Attempt to bled off 50psi WHP on P-06 - no success. Call out
LRS for load & kill.
15:00 - 19:30 4.50 RIGU P PRE PJSM. Install gooseneck & pack-offs. Stab CT. RU & cut 500'
of CT for fatigue managment. Also changing out washed out
HCR control valve on koomey.
Meanwhile W.O. Little Red Pumping Services to arrive onsite.
19:30 - 22:00 2.50 RIGU P PRE Little Red Pumping Services arrive pad. Conduct PJSM for
P-06A well kill. MIRU pumping equipment & hardline on P-06.
Open up well & observe 50psi WHP, Opsi lAP & Opsi OAP.
load/kill well by bullheading 5bbls of MeOH spacer ahead,
290bbls seawater & 5bbls MeOH spacer behind at max rate.
Observe max injectivity of 5bpm @ 2900psi. WHP dropped to
50psi after pump SID. Bled down WHP toOpsi.
Meanwhile P/U BOP stack & install X/O flange. Continue
changing out washed out HCR control valve on koomey.

koomey. Need koomey to operate the front boosters.

Prime up. Flush/PT lines to 4k. Mix & pump 15.3bbls 15.8 ppg
class 'G' cmt slurry & 5bbls of water behind @ 2.85/2.40bpm &
1500psi. Chase with KCL water @ 2.6212.27bpm & 1750psi.
With cmt 1 bbl from nozzle. Shut-in choke. Bullhead 7.5bbls of
cmt away @ 1.25bpm from 20psi to squeeze pressure of
1000psi. Open up choke. Lay-in remaining 7.8bbls cmt 1:1 to
TOC @ 9588'.
POOH to safety @ 9000'. Shut-in choke.
Hesitate squeeze 5x from 500psi to 1000psi final squeeze
pressure. Squeeze in additional 5.1 bbls of cmt.
Hold final squeeze pressure of 1000psi while circ down
bio-KCL fluid to the nozzle @ 1.6/1.6bpm & 1700psi.
RBIH contaminating cmt to 9964ft with 3.25bpm/1550psi. Make
3 down & 2 up passes.
Jet OOH chasing contamintaed cmt. Jet thoroughly across
jewelries & GLMs.

BP has requested approval to plug-back p-01A in preparation for a sidetrack. The
production history of the well is shown below. The current wellbore was drilled as an
injector, and was pre-produced from 1998 until August 2002. The production had
declined to less than 100 BOPD. The well has a collapsed 2 7/8" liner at around 1 0,550'
md. The new sidetrack is planned as a WAG injector into Zone 4B. There are no
reserves remaining in the well and inj ection support in the area will help improve
hydrocarbon recovery.

Re: Prep Prudhoe P-06A for CTD sidetrack
Sundry approval is requested to prepare P-06A for a sidetrack. P-06A is a Zone 4 CTD sidetrack completed in
Dec. 1997. P-06A was intended to be a pre-produced injector but was produced until August 2002 when the well
was shut-in due to low productivity and limited well work options due, in part, to a collapse in the 2 7/8" liner at /'
approx. 10,550' MD.
The sidetrack, P-06B, is scheduled to be drilled by Nordic 1 on or about October 12,2005. The,plan'callsfor
plugging the P-06A 2 7/8" liner prior!9 setting a whipstock for an exit in the Shublik interval. P-06B will'target
Zone 4B with an undulating sidetrack to be completed as a water/MI injector to support producers P-25Ll, P-25 ~
and P-26. The following describes work planned and is submitted as an attachment to Form 10-403. A schematic, '
diagram of the proposed sidetrack is attached for reference.

Pre-Rig Work
1) Complete integrity testing.
2) Drift 2 7/8" liner and set a CIBP at 10,025' MD (8844'ss). /'
3) Set mechanical whipstock at 9974' MD (8806'ss). '/
4) Remove flow line, remove well house, and level pad.
The pre CTD work is slated to begin about Oct. 1, 2005.

