Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.

Description of Operations
Rig down. Disconnect lines at pits/sub interconnect. Move out
pits. Move Rig from N-O. Spread hurculite over mats in N-11 C
cellar area. Move and spot pits.
Move truck shop, parts house and camp. Install grating and
spill containment around well head in cella, berm around cellar.
Connect pits and sub at interconnect.
Accept rig at 00:00 hrs 8f712004
RIG ACCEPTED @ 00:01 HRS 8f712004. Take on seawater.
RlU secodary annulus valve. Check pressures on IA and OA -
both '0'.
RlU DSM lubricator & Pull BPV. Tubing pressure = '0'
RlU to circulate out freeze protection diesel. Pump 35 bbl
seawater down annulus taking diesel returns up tubing. Switch
and pump 155 bbl hot (120 F) seawater down tubing taking
diesel returns up annulus until clean. Monitor well - static. RID
& blow down circulating lines.
Install two way check valve (TWC). Test from below to 1000 psi
- OK.
N/D Tree & adapter flange. Remove from cellar. Test for
N.O.R.M.= 0.2 TO 0.3 uRemlhr.
Inspect Tubing Hanger. Threads OK. Work LOS. One lock
down screw frozen.- had to grind off end of LOS form insde of
tubing head bowl.
N/U Spacer Spool & BOPE.
Test BOPE to 250 psi. Low & 3500 psi. High. Test all but Lower
Rams. Will test after pulling tubing. This autorized by Lou
Grimaldi, AOGCC, who also waived witness of test.
Blow down choke, choke lines & Top Drive. R/U DSM lubricator
& pull TWC.
MIU landing Jt & screw into Hanger. B.O.L.D.S. Pull Hanger &
string to floor w/140K up wt. Well static. UD Landing jt. &
Hanger.
Circulate bottoms up with seawater @ 7 BPM / 320 psi. No gas
- No solids. RlU Camco control line spooler and hold PJSM

N-11 C Application for Sundry Approval - P&A for Sidetrack Operations
Approval is requested for the proposed P&A of PBU well N-11 B. This horizontal Ivishak Zone 4 producer
has been shut-in since August 2003 due to multiple liner 1 tubing leaks and poor oil production. The
wellbore is providing no production or value to the field as it currently sits. ,/
The original N-11 was drilled to a downthrown block, and P & A'd. N-11A was drilled in 1985, and POP in
1986. 3300 bbls of fluid were lost to a fault in Zone 4. Production from N-11 A was dominated by the
fault, quickly stabilizing at 8 MBFPD, 90% we, and increasing GOR. Please note that N-11A was never
fracced, so GFR is a strong indicator of fault dominance in this case. An attempt to optimize the
completion with a sand plug in 1991 failed to materially impact production.
N-11B was drilled as a U-shaped coil sidetrack in 1999. Severe problems were encountered with lost
circulation in the heel. A loss increase was also noted at a predicted fault in the "belly" of the well. The
well was cemented with partial returns and a liner top packer was set after the liner failed a pressure test.
However, N-11B did penetrate a NLOe of 108' TVD. Initial production was at solution GOR and less than
10% watercut, indicating initial isolation from fault related production. However, a trend of simultaneously
increasing GFR, we and GOR was soon established. Standalone tests in early 2002 were reminiscent of
N-11a with nearly 90% we, and a GFR in excess of 7 MBD.
In May 2003, a caliper survey indicated multiple holes in the tubing, and two parts in the 2 7/8" liner, one at
the window, and the other at one of the liner leaks observed in the earlier PL T. The mechanism for the
parting appears to be corrosion / erosion, since wall thinning can also be observed across from a number
of holes as well.
N-11 B had to be safed out due to T x IA communication, and high SIWHP. Due to leaks in the packer and "
tailpipe, an IBP in the tubing tail was unsuccessful in safeing the well out. The well was finally made safe
by setting a elBP in the liner, and pumping sand slurry down the backside of the liner. A plug is also sitting
on top of the eIBP.
The N-11e low angle well bore is designed to exploit the thick Zone 4B NLOe in a Western fault block on ....
N-pad. The wellbore path is oriented in an East to West direction. The intermediate section will be drilled /
to the top of the Sag River where a 7" liner will be set and cemented. The production section will drop
angle and TD 200' MD below the PWOC. A fully cemented solid liner will be run.
The N-11 e sidetrack is currently scheduled for Nabors 7ES beginning around 08/10104, after completing
the N-01A sidetrack. The pre-rig work for this sidetrack will be priority scheduled immediately after the
Application for Sundry is approved.
N-11 C Application for Sundry
SCODe
Pre-Ria ODerations:
a. Bleed OA, IA and tubing pressures to zero. Test the 9-5/8" pack-off, Function all lOS, repair or
replace as necessary.
b. MIT the OA to 2000 psi.
c. P & A the N-07B lateral wellbore with 8.0 bbls 15.8 ppg cement. RU and RIH with coil tubing to top of
sand plug at 9593'. lay cement from the top of the sand plug at 9593' to 9350'. This equates to - 250'
above top of liner / top of Sand plug and -100' above top of sliding sleeve. The volume to fill the 4-1/2"
tubing to -9350' is -4.0 bbls bbls. The volume of annulus below the production packer between the 9-
5/8" casing and 4-1/2" tubing is -4.0 bbls. This annulus is accessible due to existing tubing punches at
9547-9557'. Wash down and tag to verify TOC is -9350'.
d. CMIT IA and tubing to 3000 psi for 30 minutes. This should be done post coil cement plug.
e. RU Slick-line. Drift the tubing and tag TOe at 9350' with gauge ring / junk basket, POOH.
f. RU E-line. RIH with Gamma Ray, CCl and string shot tool. Perform string shot and log GR from TOC
up, tying into previous jewelry log. GR log should continue to a minimum of 500' above the THRZ.
POOH. RIH and chemical cut tubing -9230', in the middle of joint #6. Gust above first GlM & 10' pup).
g. Circulate the well to seawater. Freeze protect the IA and tubing with diesel to - 2200'.
h. Bleed all pressures to zero. Set BPV. Secure well.
i. Remove the wellhouse. If necessary, level the pad.
Ria Operations:
1. MIRU Nabors 7ES

