GeNewLL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Submit a well
schematic diagram with each 10-407 well completion report and 10-404 well sundry report when the downhole well design is changed.
IreM 1 a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Saft Water Disposal, Water Supply for Injection,
Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely
segregated. Each segregated pool is a completion.
Iren~ 4b: TPI (Top of Producing Interval).
ITeM 8: The Kelly Bushing and Ground Level elevation in feet above mean sea level. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
IreM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).

7/10/2009 T/I/0=0/60/80 Temp=Sl Assist E-Line, MIT-T PASSED to 3000 psi (Pre CTD) Pumped down Tbg with
7 bbls of 50° F diesel to reach test pressure. Tbg lost 160 psi in the ist 15 mins and 40 psi in the 2nd 15
mins for a total pressure loss of 200 psi in a 30 min test. Bled back --5 bbls. Final W HP's=200/70/80. E-
Line on well at departure.
7/10/2009 CONTINUED RIH WITH PLUG SETTING TOOL STRING: MAX OD = 2.18", LENGTH = 26'. ALL
SERVICES TIED INTO SCMT LOG DATED 6-JUNE-2004. SET HES EZSV BRIDGE PLUG AT 12410' IN
THE MIDDLE OF A FULL JT. RIH WITH SCMT TO EVALUATE TOC FOR CUTTING 2 7/8" TUBING.
CEMENT BOND LOG RUN FROM PLUG SET DEPTH TO ABOVE LINER TOP. FREE PIPE FOUND
ABOVE 12350'. CUT 2-7/8" TUBING AT 12325'. NOTIFY PAD-OP OF VALVE POSITIONS. FINAL
T/IA/OA: 0/0/0. WELL LEFT SHUT-IN.
7/13/2009 PRESSURE TEST TREE PRE CTD. IN A 15 MIN TEST VALVES HELD Q 600# AND C~3 5000# VALVES
LOST 155# THRU EITHER THE WING OR MASTER VALVE. SWAB HELD PRESSURE. TREE
PASSED.
7/16/2009 T/I/O = 280/50/50. Temp - SI. Assist Cameron (LDS drill out). IA & OA FLs near surface. Bled IAP &
OAP to 0 psi in 10 min (all gas). Held open bleed for Cameron, LDS drill out & PPPOT-IC. SI, monitored
for 30 min. IAP & OAP increased to 0+ psi. Final WHPs = 280/0+/0+. SV, WV = C. SSV, MV = O. IA,
OA = OTG. NOVP.
7/17/2009 T/I/0=280/0/0, Temp=Sl. PPPOT-IC (FAIL), Drill out broken LD5 (casing head).
DHD bled IAP & OAP to 0 psi. Stung into IC test void to verify 0 psi. RU Wach's drill, drilled out
remaining LDS nose. Miscalculated depth of cut and drilled into packoff and mandrel hanger. Rigged off
weTrsecured with new LDS assembly, attempt PPPOT-IC (FAIL) unable to pressure test void or get
returns from slotted bleeder screw. Re-energized secondary Y seal with plastic and torqued LDS to spec
(600 Ib/ft). Still unable to get test. Notified WIC of situation.
7/18/2009 T/I/0=340/80/10 Temp= SI MITIA PASSED to 3000 psi. Pumped 11 bbls diesel down IA to achieve test
pressure. IA lost 40 psi in ist 15 mins and 20 psi in 2nd 15 mins for a total loss of 60 psi in 30 mins.
Monitored well for an additional 30 mins. IAP maintained 2940 psi. OA did not pressure up during test
on IA and did not notice any leaking around LDS. Bled IAP. Bled back (- 8 bbls). Final WHPs
340/100/10 WV, SV = Shut SSV, MV, IA, OA = Open.
7/25/2009 T/I/O = 80/140/20. Temp = SI. Bleed WHPs to 0 psi pre RWO. TBG FL near surface. IA & OA FLs C~
surface. Bled OAP from 20 psi to 0 psi in 20 min. Bled IAP from 140 psi to 0 psi in 30 min. Bled TP from
80 psi to 0 psi in 15 min. SI and monitor for 30 min. WHPs unchanged. FWHPs = 0/0/0. SV, WV, MV
= C. SSV = O. IA, OA = OTG. NOVP.