08/05/03 RIH with SWS memory logging tools in carrier. Retrieved good data. RIH with SWS 2in perf
guns-6spf-60 deg-( All guns loaded the same) perf interval 1 05241-1 05491(251) POH all shots
fired. MU & RIH with SWS #2 perf gun, perf interval 1 04991 to 105241 (25') POH all shots fired.
RIH with memory logging tools. Unable to work past tubing tail/ packer stub. POH. Job in
progress.
08/06/03 RIH with SOS logging tools in carrier. Tagged up at 10369 (up). Pump 30 bbls down
backside. Log down from 10000 to 10500 at 60 fpm with slight bobble at 10369. Log up over
same interval and flag pipe at 10300. POH. Verify data. Good data.
08/06/03 RIH with SWS 2in perf guns-6spf-60 deg-( All guns loaded the same) perf interval 1 04501-

Move DS/N #1 from C-23a to P-06ai. Standby from 1530 hrs to 2330 hrs 12/7/97 while DS CTU
#6 finished cement plug. Electricians disconnect wellhouse electrical. Remove wellhouse.
Remove tree cap. Move rig over well. Accept rig for operation at 2330 hrs 1217/97. NU XO
spool and BOPE. Fill pits with SW. Grease swab, master, wing. Test BOPE and blinds leaked-
investigate.

Continue BOPE test. Witness waived by AOGCC. Replace Blind element and PT to 4000/250
psi. RU WL. Open well. WHP = 400 psi. Bleed down to 37 psi. SWI and pressure increased to
320 psi in 38 minutes. Called DS and squeeze sample was still liquid at T = 24 hfs (no
compressive strength). RU WL and TIH to tag. Tagged at 10072', (22' above peris), POOH with
WL (WHP = 400 psi). PT wellbore to 1500 psi. lost 50 psi in 2 min. Bleed pressure off to zero.
%" stream flowing. SWI and PT to 1000 psi. lost 70 psi in 60 minutes. PT and tag was approved
by AOGCC. Hold press while WO on Eline equipment. Pressure bled to 740 psi in 6 hours.
Bleed off and monitor-stable. RU SWS E-line with 20' lubricator and packoff. MU Baker
Permanent whipstock. Safety meeting. RIH with whipstock. Spend considerable time getting
acceptable orientation. Set WS with top at 10040', bottom at 10052' at 5.6° R. POOH. RD SWS.

Finish RD SWS. Load CT with water and PU BHA#1, TIH. Tagged SWN nipple, Corrected +2.5'.
Milled SWN, TIH tagged top of WS at 10040'. Milled window from 10039' to 10045' CTM. Outside.
Backed reamed twice, and dry tagged, OK. POOH for run #2. OOH, change out BHA. RIH with
BHA #2 (string reamer and undergauge bit). Ream thru XN nipple until clean. Dress window until
clean. Drill 6' of formation to 10055'. Displace seawater in wellbore to 9.1 ppg used FIo Pro mud.
POOH for drilling BHA. LD mill and string reamer. MU drilling BHA. Install new CTC. RIH with
drilling BHA #3 (2.6 deg motor).

Orient TF, Drilling, hard to get through window with BHA.. At 10090', ~ of BHA out in open hole,
ROP at 80 fph. Drilled to 10220' and orient TF to Left side. Magnetic interference is creating
problems with the G totals and Boxy (magnetic interference). Drilled to 10230' and was unable to
get 90L TF. POOH to change bend and inspect directional package. Phaze 1. Change motor
and MWD. RIH with BHA #-4. Window is clean. Drill from 10230' dropping angle. Drill and ream
a sticky shale from 10320-400'. Get horizontal at 10364' and 8953' TVD. Drill and build on the
plan line to 10425'. Phaze 2/3. Backream to window prior to POOH for temp log.