have damaged threads).
6. RU e-line. Run GRJJB for 9-5/8",47# casing to top of tubing stub.
7. MU 9-5/8", 47# EZSV bridge plug on e-line with CCL. RIH and set just above the top of the tubing stub
- 9250'. POOH for 2nd EZSV bridge plug. RIH and set the 2nd plug at - 8320' MD (Set the bridge plug
, _---.-- at ±8320' to avoid cutting a casing collar with the whipstock mill). RD e-line.
8. PU milling assy. RIH picking up DP to top EZSV. Pressure test the 9-5/8" casing to 3500 psi for 30
minutes. Change well over to 10.9 ppg milling fluid.
9. RIH with 8-1/2" whipstock and milling assembly to EZSV. Orient and set the whipstock.
10. Mill a window in the 9-5/8" casing at ±8,300' MD plus 20' from mid window (KOP is in the Miluveach).
Perform FIT to 12.5 ppg. POOH for Drilling BHA. Verify that the mud weight is at least 10.9 ppg prior
to cutting the window.
11. MU the 8-1/2" drilling assembly with DIRJGRlRES and RIH. Kick off and drill 8-1/2" intermediate
interval, turning at 4.75°/100' to -259° azimuth and holding -45° inclination.
12. Drill ahead to casing point at Top Sag River to an estimated depth of 9,930' MD.
13. Run and cement a 7" drilling liner throughout the 8-1/2" hole and back into the 9-5/8" casing 150' to
8,150' MD. Set and hanger /Iiner top packer and displace cement with mud. POOH.
14. MU 6-1/8" BHA with DIRlGRlRES and RIH to the landing collar.
15. Pressure test wellbore to 3500 psi for 30 minutes.
16. Drill shoe track. Swap over to 8.5 ppg mud and drill 20' of new formation. Perform FIT to 10.0 ppg.
17. Drill the 6-1/8" production interval as per directional plan (dropping to 20° inclination by TSAD) to and
estimated TD of 10,381'.
18. Run and cement a 4-1/2", 12.6#, 13Cr liner throughout the 6-1/8" wellbore and 150' back into the 7"
drilling liner to 9,780' MD. Set liner top packer and displace well to clean seawater above the liner top.
19. Run 4-1/2", 12.6#, 13Cr completion tying into the 4-1/2" liner top.
20. Set packer, test and record the tubing to 3500 psi and annulus to 3500 psi for 30 minutes each.
21. Set and test TWC to 1000 psi.
22. ND BOPE. NU and test the tree to 5000 psi. Pull TWC.
23. Freeze protect the well to -2200'. Set BPV and secure the well.
24. RDMO.

Seal. No pressure response. Well taking 4.0 BPM, no returns. Pump 15 bbls of 50 ppb LCM
(KS coarse, OM, LC); displace to perfs. Losses stopped. Full returns. Mix and pump 8 bbls
15.8 ppg G cement. Lay in cement from top of whipstock. Pull to safety. Sqz cement to 1500
psi.

Finish cement job to abandon perfs. Contaminate cement to 9760'. POOH jetting to surface.
LD cementing BHA. Pressure test cement plug to 1500 psi and stabilized at 1040 psi. Inner
annulus rose from 750 psi to 930 psi and stabilized. Hold for 30 min. Assume leak is by a /}
dummy gas lift valve. RIH with milling BHA (3.80" dimple mill). Mill XN nipple. RIH and found If
firm cement 3' above whipstock. Mill window starting at 9897'. Can't stall motor. POOH to 'tel:/ "ì

junk. Verify tie-in below tbg tail, close. Attempt to tag top of whipstock (9894') with low side tool
face and was clean to 9919'. POOH. LD drilling BHA. RIH with milling BHA (3.80" dimple mill).
Correct to flag. RIH and tag at 9917'. Mill to 9918' and stacked with no stall. PU, RIH, and
gained another 5' tagging at 9923'. Mill steady to 9929'. Significant metal over shakers. Slow