Sundry approval is requested for C-18A. Isolation of the existing perforations with a cast iron bridge plug
followed by a new coil tubing drilling sidetrack is proposed. The CTD side track will be a high angle step-out well
to target Z1 A/1 B reserves in the Mid-Field graben south of DS 18.
WELL HISTORY/CURRENT STATUS:
C-18A was coil sidetracked in Jan 2000 with 1000' drilled in Zi B and 443' initially perforated. During the original
completion, the tubing-conveyed perforation guns became stuck after firing and were never successfully fished
out of the well. In April 2000 the well was kicked off with an N2 lift because the well lacks GLMs. The well
produced full-time, following the initial rate decline until it reached MGOR in late 2000. In January 2001, 210' of
Z1 B perforations were added in the heel of the well, resulting in ~1 month rate buzz before reaching MGOR and
cycling.
In December 2002, 30' of adperfs were shot uphole from the January 2001 adperfs; however, these adperfs
resulted in no benefit. A third set of adperfs were added to this well in May 2005, which included re-perforating a
portion of the Dec 2002 adperfs. There was an increase in oil production, but it did not sustain for long and the
well went back to cycling with diminishing ontime.
A July 2006 STP/MCNL did not clearly identify any adperf or GSO opportunities, although there was slight
cooling (1 °F) at the base of the 13020-13050' perf interval. Because the well had very low ontime, an end-of-life
CIBP was set for a gas shut-off at 13000' MD on 9/10/08, allowing only the top set of perfs to produce. The plug
cut both oil and gas rates. The GOR remained on the same trend, and the well continued to cycling until it no
longer healed and was shut-in for high GOR in April 2009.
A SCMT log run on 6/6/04 indicated the top of cement behind the 2-7/8" liner was at ~12350' MD, which is below
the TSGR allowing for a cut-and-pull of the liner for a big coil sidetrack. During CTD screening in March 2009,
the well passed a MIT-IA to 3000 psi and a MIT-OA to 2000 psi. The PPPOT-T & IC passed on 6/5/09. A
caliper log run 2/15/09 indicated the tubing is in fair condition with a maximum wall penetration of 39%, and the
liner is in poor condition with a hole at ~11655' MD and an isolated pit with 77% penetration at 11637' MD.
REASON FOR SIDETRACK:
C-18A is not a competitive well, exhibits minimal recharge, and has low remaining value. Plan to
isolate perforations with CIBP and primary cement job.

up to window to get clicks. Landed at 12982'. POOH for motor. Change bit, motor and MWD probe.
RIH with BHA #4 (1.1° motor). Precautionary ream to TD. Drill horizontal from 12982-13000'. Still have
to go to window to get clicks.

Orient TF and RIH. Drilled to13,100 ft and oriented to left side. Drilled to 13,106 ft and started stacking
wt. Made wiper trip to window. Tied-in with RA tag and added 9 ft to CTD. RIH and drilled to 13,220 ft.
Short trip to window, hole in good condition. RIH and drilled horizontal hole to 13,328 ft. Wiper trip to
window. RIH and drilled to 13,455 ft. Wiper trip to window. Tied-in with RA tag and added 6.5 ft to
CTD. Drilled horizontal at +8770' TVD to 13644'. Wiper to window. Drilled horizontal to 13820'. Wiper
to window. Add 6.0'. Orient to right. Drill horizontal to 13943'. Wiper to window. Drill horizontal to
13960' (8972' TVD). POOH to change BHA.

Tied-in with RA tag, added 8.5 ft to CTD and flagged CT. POOH for BHA change. Changed out bit,
motor, orientor and MWD tools. Cut off 60 ft CTD due to high fatigue spike at window and M/U new
CTC. RIH with BHA #5. Shallow tested MWD and orientor - OK. RIH. Oriented TF to 20L above
window, only getting 10 deg. clicks with second new orientor. Sat down in window 3 times before falling
thru. RIH to TD of 13962 ft. Hole in good condition. Orient TF to high side and drill to 14004 ft while
swapping to new FIo Pro mud. Orient and orientor wouldn't stay locked. POOH to change orientor.
Change orientor. RIH with BHA #5rr. Drill horizontal from 14004-14185' at 8763-8771' TVD. Wiper to
window. Tie-in, add 4.0'. RIH