Backream to window. Tie-in at 10000' Corrected +19'. Orient TF and TIH and tagged bottom at
10441' POOH. To run Memory tools. Change out BHA. TIH. Stopped at 9900' TIH and logged
OH at 40FPM to 110435'. Stopped. Logged up at 40 FPM to 9900'. Stopped. POOH. RD BHA
and downloaded memory tools. Data "Good". PU Drilling BHA. TIH and tie-in at 9970'. Drilled to
10603'. Stuck pipe at 10593 and worked free. Wiper trip to window. Drilling at 10663'.
12/13/97 7 11220
Drilling, Orient and drilling. Mud Swap. Wiper trip at 10742' Drilling. Broke over to the tanget at
10730' MD (TVD = 8907') Drilling. Wiper trip at 10876' to window. Drilled to 10897' PUH to
switch MP and Tie-In inside 5.5" casing. Corrected (+20") RIH, tagged at 10100' and 10125'.
Worked through. TIH. Up 3000# extra drag in the inverted ramp.. Orient TF, Drilling. Drilled to
10969', lost packing on MP#2. Hole getting sticky at due to mud differential. NE(MP's) time
started at 1730 hrs. PUH to window. Sticky at 10760' to 10650' ,10k over, (Turn down section of
hole). Return both mud pumps to operational status. NE ended 2030 hrs. Ream in hole. Ream
thru 10110-26', 10200', 10362'. Spent 1.5 hrs reaming 10700-800' with several 20K over pulls.
Drill from 10969-11100'. Backream to 10300'-considerable improvement. Drill from 11100-11220'
12114/97 8 11652
Drilled to 11243', Wiper trip to window. Tight from 10760' to 10696'. (10k over). Pulled to 9960'
and Tie-In. (+17' correction), Orient TF and RIH. Tagged several times from 10113' to 10143'
(9R). RIH, worked through 10700 to 10856' Tagged bottom, drilling. ROP slow, Bit trip. Change
Out BHA. TIH, Tie-in and no correction. Orient TF and RIH. Ream tite hole 10100-140' and
spend 40 minutes getting by 10770'. RIH. Drill from 11350-500'. Backream (clean) to window.
Tie-in in liner, add 11.0'. RIH clean to corrected TD. Drill from 11505-11652' while turning right to
bullseye target. Short trip.
12/15/97 9 11925
Finish short trip to 10300'. TIH and tagged at 10700'. W/R from 10700' to 10800' in 30 minutes.
RIH. Tagged bottom at 11652'. Drilling. Tagged bottom at 11652' Drilled to 11680' Started having
trouble with TF. Oriented TF and acted like its faulted. Beat ourway through to 11700'. ROP on
the other side is +100 FPH, No fluid lose at this time. Orient TF. Drilling. Drilled to 11775', Losing
4 to 5 BPH of mud. Wiper Trip to window. 70k at 11038', Pulled 2 to 10k lb. over CT weight from
10824' to 10675',(@ 10 ft intervals). Smooth from 10675' to 10300'. TIH. Tagged at 10687' and
10734'. Good trip in. Tagged bottom. Orient TF and back to drilling. Taking 3000 to 10,000#WOB
to get bit to drill. Drill from 11775-925' (with no mud losses). Short trip to window. Spent 55 min
getting by 11727' and 45 min getting by 10680'. The rest of hole is clean. Park at window to tie-in
and MWD quit. POOH for MWD at 2330 hrs (NE). Change MWD, bit and CTC. RIH with BHA
#8. Tie-in at 0500 hrs (OE). Add2.0'. RIH.
12/16/97 10 12130
Continue RIH after trip. Tagged 10765',11067'.11325, 11606'. 11633' (1 hrs to get through) Clean
below 11700' RIH and tagged bottom. Orient TF, Drilling. Having trouble sliding. Taking 10,000

12130' which is uncorrected TD of well. Attempt to get survey w/o success. GR is shot too.
TF is working. NE starts at 0345 hrs. Backream to window.

get through 10732' MD with 90L TF. This area is getting progressively worst with wiper trips. RIH
to clean out to bottom. Tie in at 11854' (+11') RIH and tagged at 12130' (Hard). POOH for logs.
OOH, Backed flushed 4 bbl methanol in CT. RD BDH. Sperry on standby at 1430hfs and
Baker INTEQ released at 1430 hrs. ND BOP and move rig off well. RU well house and Chuck
"Shack". Hold Safety and Operational Meeting. RU CTE Unit. RIH w/CT E-line tools. Tools
stopped at 10,115'. Unable to work tools deeper. POOH.

from 2150 psi to 1700psi quickly. Cir well and wait on cement trucks. Pump 3.5 Bbl squeeze
cement. With 1 Bbl cement out nozzle, pull up hole at 45 fpm while pumping 1.0 bpm. Pull above
cement top and begin to ramp pressure up with walking squeeze. Pressure ramped to 1625 psi.
Shut down pump, close choke. Pressure stable. Release pressure and wash in hole
contaminating cement. Wash 100' down inside liner. Circulate out cement. Estimate 1.5 Bbl.
Cement in returns to surface. POOH. Stand back and SLM 1 ¼" CS Hydril. Found error in
electronic depth counter (+45'). WOC to test liner top squeeze.

