Each of these wells was found to have a void in the conductor. These voids were filled with cement if needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. The cement was pumped August 15~' and 23`d 2009, and the corrosion inhibitor/sealant was pumped August 27~', 28;1i and 29~h 2009. T'he attached spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. The treatment consisted of 295 BBLS of gelled diesel as pad and carrying fluid and 750 LBS of 100 mesh and 14,280 LBS of 20/40 sand as proppant. 61 BBLS of gelled diesel was used as a flush. If coring is conducted, a brief description of each core and a one cubic inch size chip from each foot of recovered core is required. Samples of drill cuttings and a mud log are not required. A continuous directional survey is required as per 20 AAC 25.050(b)(5). If available, a tape containing the digitized log information shall be submitted on all logs for copying except experimental logs, velocity surveys and dipmeter surveys. Many rivers in Alaska and their drainage systems have been classified as important for the spawning or migration of anadromous fish. Operations in these areas are subject to AS 16.05.870 and the regulations promulgated thereunder (Title 5, Alaska Administrative Code). A 20", 2000 psi annular diverter will be installed on the 16" conductor while drilling the surface hole. The 9-5/8" casing will be set through the West Sak Sands at approximately 4656'MD (3800'TVD). A 13-5/8", 5000 psi, RSRRA BOPE stack will be installed on the 9-5/8" surface pipe. It will be tested upon installation and weekly thereafter. Test pressure will be 3000 psig (3000 psi for annular). Expected maximum surface pressure is 2856 psi. Pressure calculations are based on latest reservoir pressure. Surface pressure is calculated with an unexpanded gas bubble at surface, including temperature effects. Selected intervals will be logged. The well will be completed, using the workover rig, with 2-7/8", i6~J-55, EUE 8rd tbg.