MU Baker CTC and CTLRT. RIH with 96 jts 2-7/8" 6.16# L-80 STL liner on CT to TD. Had to work to
get last 50' due to shales. Drop ball and release from liner. Displace mud to 2% KCI water. Hole began
packing off after KCI water rounded shoe. DS cementers Icad CT with 24 bbls Class G cement with
latex. Load wiper dart. Displace with 2% KCL water to latch collar losing 0.5 bpm. Unsting from liner.
POOH. Run in hole w/2.25" nozzle and 1 ~A" CS Hydril pipe. Wash into liner to latch collar at 14378'.
POOH, stand back 1 ~,4" CS Hydril.
01/28/00 8 14451'
Finish stand back CSH workstring. MU SWS memory CNL. RIH with 46 stands CSH. RIH with CT.
Log at 30'/min from 12500-PBD. Leave new FIo Pro mud in liner. POOH with CT. Stand back CSH.
LD CNL tool. Pressure test liner at 2200 psi for 30 min. MU per[ guns. TIH, tag up on latch collar.
Correct depth to latch (reference CNL Memory Icg of 1/27/00). Pull uphole and position bottom shot at
14250', drop 5/8" firing ball and displace w/2% KCI water. Guns fired 7.5 bbl early. Lost 5 bbl fluid in +/-
5 minutes when guns fired. Attempt to POOH, pipe stuck. Suspect Iow-order detonation and swollen
guns. Work pipe to 80,000#-no progress. Spot 50 bbl FIo-Pro to increase lubricity. Work pipe-no
progress. Drop 3A" dis-connect ball. Displace w/2% KCI water.

Disconnect from perf guns (top at 13563'). POOH with CT. Stand back CSH workstring. MU Baker
fishing assy (GS, jars, wt bars, accelerators). RIH with work string. RIH with CT to fish. Latch fish. Jar
up and down and no movement. Pull maximum and dragged uphhole 8' slowly and stopped. No further
progress. POOH with CT. Stand back CSH work string. LD Baker fishing tools. MU nozzle. TIH and
freeze protect tubing to 2000' w/MeCH. ND BOP, clean pits. Release Rig 01/28/00 23:30 hrs.

Friday, January 21~ 2000: I traveled to BPXs PBU C Pad to witness the initial BOPE
test on Well C-18A being drilled by the Dowell Schulumberger / Nordic 1 coil tubing
drilling unit.
As the attached AOGCC Test Report indicates the test lasted 5 hours and there was 1
failure. One test on the choke manifold (4 downstream valves) failed to hold pressure.
An individual leaking valve was not identified, instead the entire choke manifold was
serviced and testing resumed with no further failures. In addition to servicing the choke
manifold several problems were encountered with the test pump adding to the over all
test time.
The Rig was clean and all related equipment appeared to be in good condition.
SUMMARY: I witnessed the initial BOPE test on Nordic Rig 1 drilling BPXs C-18A Well.

The permit to redrill does not exempt you from obtaining additional permits required by law
from other governmental agencies, and does not authorize conducting drilling operations until all
other required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

C-18a Sidetrack
Summary of Operations:
C-18 is being sidetracked to targe.t Zone 1 reserves. This sidetrack will be conducted in two phases.
Phase 1' Set Whipstock, squeeze perfs: Planned for Jan. 5, 2000. · A Mechanical Integrity Test was successfully performed on 11/19/99.
· The whipstock drift and PDS caliper were run on 12/14/00
Phase 2: Mill Window, Drill & Complete sidetrack: Planned for Jan. 12, 2000.
Directional drilling coiled tubing equipment will be rigged up to mill the window then drill and complete
the directional hole as per attached plan to planned TD of 14,846' MD (8,774' TVDss).
Mud Program: · Phase 1' Seawater
· Phase 2: Seawater and FIo-Pro (8.7 - 8.8 ppg)
Disposal: · No annular injection on this well.
· All drilling and completion fluids and all other Class II wastes will go to Grind & Inject.
· All Class I wastes will go to Pad 3 for disposal.
Casing Program:
· 2 7/8", 6.16#, L-80, ST-L liner will be run from TD to approx. 11,576' MD and cemented with approx.
25 bbls. cement to bring cement 200 ft above the window. The liner will be pressure tested to 2200 psi
and perforated with coiled tubing conveyed guns.

Pipe rams, blind rams and the CT pack off will be pressure tested to 400 psi and to 4000 psi.
The annular preventer will be tested to 400 psi and 2000 psi.

· Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging · A Gamma Ray log will be run over all of the open hole section.
· Open hole will be logged with Memory Induction log.
Hazards
No faults or lost circulation are expected. Res. pressure is normal.
Reservoir Pressure
Res. pressure is 3400 @ 8800ss. Max. surface pressure with gas (0.12 psi/fi) to surface is 2344 psi.
