GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in
Alaska.
ITEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
ITEM 4b: TPI (Top of Producing Interval).
ITEM 8:
The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).

function/pressure test. AOGCC test witness waived by Jeff
Jones. Meanwhile fill rig pits with 2% KCL-water.
20:15 - 21:00 0.75 BOPSUPP PRE Check well head pressure. Well head pressure is zero. Stab
injector onto well and circulate MeOH out of coil. Took 36 bbls
to fill the hole.
21:00-21:15 0.25 BOPSUF P PRE Unstab injector and pull back.
21:15 - 21:30 0.25 WHIP P WEXIT PJSM for PU whipstock.
21 :30 - 23:00 1.50 WHIP P WEXIT SWS install 3 RA tags in WS. MU Baker whipstock, setting

3/19/2004 00:00 - 02:00 2.00 WHIP P WEXIT RIH to 7,300'. Not getting returns while RIH. Circulate at 1
bpm down coil. Not able to get returns at 1 bpm. Fill hole
through kill line. Took 10 bbls to fill the hole. Continue RIH
pumping at .58 bpm, no returns. RIH to 9,310'.
02:00 - 02:40 0.67 WHIP P WEXIT Tie in to Sag River marker. +6' correction.
02:40 - 03:00 0.33 WHIP P WEXIT RIH to 10,189'.

Discrepency with MWD time/Depth file on 2nd tie in. Relog
from 10,189' to 10,209'. -5' correction. No retums during tie in
logs.
04:15 - 04:50 0.58 WHIP P WEXIT Place whipstock top at 10,185'. Increase pump rate to 1.87
bpm, 4,200 psi. Bleed off pressure and let sit for 1 minute.
Bring pump up to .9 bpm, 1,500 psi. FAIV is not shifted. Got a
click, TF=147L. Bleed off pressure and let sit for 5 minutes.
Bring up pump to 1.24 bpm, 2,205 psi. TF=125L.
04:50 - 05:10 0.33 WHIP P WEXIT Orient to 123R at 1.2 bpm.
05:10 - 05:20 0.17 WHIP P WEXIT Pump at 1.89 bpm, 4,750 psi. Bleed off pressure. Bring up
pump and set whipstock. Whipstock sheared at 2,100 psi. TF
after shearing off WS is 145R. Shut down pump and set down
2K. PU=33K.
05:20 - 06:00 0.67 WHIP P WEXIT POOH. Hanging up in jewelry while POOH. Pull through
nipples at 9,335' and 9,316' with 41K Ibs. POOH.

seeing returns after pumping 26bbls. Continue RIH to 9500'
(below tbg tail) circ @ 1.2I0.2bpm & 850psi. Up wt check @
9500' - 31.5klbs. PUH into tbg tail to 9175' (above all
X-nipples) - no a/pulls. Continue RIH to WS pinch Pt tagged
2x @ 10157'. Correct CTD (+30') to 10187'. SLB mech depth
counter is 34' off while Sperry counter is 13' off. Review tie-in &
reconfirm W/S tag depth. PUH to 10175' & flag CT.
PJSM with all personnel concerning cementing operations.
~p p bbls of water ahead. PT cement lines to 4k. Pump
39bb of 15.8ppg class G cmt slurry @ 1.95/0.65bpm & -
psi. Follow with 5bbls of water behind. Chase with
KCL-water @ 1.5/0.60bpm & 1550psi. Pump 1 bbl of cmt
around nozzle. POOH laying in remaining cmt @ 1:1 to 8667' -
(TOC). Observe retums gradually improve to 1:1 @
1.47/1.24bpm & 1000psi while laying-in cmt. CIP @ 12:30.
PUH to 7500'.
SI choke & increase WHP to 1000psi. Hesitate-squeeze from
1000psi - 1500psi. Gradually increase squeeze pressure to
2000psi. Squeezed away ca. 1.5bbls cmt.
Circ down 30bbls bio-KCL water to nozzle @ 2.2I2.2bpm &
holding 1000psi WHP.
RBIH contaminating cmt @ 1:1 & 500psi WHP. Observe slight
CTP bump @ 8840' (TOC). Continue washing down to 10175'.
Wash up to 8800' @ 2bpm/3500psi. Start getting contaminated
cmt on surface @ 8840' btms up. Out wt climb up to a max of
12ppg.
Make additional 2 down & 1 up wash passes @
2.0bpm/2700psi.
Jet COH @ 2.0bpm/27oopsi. Tag up with 36.6' CTD on SLB
depth counter - BHA length '" 10.84'.
COH. UD cementing nozzle BHA#2.
MIU swizzle stick. Stab on injector & jet BOP stack. Flush
surface lines.
Pull back injector. LD swizzle stick.
PJSM for PU window milling BHA.
PU window milling BHA.
Stab injector.
RIH to 10,100'.
Pressure up well bore to 1000 psi. Pressure bled off to 500 psi
in 2 minutes. Bleed off pressure. Open up well and circulate
at 1 bpm. Getting full retums. SI well and retest with same
results. Discuss with Anchorage.
RIH to mill window. Tag at 10,161.5', PU and retag.
Mill to 10,161.7'. Stalled motor, pinch point. Correct depth to
10,187'.
TIme mill window from 10,187' to 10,188'.

