This discrepancy was successfully discovered on the required Miscellaneous Test within the required time period and the well was immediately shut in, pilot replaced, and pilot retested and passed today. Investigation revealed that we had an incorrectly set pilot installed on this well that was not identified when the pilot was cycled prior to bringing the well on line on March 3rd, so we began an LCA on this event today. Initial findings are that we had conflicting set point labeling on this pilot, and the operator incorrectly identified the set point when it was installed. We will also be working to clarify what steps we will take when function testing the SVS system prior to bringing wells on line. While unhappy that this event occurred, we are pleased that our systems correctly identified the well to be tested, and our procedures for the required performance testing correctly identified the issue. Box 100360 J\nchorage,AJ( 99510-0360 Dear Mr. Wheatall: The Alaska Oil and Gas Conservation Commission ("Commission") received a well safety valve system (SVS) test report from ConocoPhillips at J(uparuk River Unit 2H pad ("J(RU 2H") dated November 10, 2004. Attached is a copy of the SVS Test Report. Regulation at 20 AAC 25.265 requires all wells to be equipped with a Commission approved fail-safe automatic surface SVS capable of preventing uncontrolled flow by shutting off flow at the wellhead and shutting down any artificial lift system where an overpressure of equipment may occur. If installed, a subsurface safety valve must be maintained in working order and is subj ect to performance testing as part of the SVS. AnSVS component failure rate of 11.1 percent was identified during the above-noted inspection. Commission policy, as outlined in a letter dated November 14, 1995, states that the testing frequency will be increased when a failure rate exceeds 10 percent. The AOGCC-AOGA Safety Valve System Task Force report issued June 13, 2003, supported retention of the 10% failure threshold. Effective November 10,2004 and until an acceptable level of performance is achieved, the safety valve systems associated with wells located on J(RU 2H must be tested every. 90 days. Surface pressure is calculated with an unexpanded gas bubble at surface, including temperature effects. Selected intervals will be logged. The well will be completed with 3-1/2", 9.2#, J-55, 8rd tbg.
