(MSCF). The event lasted for 150 minutes. The event had a volume greater than 5000 (MSCF) and occurred when
Tandem Compressor 1854 shutdown due to control/instrument failure associated with the PLC. The event stopped when
1854 T/C was depressurized and shut down.

A flaring event, specifically a leaking flare valve, has occurred at CGF starting at 02/18/2015 00:01 and will be
ongoing until valve repair parts are received and the valve is repaired. The leaking valve requires a cage
replacement, which is an estimated 3 month lead time for the new parts. We will continue to log the event daily
to record gas volumes flared and I will follow up with a phone call on Monday

48066 (MSCF). The event lasted for 302 minutes. The event had a volume greater than 5000 (MSCF) and was due to
tandem compressor 1805 shutdown due to a faulty card. Flaring stopped when the equipment malfunction was repaired
and the T/C restarted.

A flaring event occurred at CGF starting at 04/13/2015 01:30 and stopping at 04/13/2015 04:00 with a volume of 430
(MSCF). The event lasted for 150 minutes and occurred when Tandem Compressor 1 was shut down for water -wash. The
event stopped when T/C 1 was depressurized to a zero pressure state.

A flaring event has occurred at CGF starting at 04/01/2015 08:50 and stopping at 04/01/2015 13:52 with a volume of
48066 (MSCF). The event lasted for 302 minutes. The event had a volume greater than 5000 (MSCF) and was due to
tandem compressor 1805 shutdown due to a faulty card. Flaring stopped when the equipment malfunction was repaired
and the T/C restarted.

In reference to the following events:
Facility Dates Amt Flared, MSCFD Reason
1. FS -3 5/26-5/28/15 16226 Planned 1802 T/C down, flaring STV
and IP gas Issues with the machine, blew it down, they struggled with several additional restarts
and finally got 1802 to restart.
2. CGF 4/1/15 48066 Unplanned 1805 T/C down, stabilizers
overpressure to flare This was an unplanned issue and they flared to stabilize the plant, they
were able to reload 1805 and get the plant stabilized to stop the flaring.
3. FS -2 4/1-4/5/15 28957 Planned 1801 T/C shutdown for
planned maintenance This was planned maintenance and to keep the plant from going
completely down during the colder temperatures they had to flare
4. FS -1 8/25/15 18944 Unplanned 1803 T/C shutdown, god
stage nozzles There was problem associated with 2n1 stage nozzle/clevis slop (2%) they Shut in
wells 2-8, 2-34, 2-19, 2-21, 18-11, 2-6, 2-28, 2-22 to stop the flaring.
5. CGF 8/25/15 35569 Unplanned Flared stabilizer overhead
after TAPS outage, Low rate issues coming out of the TAPS outage, They were able to start the
MI Machine which closed the flare valves.
For each event, explain what happened and what measures were taken to mitigate flaring such as shut
in production. If production was not shut in, why not?

A Flaring event occurred at CGF starting at 06/23/2015 19:27 and stopping at 06/23/2015 21:50 with a volume of 3200
(MSCF). The event lasted for 143 minutes and was due to depressurizing the AGI #1 supply line to allow for a line lift. The
event stopped when the depressurizing was completed.
A flaring event occurred at CGF starting at 06/17/2015 16:00 and stopping at 06/17/2015 16:34 with a volume of 1
(MSCF). The event lasted 34 minutes, was a black smoke event and was due to operations personnel blowing down low
point drains during the LTS 2 Derime. The event stopped when the low point drains were closed.

In reference to the following events:
Facility Dates Amt Flared, MSCFD Reason
1. FS -3 5/26-5/28/15 16226 Planned 1802 T/C down, flaring STV
and IP gas Issues with the machine, blew it down, they struggled with several additional restarts
and finally got 1802 to restart.
2. CGF 4/1/15 48066 Unplanned 1805 T/C down, stabilizers
overpressure to flare This was an unplanned issue and they flared to stabilize the plant, they
were able to reload 1805 and get the plant stabilized to stop the flaring.
3. FS -2 4/1-4/5/15 28957 Planned 1801 T/C shutdown for
planned maintenance This was planned maintenance and to keep the plant from going
completely down during the colder temperatures they had to flare
4. FS -1 8/25/15 18944 Unplanned 1803 T/C shutdown, 2nd
stage nozzles There was problem associated with 2"d stage nozzle/clevis slop (2%) they Shut in
wells 2-8, 2-34, 2-19, 2-21, 18-11, 2-6, 2-28, 2-22 to stop the flaring.
5. CGF 8/25/15 35569 Unplanned Flared stabilizer overhead
after TAPS outage, Low rate issues coming out of the TAPS outage, They were able to start the
MI Machine which closed the flare valves.
For each event, explain what happened and what measures were taken to mitigate flaring such as shut
in production. If production was not shut in, why not?

4040 (MSCF). The event lasted for 720 minutes and was due to depressurizing LTS-3 for planned MEOH quill
installation. The event stopped when the Flare valve closed after zero energy, when de -pressuring was complete.