Description of Operations
Continue time milling window. Motor stalls at 10,192.5'. Break
into formation at 10,192.6'. Mill formation to 10,203'. Tentative
window depths are: TOWS = 10185', TOW = 10187'; BOW =
10193', RA PIP Tag = 10192'.
Backreamlream window/rat hole several times. Attempt to drift
window 2x without pumping - stacking out @ 10187' (TOW).
Reamlbackream TOW area without motorwork. Drift again &
stack out @ TOW. Set down 3k. Roll pumps & fall thru window.
SID pump & drift rest of window/rat hole OK. PUH dry - no
a/pulls. Suspect WS ramp may have been slightly shaved
while milling LS window & we're currently stacking out @
shaved area. Otherwise window looks clean.
Continue drifting/reaming window while swapping well bore to
reconditioned 8.6ppg flopro mud. Meanwhile Phase-1 declared
in all GPB main roads & pads.
Paint EOP flag @ 10000' & POOH window milling BHA#3.
COHo PJSM. SIB injector. Flush BHA. UD 2jts of 2-1/16' CSH
tbg, motor, diamond string reamer & window mill. Window mill
gauge 2.73' go / 2.72' no-go & center wear indicates mill
crossed lor wall. String reamer gauge 2.74' go / 2.73' no-go.
Held pre-spud operations/safety meeting. Reviewed well plan,
potential hazards & BHA M/U procedures. Also reviewed safety
& emergency procedures for handling Sperry MWD lithium
batteries.
M/U & lock tite drilling BHA #4 with used HYC DS100 2.7' x 3'
bicenter bit, 2deg BHI Xtreme motor, Sperry GR-DIR MWD,
orienter & BKR MHA (OAL = 59.58'). Scribe tools. M/U injector.
RIH drilling BHA #4. SHT MWD & orienter @ 1000' - OK.
Continue RIH. Correct +27' @ the flag. RIH dry & tag window
@ 10190'. PUH & orient TF from 20L to 100R. RBIH & set
down again @ widow. Orient to 136R & RIH clean thru window
with pumps off. Tag TD @ 10206'. Correct CTD to 10203'.
Orient TF to 174L.
Resume drilling with LS TF, 1.55/1.55bpm, 3350/35OOpsi,
2Ofph ave ROP. Drill to 10213' & see hi GR spike @ 1
PUH to 9924' for tie-in log. GPB now in Phase- all main
roads & Phase-2 on all pads & access r
Log down GR & tie-into formatio er @ 9945' - subtract
26' from CTD.
RIH clean thru win

Drill 215' with LS TF, 1.55/1.55bpm, 3350/3700psi, 2Ofph
e ROP. See RA PIP Tag @ 10168'. Confirmed window
depths are: TOWS = 10161', TOW = 10163'; BOW = 10169',

Orient TF to 20L.
Drill to 10,263'.
POOH to PU 1st lateral drilling assembly.
Pull back injector. Change bit and motor bend to 1.1 deg.

of a mechanical whipstock, E-39A will build from Zone 1 kick off point, to a Z1 B target. A cement ./
downsqueeze will P&A the parent well perfs.
The following describes work planned to prep for sidetracking and is submitted as an attachment to Form 10-
403. A schematic diagram of the proposed sidetrack is attached for reference.

5. Pressure test squeeze to 1000psi

· The wells listed above in BOLD were not Permitted for annular disposal. Wells drilled
after June 6, 1996 were required by SSD procedure to be permitted with a Sundry
Approval (10-403) form. Our records show that no Sundry Approval for Annular
Disposal was filed for these wells. Wells drilled between July 25, 1995 and June 6, 1996
were automatically approved for annular disposal with the Permit to Drill.

Please find attached a copy of the Form 10-403 to temporarily suspend this well as per AOGCC 20-
AAC 25.110. As discussed this morning we propose setting three plugs in this well as detailed in the
attached form. This will allow us to retain the wellbore for potential future sidetrack or service well
use.

