total of 10 bbl neat methanol, 433 bbls 80*F crude.
Pumped IA w/205 bbls, TP increased from 1750 psi to 3000 psi. Bled TP from 3000 psi to 1950 psi in 3 hrs, all
gas. IAP decreased from 2400 psi to 1350 psi while bleeding tbg. Pumped down IA w/additional 90 bbls crude
maintaining open bleed on tbg. TP increased from 1950 psi to 3000 psi. Bled TP from 3000 psi to 1900 psi in 2
hrs 20 mins, all gas. IAP decreased from 2500 psi to 1100 psi while bleeding tbg. Bled tbg from 3000 to 1600 psi
in 2hrs and 40 min, pumped an additional 68 bbls down IA. FWHP's=1250/600/0. Tbg/Perfs appear to be
plugged. Tagged wing and IA valves, Annulus valves left open to gauges.
Note: Flow line Blinded @ IGV.
14-18A 9/29/2007 CTU #5 Job Scope: Set CIBP/Cement P&A*
MIRU 1.75" CT Perform BOPE test to meet conditions of Sundry approval. Commission representative waived
witnessing test. RIH w. 2.5" JSN and drift & tag top of 2 3/8" liner @ 9883. Continue job on 9/30/2007.
14-18A 9/30/2007 CTU #5 continue from 09-29-07
RIH w. 2 1/2" CIBP sat down on 3 3/16" liner top @ 8834' attempted to work by no luck. Straightened CT and
added wt bars between MHA and setting tool in an attempt to straighten out the BHA. 2nd attempt no luck. Once
OOH went to dress shoulders on CIBP and slips fell out broken on to back deck of CTU. 3rd run was made with
Baker CIBP with machined cone continued to SD @ 8834' Called APE and discussed situation. RIH w. slick
assembly and 1.75" JSN. Continue job on 10/1/2007.
14-18A 1011/2007 CTU #5 Continue WRS from 9130/07
RIH w. 1.75" CIBP set @ 9915' CTM set down 3k twice. Attempted to varafiy integrety of CIBP would not
pressure test. Called town APE. RIH w. 2.5" CIBP set @ 9886 CTM Pressure test CIBP to 1000 WHP w. hole
full of 1 % KCL. RIH w. 1.75" BDN Tag CIBP & correct depth to CIBP (9885') PUH and flag pipe w. white & blue
flag. Lay in 11 bbls of 15.8 ppg Cement from 9885 to 8694' Wash down 2:1 with Biozan to 8830' POOH @ 80%
washing across GLM's Freeze protect well from 2500' w. 50/50 methanol. Continue Job on 10/1/2007.

Rig accept at 1100 hours.
13:30 - 14:30 1.00 WHSUR P DECOMP PJSM. Test 9-5/8" x 13-3/8" annulus to 2000 psi for 30
minutes. Test OK.
14:30 - 15:30 1.00 WHSUR P DECOMP PJSM. Rig up DSM lubricator. Test to 500 psi. Test OK. Pull
BPV and rig down DSM.
15:30 - 17:00 1.50 WHSUR P DECOMP PJSM. Rig up to circulate out diesel. Test mud lines to 3000
psi.
17:00 - 19:30 2.50 WHSUR P DECOMP PJSM. Circulate diesel out of tubing at 145 gpm with 310 psi.
Swap hoses and circulate down tubing at 155 gpm with 240
psi. Blow down lines and rig down.

BPV and install dart to convert to TWC. Test from below to

Unable to get low pressure tests. Trouble shoot and isolate

to 250 psi low, 3500 psi high and annular to 250 psi low, 2500
psi high as per AOGCC. Test witness waived by Chuck Scheve
with AOGCC. Test witnessed by WSL Dick Maskell and
Toolpusher Charlie Henley. Rig down test equipment and blow
down.

Description of Operations
DECOMP spooling unit. Continue to pull 4-1 /2" tubing. Recovered 148 full
joints, 1 cut joint at 18.50', 2 GLM, 1 SVLN, 2 SS bands and 28
control line clamps.
DECOMP Clear and clean floor. Prepare for testing BOPE.
DECOMP PJSM. Make up 5" test joint. Install test plug and test rams,
lower mud cross and top drive valves. Test to 250 psi low,
3500 psi high and Hydril to 250 psi low, 2500 psi high as per
AOGCC. Test witness waived by Chuck Scheve with AOGCC.
Test witnessed by WSL Dick Maskell and Toolpusher Charlie
Henley. Unable to get a good test on the manual IBOP.
DECOMP PJSM. Change out manual top drive TIW valve and adjust
grabber.
DECOMP Make up floor valve to test joint. Continue to test ROPE as per
AOGCC. Rig down and blow down lines. Install wear ring.
DECOMP Clear and clean floor.
DECOMP PJSM. Pick BHA up beaver slide.
RREP DECOMP Columbia installing skate indicator lights.
DECOMP PJSM. Make up 9-5/8" scraper BHA to 559'.
DECOMP PJSM. Pick up 5" DP and RIH to 4250'. Pull one stand and
rack back.
DECOMP Circulate hi-vis sweep surface to surface at 575 gpm with 750