aggregate, 2 neat). Hold 100 psi backpressure wlchoke when one barrel cement out nozzle.
Start up hole at 25 fpm while pumping 0.5 bpm. Increase squeeze pressure to 300 psi across
loss zone. Continue uphole holding 300 psi backpressure to 9700 psi. Release back pressure
and circulate excess cement out of hole. Apply squeeze pressure of 450 psi. Release pressure
and wash in hole to 9950'. Circulate and POOH. Jet out BOP stack. PU Drilling BHA, TIH.
Tagged at 9950'. Cleaned out to 10027'. Took wt fl 10027-35'. Started losing mud at 10028'
(0.5-1.0 bpm). Reamed to 10083'. 100 ft wiper trip. Pulled heavy at 10030-20'. Reamed & drld
to 10100'. 100 ft wiper trip-no tight spots. Reamed & drilled to 10126'. POH to 9780'. WO on
ISW. RIH. Drilled to 10253' wi SW & Flo-Vis sweeps, losing 0.3-.5 bpm.

POH. Changed motor fl 2.8° to 2.0°. RIH wi new M-09 PDC bit. Drilled t01 0455' wi SW & Flo-
Pro sweeps. Wiper trip to window. Drilled t01 0475' wi SW & Flo-Pro sweeps. Running Iowan
seawater due to poor weather conditions. POH to 9700' (Above cmt ramp). Weather conditions
at Phase III. Wait on weather to improve. Filled well wi sw until 100 bbls left in pits. SI well &
monitor for pressure while WO weather to improve to Phase 2.

WO weather to improve to Phase 2. Monitoring Coil and annulus pressure. Coil freeze protected
wi methanol. Recovered MEOH from coil. Circulate out gas from well through choke and Poor
Boy de-gasser. Total time Waiting on Weather was 21 hours. Orient TF. RIH to TO of 10,475
ft. Drilled to 10,602 ft with seawater and 15 BBL FloPro sweeps every 30 ft drilled. POOH for
1.0 degree bent motor. Change out OS CT connector, Sperry MWD, Motor and Bit. RIH with
drilling BHA #20 [0.9 deg. bent motor and DS49A4 Bit #9. Shallow test MWD and cont. RIH.
Orient TF to 45R. RIH thru window and log GR Tie-in with BHCS/GR, Added 2 ft to CTD. RIH
to TO of 10,602 ft. Drilled to 10,665 ft. Difficult drilling for 5 ft. Lost additional returns. Drilled to

rubber. Shear released from packer at 3200 psi. Pressure tested liner and liner top packer with

tubing to surface with 20 BBLS of MEOH. Closed swab valve and blew down CT with Nitrogen.
Bled down pressure in coil. Cut off CTC and pulled stub of CT to reel and secured coil. Clean
rig pits. N/D BOPE. Released Nordic #1 rig from N-11 B at 06:00 Hrs, 11-21-99. Total time on
well = 16 Days and 4 Hours.

The permit to rcdrill does not exempt you from obtaining additional permits required by law from
other governmental agencies. and does not authorize conducting drilling operations until all other
required permitting determinations are made.
In accordance with 20 AAC 25. 005(f), all records, data and logs acquired for the pilot hole must
be clearly differentiated in both name (N-ll BPH) and API number (50-029-21375-70) from
records, data and logs acquired for well N -11 B.
Blowout prevention equipment (BOPE) must be tested in accordance ,Ùth 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allo,,- a representative of the
Commission to witness a test of BOPE installed prior to drilling ne,," hole. Notice may be given
by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

N-11 b Sidetrack
Summary of Operations:
N-11 b is currently shut in due to high water cut. The sidetrack, N-11 b, will target Zone 4A and 4B
reserves. This sidetrack will be conducted in two phases.
Phase 1: Set Whipstock: Planned for Nov. 5, 1999.
· A Tubing Integrity Test will be performed.
· The whipstock drift and Kinley caliper will be run.
· A mechanical whipstock will be set on e-line at approx. 9900' MD.
Phase 2: Cement perfs, mill window and drill pilot hol~and lateral: Planned for Nov. 10,
1999. ~---
Directional drilling coiled tubing equipment will be rigged up and utilized to cement
perforations, mill the window and drill the sidetrack to a planned TD of 11,212' MD
(8,637' TVDss).
Mud Program:
· Phase 2: Seawater and Flo-Pro (8.6 - 8.7 ppg)
Disposal:
- No annular injection on this well.
- All drilling and completion fluids and all other Class II wastes will go to Grind & Inject.
- All Class I wastes will go to Pad 3 for disposal.
Casing Program:
- 27/811,6.16#, L-80, ST-L liner will be run from TD to approx. 9,585' MD and cemented with
approx. 17 bbls. cement to bring cement 200 ft above the window. The well will then be
perforated with coiled tubing conveyed guns.
Well Control:
- BOP diagram is attached.
- Pipe rams, blind rams and the CT pack off will be pressure tested to 400 psi and to 4000 psi.
- The annular preventer will be tested to 400 psi and 2000 psi.
Directional

- Kick Off: 9,910' MD (8,488' TVD ss)
- TD: 11,212' MD (8,637' TVD ss)
- 'Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
