ITEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is a completion. ITEM 4b: TPI (Top of Producing Interval). ITEM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other spaces on this form and in any attachments. ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00). ITEM 20: True vertical thickness. ITEM 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. ITEM 23: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the producing intervals for only the interval reported in item 26. (Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval). ITEM 26: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In, or Other (explain). ITEM 27: If no cores taken, indicate "None". ITEM 29: List all test information. If none, state "None". ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. ITEM 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.). ITEM 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. ITEM 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In, Other-explain. ITEM 28: If no cores taken, indicate 'none'. The drilling fluid program and surface system will conform to the regulations set forth in 20AAC25.003. b. Maximum surface pressure is based on BHP and a full column of gas to TD (0.10 psi/fi). c. The most recent H2S level on this well indicated 8 ppm H2S (maximum 12 ppm). Even though very little H2S has been detected, proper precautions will be taken as needed. d. No hydrocarbon-bearing zones expected above the Sag River formation. e. Gas cut mud has occurred on several wells on F Pad in the IKRU/LCU. Must be prepared for the possibility of shallow gas. f. Stuck pipe and lost circulation problems have occurred on F Pad in the Kingak, Sag River, Shublik and Sadlerochit formations. Faults have shown to be highly conductive on F Pad. F-34A Permit Information Page 3 F-34A Operations Summary Pre-Rig Operations 1. Pull the 4-1/2" SSSV. Valve is set at 2,113' MD. Test the 9-5/8" mandrel packoff to 5,000 psi. 2. Notify an AOGCC representative prior to commencing plug-back operations. RU CT unit. RIH with coiled tubing to 10,285 MD - 100' MD below the bottom perforation (bottom perforation at 10,1858' MD). Lay a cement plug from 10,285' to 9,600' MD. This will leave cement about 300' above the top perforation (top perforation at 9,900' MD). NOTE: Top of cement must be to at least 9,800' MD to comply with State regulations on cement being 100' MD above perforations. 3. With an AOGCC representative on location to witness the operation, R/U slickline and tag the top of the cement to verify cement top is at least at 9,600' MD. 4. RIH and cut the 4-1/2" L-80 tubing with a chemical cutter in the middle of a full joint of tubing at approximately 8,600' MD. NOTE: This will place the tubing cut approximately 231' MD below the bottom of the section window cut in the 9-5/8" casing. 5. Displace the tubing and wellbore to 8.5 ppg seawater. 6. Freeze protect top 2,200' MD with diesel. 7. Bleed tubing and annuli pressures to zero. 8. Set BPV in the tubing hanger and test from below to 1,000 psi. Secure well. 9. Remove the well house and flowlines. If necessary, level the pad and dig out the cellar. Rig Operations 1. MIRU drilling rig. 2. Pull BPV. Circulate out freeze protection from inner and outer annuli and recover for future use. Test 9-5/8" mandrel packoff to 5000 psi. If pack-off fails, change out packoff in Step 5 prior to cutting the section. 3. Install a 'FWC and test from below to 1000 psi. N/D the production tree. 4. N/U BOP. Test BOP to 250/5000 psi against TWC. Test annular preventer to 250/2500 psi. Remove TWC. Send adapter flange and tree to DSM for inspection and repair. 5. Pull and lay down the 4-1/2" tubing. (Tubing should be jet cut at 8600' MD. Check the location and exact depth of the cut on production handover form) Inspect for NORM. Label tubing as it is laid down with well name and joint number. Install thread protectors on pipe which upon visual inspection appears to be re-usable. If tubing is salvageable, send tubing to Tuboscpoe for inspection. If 9-5/8" mandrel packoff did not test in StePl~'~{t1111'~, L~,., packoff through the BOP stack. JUN 0 7 199G Alaska 011 & {3as Cons. This will place the bridge plug 3' below a 9 5/8" casing collar at 8294'. The whipstock will begin to mill out the casing 30' above the bridge plug. Shut in well and test casing to 3500 psi for 30 minutes to ensure casing integrity. P/U and RIH slowly with a Baker Anchor/Whipstock assembly with MWD. MWD will be used to align the whipstock to 45° left of highside. The bottom trip anchor will be set by slacking off on the bridge plug. Continue RIH 40' above bridge plug. At this time start pumps and pulse MWD to check whipstock face orientation. Using MWD, align whipstock face to 45° left of high side making sure all excess torque is worked out of string. When proper orientation is achieved slack off with pumps watching for undesired whipstock rotation until bridge plug is tagged. At this point slack off 10 klbs to shear run-in pins on anchor. After pins are sheared pick up 10 klbs over to assure anchor is set. Slack of _+45 klbs to shear whipstock from mill. Once sheared from the whipstock, displace well with sidetrack drilling fluid. Begin cutting window using only enough WOB and RPM to cut window as smoothly as possible. Continue milling casing and try to make at least 22' of formation, approximately 38' total of milling and drilling from the top of the whipstock. This will allow the full-sized watermelon mill to pass through the window all the way down to the bottom of the window effectively reaming the sidetracked interval. POOH after ensuring the hole is clean and free of metal cuttings. RIH with an 8-1/2" steerable assembly with MWD - directional and build to 65° tangent inclination. Hold tangent to pilot hole TD.