We would like to keep the well's 9-5/8" x 7" annulus available for the injection of liquid wastes. There
are no other well annulii on this pad for injection. The annulus for MPB-24 was approved for injection
as part of the permit dated 25 January 1996. The annulus will be freeze protected before moving the
rig.

Friday, September 15, 1995; I witnessed the function test of the Diver[er system on
Nabors rig ¢28-E which was preparing to drill BPX well ¢E-39 in the Prudhoe Bay Field.
The flowlines from the adjacent wells were in such close proximity to the rig that it
necessitated terminating the 16" vent line at 47'. I authorized the rig to put a slight
upswing in the vent line so as to direct the possible blast up and over the existing
flowlines. This allowance was for this well only and should be avoided as the outside
ambient temperature drops in the upcoming months.
The Diver[er functioned properly with a quick closure time of 52 seconds. I found a
problem with the accumulator set, Once the Diver[er was actuated there was a tendency
for the hydraulic reserves to bleed down. This was due to the accumulators valving not
completing its travel. Mike Downey will have this repaired and report to me.
SUMMARY: I witnessed the function test of the Diver[er system on Nabors rig ¢28-E. I
found a problem with the rigs accumulator system.

Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25.035. Sufficient
notice (approximately 24 hours) of the BOPE test performed before drilling below the surface casing
shoe must be given so that a representative of the Commission may witness the test. Notice may be
given by contacting the Commission petroleum field inspector 659-3607.

Request for Annular Injection
Disposal:
Cuttings Handling: Cuttings should be hauled to the ball mill at CC-2A.
Fluid Handling: All drilling and completion fluids can be annular injected. Request to have well E-39 for annular
pumping.
Request to AOGCC for Annular Pumping Approval for E-39:
1. Approval is requested for Annular Pumping into the E-39 9-5/8" x 7" casing annulus.
.
Aquifers in the Prudhoe Bay Field Western Operating Area (including K Pad) have been exempted from
Class II injection activities by the AOGCC by Aquifer Exemption Order #1 as referenced in Area Injection
Order #3. There are no domestic or industrial use water wells located within one mile of the project area.
3. The 9-5/8" casing shoe will be set at 3612' MD (3600' TVD) which is below the Permafrost and in shale
formations in the Sagavanirtok above the Ugnu Formation.
.
The burst rating (80%) for the 9-5/8" 40# NT-80 casing is 4,600 psig while the collapse rating
(80%) of the 7" 29# L-80 casing is 5,615 psig. The break down pressure of the Ugnu formation is 13.5
ppg equivalent mud weight. The Maximum Allowable Surface Pressure while annular pumping is calculated
according to the following equation and is 4095 psi for a 10.0 ppg fluid in this well (Assumes worst case in
which 7" casing is filled with gas).

5. A determination has been made that the pumping operation will not endanger the integrity of the well being
drilled by the submission of data to AOGCC on 7/24/95 which demonstrates the confining layers, porosity,
and permeability of the injection zone.
6. The cement design for this well ensures that annular pumping into hydrocarbon zones will not occur.

@ 10,050' MD/8,879' TVD. TIH w/8-1/2" hole opener if needed. Run 7" 29# L-80
NSCC casing and cement with Class G cement to 1000' MD above the top of the
hydrocarbon-bearing sand @ 8,000' MD. The annulus will be freeze protected with dry
crude if not annular injecting. Test casing to 3,500 psi.

Run 4-1/2" 12.6# L-80 NSCT solid liner with liner hanger. The liner will be set at TD
and lapped +150' inside 7-5/8" casing shoe. Cement liner in place with sufficient cement
to fill entire liner annulus. Make cleanout run. Test liner and liner lap to 3,500 psi for 30
minutes. Displace Xanvis KC1 mud with filtered seawater.

Run 3-1/2" L-80 tubing with SSSV, GLM's, and 7" x 3-1/2" packer assembly. Circulate
corrosion inhibitor into tubing annulus. Set packer and test tubing annulus w/3,500 psi.
9. Set BPV. ND BOPE.
10. Freeze protect well to 2200' TVD through shear valve in top GLM station. Release rig.
Drilling Hazards
· One hydrocarbon-bearing zone is found above the Sag River formation @ 8,000' md based on
E Pad logs. The zone is expected to be normal pressure with a relatively low permeable sand.
· A minimum of 9.5 ppg mud weight will be used to drill the Cretaceous injection interval. This
will allow a 100 psi trip margin over the expected 9.0 ppg EMW @ 4000' sstvd.
· A range of mud weights from 8.9 ppg to 9.0 ppg will be used to drill the Sadlerochit interval
after intermediate casing has been set.