The purpose of this sidetrack is to create offtake in the Sag River Formation. This new sidetrack will replace
14-18A, which has been a long term shut-in well since 2000
The Ivishak reservoir (e.g. Z4) will be completed if it turns out to be economically attractive. Should the
Ivishak reservoir be completed, the Sag target will be preserved as a future CTD candidate.
N2ES will pull the existing tubing string before the well is sidetracked at -4,300' to the 8.5" TD at -11,056' to
top set the Sag River. A 7" liner will be run, fully cemented. A 6-1/8" pilot hole will be drilled out of the 7"
shoe down to the Ivishak. Depending on the logging results, the Asset Team may case and produce this pilot
hole (via cemented and perforated solid liner) or plug it back with cement and drill horizontally into the Sag for
-2,500' to a TD of 13,552' MD. This horizontal production hole will be completed with a 4-1/2" slotted liner. A
4-1/2" chrome tubing string will be run for production.

Pre-Riq Operations:
1 E Run gyro surveys in the X-15A and 14-19 wells for anti-collision prevention. Report results to
ODE ASAP.
P&A the 14-18A well bore with CT. Place CIBP just above top of 2-3/8" liner (at -9,886' MD).
Lay in -9 bbls of 15.8 ppg cement on top of CIBP, leaving TOC approximately at the 4-1/2"

5. Pull 4-1/2" tubing from the pre-rig cut at -6,000' MD.

well bore with 9.8ppg brine.

before drilling HRZ.
15. Circulate the well bore clean. Short trip, circulate the well bore. POOH, UD assembly.
16. Change out to 7" rams and test.

32. Run and cement a 4- W', 12.6#, L-80 IBT -M solid liner. Ensure that there is 150' of overlap from the 7"
shoe. Set liner hanger and liner top packer. Displace well to clean seawater above the liner top.

CT Drilling Completion Intended Procedure
Background:
Well 14-18A was sidetracked from 4/12/98 to 4/30/98. The Shublik Interval was drilled as planned and a liner
cemented in place. After drilling out 272 feet into the Sag River Formation, CTD operations were suspended due to
MWD problems. Target TD was not achieved. Arco attempted to produce this well but it was uneconomic. Plans are
to return to the well to complete ddlling operations. Permit to Drill #98-07 was approved on 1/22/98. The operations
below are consistant with the odginal ddlling permit. Ddlling operations are planned to resume around March 1, 1999.
Intended Procedures:
Directionally drill 2-3/4" x -2000' horizontal well to access Sag reserves. Drilling will start at approximately 10260' in
the existing open hole. Crude will be used as the drilling fluid to help overcome fluid loss problems encountered
during the previous drilling of the Sag interval in this well. A new MWD system will be run due to problems
encountered in getting a signal in the MWD tools originally used.
Run 2-3/8" slotted liner to TD.
Return well to production.

DIRECTIONAL
Survey (DSS) were provided over the entire well. Continuous inclination was provided, which proved to be
a major advantage in determining if we were building or dropping angel before we got to a survey point.
Survey on the fly was used through out the well, thereby reducing the risk of getting stuck.
FORMATION EVALUATION
Realtime GR values were provided in order to identify formation lithology. In doing so we were possible to
geosteer the sands and shales.

LESSONS LEARNED
Survey quality based on direction we were drilling, again was a problem on this well as in the past. We will
be using a survey program called MAGCOR,(which after 4 surveys have been taken at the same depth, in
4 different tool faces, 0, 90, 180, 270.) Which will determine what axis is most effected, and apply a
correction to compensate for that interference.
Anadrill appreciated the opportunity to provide ARCO Alaska Inc. with MWD SHARP and (GR) services
on 14-18a.

Intended Procedure
Objective:
Set whipstock. Plugback existing wellbore. Directionally drill a horizontal well
from the existing wellbore. Complete with 2-7/8" slotted liner. Run tubing
patch across GLM #5.
Intended Procedure:
Set top of whipstock @ - 9280'. Cement squeeze the current
perforations and leave cement @ ~ 929q md.
Mill window in 7" casing at ~9285' md. Drill 10' of formation and
perform Leak off test to 9.4 ppg, 0.5 ppg above the highest anticipated
mud weight (8.9 ppg FloPro).
· Directionally drill 3-3/4" x -20D0' horizontal well to access Sag reserves.
· Run 2-7/8" slotted liner to TD.
· Run tubing patch across GLM #5 and return well to production.
