ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. ITEM 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.). ITEM 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. Several fishing attempts have proved unsuccessful. The wellbore is prodidJ~3~ production or value to the field. The original parent well, C-23, was drilled in September 1982 as a conventional Ivishak producer. In October 1996 C-23A was drilled TTRD as an Ivishak Zone 2A/lB horizontal producer. The well came on at high oil production rates with solution GOR. With-in 4 months, C-23A had reached MGOR and was shut-in. Several evaluation logs were run with limited success. In October 1997 a GSO operation was initiated. The C-23A horizontal was successfully cement squeezed with a service coil unit. After washing out the liner, perf guns became'stuck while RIH on the first run. C-23A remains unperforated since the cement squeeze. All subsequent fishing attempts have been unsuccessful. /'" The C-23B sidetrack is currently scheduled for Nabors rig 2ES, beginning around May 19, 2003, after the completion of the C-16A sidetrack. The pre-rig work for this sidetrack is planned to begin as soon as the attached Application for Sundry is approved. Scope Pre-Ri.q Work: * Notify the field AOGCC representative with intent to plug and abandon * 1. Rig up slick line, drift 5-1/2" tubing to top of fish at ~ 8,742' MD. 2. Fluid-pack the 5 ½" tubing and 9-5/8" x 5 ½" annulus to surface. 3. R/U E-line. RIH with Chemical cutter and cut the 5 ½" tubing at -8,692' MD. 4. Test the 9 5/8" pack-off and tubing hanger seals to 5000psi for 30 minutes. Function all Lock Down Screws, repair or replace as needed. 6. Bleed casing and annulus pressures to zero. 7. Set a BPV in the tubing hanger. Secure well. 8. If necessa_? level the pad. Please provide total well preparation cost on the handover form. Ri.q Operations: 1. MIRU Nabors 2ES 1. Pull BPV. Circulate out freeze protection and displace well to seawater. Set TWC. 2. Nipple down tree. Nipple up and test BOPE. Pull TWC. 3. Pull 5-1/2" tubing from above pre-rig cut. 4. Bullhead kill well with 340 bbls of produced water. Jetted liner from 10,000' - 11,500' with water. Liner was · cooled with about 200 bbls of water. Injectivity was 2 bpm on vacuum. Acidized the perfs with 13 bbls of 15/10 HCL with no noticeable change in injectivity. Loaded backside with water while pumping cement. BH 8 bbls of cement from 11,020' with chokes closed. The well began to lock up after 7 bbls went behind pipe. Laid in remaining 12 bbls holding 3000 psig. Held Sqz pressure for I hr. Noticed a slight leak off rate but squeeze looked fairly tight. Jetted dual polymer from 8900' to 11390'. Had trouble Jetting cement at 80 fpm. We had to adjust RIH and pump speeds to finish C/O. PUH to 8900' then jetted liner to a depth of 11390' with water. POOH at 80% while jetting focusing on jewelry. FP well from 2100' with methanol. Final PT lost 200 psig in 5 mins. MI was not on location to estimate returned cement.7 bbls behind pipeC/O to 11,390' CTMD bpm down, 1.0 bpm up. Reversed out 12' of sand. Top of sand at 9980'. Fluids Pumped BBLS. ITEM 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In, Other-explain. ITEM 28: If no cores taken, indicate 'none'. Set liner hanger / packer with 4200 psi. Reverse out clean. Please note that approximately 2 bbls of cement retard biozan solution will KO plug prior to switching to SW. 11. M/U 1-11/16" drill pipe conveyed perforating with 10.5 gram charges, 4 spf, 60 / 120 phasing and RIH on 2 3/8 x 2-7/8" drill string. Include tools as follows: Absolute pressure firing head Perforating guns & blanks, as per enclosed production specs. Firing Head Adapter Primary Firing Head ( Annulus Pressure) x/o 2-3/8 EUE to 2-7/8 PAC DP 2-7/8" PAC 6.4# L-80 tubing ( approx. 12 joints ) x/o and Below Packer Safety Joint Bypass Port 5,5" Champ Packer ( To be set @ xxx' MD / xxx' TVD ) 2-3/8" PAC pup joint Jars PR FAS-FIL Valve to be set at @ xxx' MD / xxx' TVD to achieve xxx psi under balance. 2-3/8 x 2-7/8 drill string to surface 12. POOH with perforating guns and L/D same. 13. R/U slick line and RIH to 2112' MD and install SSSV. Close the same. This will serve as a second barrier for N/D and N/U operation. 14. Install BPV and N/U production tree. Complete all appropriate documentation and hand the well over to production. RECEIVED JUN 2 c 1996 Alaska Oil & Gas Cons.
