It appears that we are needing a permit extension on the 3R-108 well in Kuparuk. This
permit was applied for and approved in October of 2019. Unfortunately with the Covid
pandemic hitting in early 2020 and our subsequent shutting down of operations, we
are approaching the 2 year limit for the PTD. We will be picking up a rig early next year
to begin drilling in Kuparuk again, with the 3R-108 well on the rig schedule to be spud
around September 2022.
Could you please advise if there is any additional paperwork that we need to provide,
to extend this permit to be used next year?

Enclosed is the approved application for the permit to drill the above referenced service well.
Per Statute AS 31.05.030(d)(2)(B) and Regulation 20 AAC 25.071, composite curves for well logs
run must be submitted to the AOGCC within 90 days after completion, suspension or abandonment
of this well.
This permit to drill does not exempt you from obtaining additional permits or an approval required
by law from other governmental agencies and does not authorize conducting drilling operations
until all other required permits and approvals have been issued. In addition, the AOGCC reserves
the right to withdraw the permit in the event it was erroneously issued.
Operations must be conducted in accordance with AS 31.05 and Title 20, Chapter 25 of the Alaska
Administrative Code unless the AOGCC specifically authorizes a variance. Failure to comply
with an applicable provision of AS 31.05, Title 20, Chapter 25 of the Alaska Administrative Code,
or an AOGCC order, or the terms and conditions of this permit may result in the revocation or
suspension of the permit.

Conditions of Approval:
Approval is granted to run the LWD-Sonic on upcoming well KRU 311-108 with the following provisions:
1. CPAI will provide a written log evaluation/interpretation to the AOGCC along with the log as
soon as they become available. The evaluation is to include/highlight the intervals of competent
cement (and lengths) that CPAI is using to meet the objective requirements for annular
isolation, reservoir isolation, or confining zone isolation etc. Providing the log without an
evaluation/interpretation is not acceptable.
2. LWD sonic logs must show free pipe and Top of Cement, just as the a -line log does. CPAI must
start the log at a depth to insure the free pipe above the TOC is captured as well as the TOC.
Starting the log below the actual TOC based on calculations predicting a different TOC will not
be acceptable.
3. CPAI will provide a cement job summary report and evaluation along with the cement log and
evaluation to the AOGCC when they become available
4. CPAI will provide the results of the FIT when available.
5. Depending on the cement job results indicated by the cement job report, the logs and the FIT,
remedial measures or additional logging may be required.

Dear Commissioner:
ConocoPhillips Alaska, Inc. hereby applies for a permit to drill an onshore grassroots injector well from the 3R pad. The
expected spud date for this well is November 28, 2019. It is planned to use rig Doyon 142. '
3R-108 is planned to be a horizontal injector in the West Sak D sand. The surface section will be drilled with a 13-1/2"
bit and cased with 10-3/4" casing to 12,307' MD / 2,171' TVD. The intermediate section will be drilled with a 9-7/8" bit
and cased with 7-5/8" casing to f8,178'MD / 3,675' TVD. The production section will be drilled with a 6-3/4" bit in the
West Sak D sand +/- 15,696' MD / 3,965' TVD and lined with 4-1/2" ICDs approximately every 500' MD, isolated with
6-1/8" swellpackers and 4-1/2" 12.6# blank pipe. The upper completion will be run with 4-1/2" 12.6# tubing.
Please find attached the information required by 20 ACC 25.005 (a) and (c) for your review. Pertinent
information attached to this application includes the following:

It is requested that a variance of the diverter requirement under 20 AAC 25.035 (h)(2) is granted. At 3R,
there has not been an indication of shallow gas or gas hydrates through the surface hole interval. The
previously drilled West Sak wells 3R-101, 3R-111, and 3R-106 are close offsets, with neither well
encountering shallow gas. The referenced wellbores are 410', 192' and 172' from the proposed base of
permafrost location in well 3R-108.

The proposed drilling program is included below. Please refer to Attachment 4 for proposed well
schematic.
1. Move in / rig up Doyon Rig 142. Install Annular with 16" diverter line and function test. Notify AOGCC 24 hrs
before test.
2. Spud and directionally drill 13-1/2" hole to surface TD as per directional plan. Run GWD/MWD/LWD tools
[gamma ray & resistivity] required for directional monitoring.
3. Run and cement 10-3/4", 45.5#, L80, HYD563 casing to surface. Displace cement with mud and check floats
upon bumping plug. Notify AOGCC if top job is required.
4. Remove diverter system and install wellhead. Install and test 13-5/8" x 5,000 psi BOP's to 250 psi low and 3,500
psi high (annular preventer to 3,500 psi). Notify AOGCC 24 hrs before test.
5. PU 9-7/8" bit and 6-3/4" drilling assembly, with MWD/LWD [gamma ray & resistivity]. RIH, clean out cement
to top of float equipment and test casing to 3,000 psi for 30 min.

evaluation)]. Log top of cement. Clean out cement to shoe track and test casing to 3,500 psi for 30 min.
14. Drill out shoe track and 20' of new hole. Perform shoe test to a minimum of 11.5 ppg EMW LOT and maximum

15. Directionally drill to production TD.
16. Circulate hole clean. BROOH to intermediate shoe.
17. Displace cased hole to 9.2 ppg solids free WBM.
18. POOH on elevators and LD 6-3/4" BHA.
19. P/U and run 4-1/2", 12.6#, blank liner, ICDs, swell packers, and liner -top packer.
20. Displace wellbore to 9.2 ppg filtered brine.
21. Pump formic acid and allow to soak for 6 hours.
22. Displace formic acid with brine and pump second formic acid job. Allow to soak for 6 hours.
23. Displace formic acid with brine.
24. Drop WIV ball and land. Close WIV and set liner hanger.
25. Set liner -top packer and pressure test for 10 minutes, chart results.
26. Spot CI brine to lowest most GLM.
27. POOH and lay down running tools.
28. Pull wear bushing & run 4-1/2" 12.6# upper completion tubing.
29. Land hanger, run in lock screws, and test seals to 5,000 psi.
30. Set BPV and test in the direction of flow.
31. Nipple down BOPE, nipple up tree and test. Pull BPV.
32. Freeze protect 4.5" x 7.625" annulus
33. Set BPV and move rig off.
Discussion of Mud and Cuttings Disposal and Annular Disposal
Requirements of 20 AAC 25.005 (e)(14)
An application for a Permit to Drill must be accompanied br each of the folio",ing items, except fm an item already on file with the commission and
identified in the application:
(14) a general description of how the operator plans to dispose of drilling mad and cuttings and a statement of xdtether the operator intends to
request authorization under 20 ,IAC 2 i.080 for an annular disposal operation in the well.;
Waste fluids generated during the drilling process will be disposed of by hauling the fluids to a KRU Class II disposal well
(DS 113 CRI), Milne Point, Prudhoe DS-4. All cuttings generated will be hauled to the DS 1B CRI, Milne Point, Prudhoe Bay
Grind and Inject Facility (for temporary storage and eventual processing for injection down an approved disposal well), or
stored, tested for hazardous substances, and (if free of hazardous substances) used on pads and roads in the Kuparuk area in
accordance with a permit from the State of Alaska.
