Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is a completion. TPI (Top of Producing interval). The Kelly Bushing elevation in feet above mean Iow Iow water. Use same as reference for depth measurements given in other spaces on this form and in any attachments. The APl number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00). True vertical thickness. Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the producing intervals for only the interval reported in item 26. (Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval). The sidetrack, G- 12B, will kick off from a window milled in the Sag R. formation after cutting, pulling and possibly milling a portion / .... of the existing G-12A 2 7/8" liner. A cement evaluation log recently run on the G-12A 2 7/8" liner shows free pipe just above the Sag R. formation. Some cement is indicated across the Sag R. interval but the mechanical strength of cement across the Sag is difficult to predict. The desired KOP is the Sag R. interval or below. The plan summarized below calls for making an initial cut of the 2 7/8" liner below the Sag R. If the 2 7/8" cannot be pulled free from this cut then a second cut will be made just above the Sag R. where free pipe is more certain."~f the higher cut is required then approx. 50 feet of 2 7/8" will be removed by milling such that a whipstock may be installed within the Sag R. '"'" interval. The G-12B target is Zone 2. The following describes work planned to prep for sidetracking and is submitted as an attachment to Form 10-403. A schematic diagram of the proposed sidetrack is attached for reference. 1. Pulled 7" POGLM (4/14/02). 2. Log 2 7/8" liner with Cement Mapping Tool (6/08/03). 3. Ran PDS Caliper from top of 2 7/8" liner to surface. S_.et plug in tailpipe. (6/14/03). 4. Combo MIT (tbg. X IA) tested to 3000 psi (6/17/03).~ /,.-- 5. MIT-OA to 2000 psi (8/02/03). Tubing, IA and OA are at zero pressure. ' 6. Slick line will pull the tailpipe plug. 7. E-line will set and test a cast iron bridge plug at 10050' in the existing 2 7/8" liner. 8. E-line will chemically cut the 2 7/8" at approx. 10,000', in the Zone 4 interval. 9. Service coil will pull on the 2 7/8" liner. If the liner is free then service coil will rig down. 10. If the 2 7/8" liner is not free then e-line will make a second cut at 9770', just above the Sag R. 11. Service coil will rig up and pull on the 2 7/8" to confirm it is free. 12. Severed coil will be pulled by Nordic 1. E-line is scheduled to set the cast iron bridge plug on Sept. 1,2003./"Drilling coil is scheduled move onto the well for sidetrack drilling on Sept. 18, 2003. Existing Ivishak perforations will be squeezed in preparation for sidetracking with coiled tubing. The sidetrack will be conducted in two phases. Phase 1: Mill Window (planned for June 10-17, 1996.) Existing Ivishak perforations will be squeezed. A thru-tubing whipstock will be set on E- line and coiled tubing will be used to mill a window in the 7" liner at 10245' MD. Coiled tubing will then be rigged down and 1 to 2 weeks will lapse before drilling operations begin. Phase 2: Drill Sidetrack (planned for June 21-July 4, 1996) A 3 3/4" hole will be drilled with coiled tubing to 11,532' MD per the attached directional plan. An MWD will be used for directional control and surveys will be taken every 25 ft. No gyro will be run. Drilling Fluid Program: Phase 1: Seawater with Biozan sweeps. Phase 2: Solids Free FIo-Pro (8.8 ppg to provide approx. 500 psi overbalance) Logging Program: A Gamma Ray log will be used in geosteering while drilling and will be run over the entire drilled section. A GR/Induction log will be conveyed on CTE prior to running a liner. Liner / Completion Program: A 2 7/8", FL4S, L-80 liner will be run to TD and cemented with 90 cu.ft cement. A PBR at the top of the liner will be located in the existing 4 1/2" tubing tail at approximately 9230' MD. The liner will be perforated with CT conveyed thru-tubing guns. Well Control: BOPE schematics for the drilling and completion operations are attached. The full production tree will remain in place during all drilling and completion operations.