WOC .DS Lab results at 1900 psi (150F) 10 Hrs. PT liner lap. Stabbed on well with Injector head.
PT to 2000 psi, lost 400 psi in 4 minutes, re-tested and and pressure loss was the same. PT to
1000 psi and lost 250 psi in 8 min. This is the same pressure loss as before the liner cmt
squeeze. Cir and warm up CT. Ambient T =-48F at Chill Factor ~.. -85F. Change out Blind
Rubber. PT to 250/4000 psi. Re-tested wellbore. Same results. Wait on Baker tools to PT the
liner internally. PU BHA. TIH. Stag into liner and test internally to 3000 psi-Held OK. Pull out of
liner and test liner lap. Pump in 0.85 BPM @ 2500 psi. POOH. Run in hole with squeeze nozzle.
Spot cement, pull above. Close choke, pump in 2.3 Bbl. Pressure went to 3000 psi. Open choke
and begin circulating. Ramp pressure up using choke. Pressure ramped to 3120 psi. Had several
pressure breaks while ramping. Shut down pump, close choke. Pressure stable at 2875 psi. Wash
in hole contaminating cement, holding 1000 psi back pressure while washing. POOH. Holding
500-600 psi back pressure. WOC for liner top test.

perf guns loaded at 4 SPF. RIH on 32 stands of CSH workstring. Change out well / rig pits to
FIoPro. Tag PBTD of 12,063 ft at 12,032 ft CTD, correct CTD and position bottom shot at 11,900
ft. Pump 0.5" steel ball, ball on seat at 48.9 BBLS. Firing head sheared at 4300 PSI. POOH.
Held safety meeting. Lay out SWS guns, All shots fired, Perforated 11,812 - 11,900 ft BKB.
RIH with second set of perf guns [128.43 ft]. RIH on same 32 stds of CSH workstring. RIH with
CT. Tag PBTD of 12,063 ft at 12,037 ft. Correct CTD. Position bottom shot at 11480 ft. Charges
set at 11,480 - 11,460 ft and 11,440 - 11,370 ft BKB. Pump 0.50" steel ball.

other governmental agencies, and does not authorize conducting drilling operations until all other
required permitting determinations are made.
The blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25.035;
and the mechanical integrity (MI) of the injection wells must be demonstrated under 20 AAC
25.412 and 20 25.030(g)(3). Sufficient notice (approximately 24 hours) of the MI test before
operation, and of the BOPE test performed before drilling below- the surface casing shoe, must be
given so that a representative of the Commission may witness the tests. Notice may be given by
inspector on the North Slope pager at 659-3607.

Summary of Operations:
This sidetrack is designed to improve injection conformance by abandoning the existing injection
interval and completing a new injection interval in Zone 4.
This sidetrack will be conducted in two phases.
Phase 1: Abandon existing perfs and set whipstock: Planned for 1213/97.
- The existing perforations will be abandoned with cement containing 20 ppb aggregate. After
underreaming, the cement will be pressure tested and tagged with AOGCC witness.
- A whipstock will be set on E-Line at approx. 10,040' MD.
Phase 2: Drill Directional Hole: Planned for 12110197.
- Directional drilling coil tubing equipment will be rigged up and utilized to mill the window
and drill the directional hole as per attached plan to planned TD of 11,971' MD (8,960'
TVDss).
Mud Program:
-Phase 1: Seawater
- Phase 2: FIo-Pro (8.8 - 8.9 ppg)
Casing Program:
- 2 7/8", 6.16#, L-80, ST-L liner will be run from TD to approx. 10,000' MD and cemented with
approx. 20 bbls. cement. This will place the top of the 2 7/8" liner within the existing 5 %"
liner. The well will then be perforated with coil tubing conveyed guns. The depth of the top of
the cemented 2 7/8" liner is planned to allow for a future Sag R. sidetrack.
Well Control: - BOP diagram is attached.
- Pipe rams, blind rams and the CT pack off will be pressure tested to 400 psi and to 4000 psi.
- The annular preventer will be tested to 400 psi and 2000 psi.
Directional - See attachments.
- Kick Off: 10,051' MD (8,864' TVD ss)
- TD: 11,971' MD (8,960' TVD ss)
- Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging - A Gamma Ray log will be run over all of the open hole section.
- CTE-Line will be used to run GR/Induction logs at TD.
