NU BOPE.
Spot tanks and trailers. RIU hardline to frac tank. Grease tree.
Trouble shoot swap and master. Grease test regrease cycle
valve etc.
Pressure 1 Function test BOPE.
AOGCC Test Witness waived by Chuck Scheeve
Continue with general rig up. Complete Reel swap install hard
line in reel.
Repair Annular Preventer. It failed to test, replaced inner
seals.
Changed out BOPE fluid.
Continue testing BOP. Send BOP test report to AOGCC.
Pull 2' coil to injecter head. Stab pipe.
Weld on CTC using rig welder. Pull 1 pressure test CTC.
Fill coil with Biozan KCL water. Press test inner reel valve.
Circ. Dart. Dart on seat at 40.3 bbls.
Open well. Well was dead.
M/U BHA #1, 2.74" Pilot hole mill, 1.75 deg motor and Sperry
MWD with BOT MHA. OAL = 62.51'.
RIH with pilot hole MWD BHA. SHT at 3000'. Orienter OK.
MWD OK,
RIH to 9200'.
Log GR tie-in inside 3-1/2" tubing. Correct +45'. Resolve tie-in
issues and verify tie-in log is depth corrected.
RIH . Orient to high side. Tag cement at 9452'
Drill pilot hole in cement plug. Drilling to high side at 1.74 bpm
@ 3400 psi, FS= 3200 psi, PU Wt 30K. ROP 16. Drill to 9513
beginning of ramp.
PU above 9450 to orient to low side.
Drill cement ramp 1.64 bpm at 3360 psi. from 9513 to 9519.5.
Motor work increase and Tool face change. Ramp in contact
with low side of 7" casing. PU 15' and retag. POH for window
mill.
POH to surface.
BHA swap pilot milling assy for window milling assy.
RIH with window milling BHA #2 with 2.5 deg motor.
RIH to 9400'.
Orient TF to 130R.
RIH to bottom. Tag 3 times at 9517', no depth correction. Pick
up to tie-in and confirm depth.
Log GR tie-in at 8942'. Added 5.5 ft to CTD.
RIH. Tag TD at 9522'. Pick up to 9510'. RIH to 9518' and
start pump. Ease down to 9520.5'. Park and had 145L TF.
Pick up to 9450' and orient TF to 140R.
RIH to 9518', start pump and RIH to 9520.5'. TF = 170R. Pick
up 1 ft to reduce down force.
Mill 2.74" window as per procedure. 1.6 bpm, 0 WOB, 3150
psi, 175R.
Procedure takes 3 hours to mill first foot.
Flagged coil at 9519.5 ft.
Milling 2.74" window, 1.65 bpm, 2500 psi, 0 wob, 175R,
Milled to 9521'.

Make up HC 331 pabarolic pilot mill , string mill, 1.75 motor &
new MWD probe (limiting out battery hours)

Log GR tie-in. Added 7' to CTD.
RIH with 170L TF. Ream window starting at 9521'. Several
stalls. Orient around to 175R. Several stalls getting to 9524'.

Mill new formation, 1.65 bpm, 2500 psi, 175R TF, 12 fph,
full returns.
Milled to 9535'.
Ream window up and down 3 times. Swap well to FloPro while
reaming. RIH clean thru window with no pump. Back ream up
thru window clean.
POH to surface while circ at 1.6 bpm. Flag coil at 9200' EOP.
Layout milling BHA. Mill still at full gauge [2.74" no go]. String
reamer at 2.73" no go.
MIU BHA #4 with 2.8 deg BHI X-treme Motor and re-built HCC

petroleum field inspector at (907) 659-3607 (pager).
As provided in AS 31.05.080, within 20 days after written notice of this
decision, or such further time as the Commission grants for good cause
shown, a person affected by it may file with the Commission an application
for rehearing. A request for rehearing is considered timely if it is received by
4:30 PM on the 23rd day following the date of this letter, or the next working
day if the 23rd day falls on a holiday or weekend. A person may not appeal
a Commission decision to Superior Court unless rehearing has been
requested.

Sundry approval is requested to prepare G-30A for a sidetrack. G-30A is a Zone 1/2 sidetrack that reaChe;}
marginal GOR in 2004 and has cycled since then. During the past 12 months the on-time has been less that 20%.
With no wellwork options, a sidetrack to a new area is planned.
The sidetrack, G-30B, is scheduled to be drilled by Nordic 1 on or about Jan. 7. 2006. The plan calls for laying
17 ppg cement across the entire production liner to prep for a cement ramp exit in Zone 2 with the target in Zone
lB. The following describes work planned and is submitted as an attachment to Form 10-403. A schematic
diagram of the proposed sidetrack is attached for reference.

Pre-IDe Work
1) Integrity testing of the IA and OA has been successfully completed.
2) Tubing has been inspect~d . a PDS calliper and is in good condition.
3) 17 ppg cement (minimu 14 bls) will be layed across the 4 Y2" production liner to shut-off all
perforations and provide a cement ramp exit.
4) Remove flow line, remove well house, and level pad.
The pre CTD work is slated to begin about Jan. 1. 2006.

2) Mill 2.74" window in Shublik interval.
3) Drill sidetrack into Zone 1B target.

6) Freeze protect well and RDMO.
Post-IDe Work
1) Install well house, flow line and gas lift valves.

The 7" liner procedure for cementing went a~ording to plah up until the casing
test procedure where an attempt to pressure up to 3`500 psi was made. A
pressure of 3600 psi was reached and the pressure bled off to t 100 psi, A 9 5/8"
RTTS with a set of ?" seals was run in the hole and the 7" seals were stung into
the PBR on top of the 7 X 9,5/8" liner top packer. A pressure of 8,500 psi was
held on the annulus, but the 7" liner would not test, The 9 5/8" RTT$ and 7"
seals were POOH and a 7" R'I-rS was run to isolate the leak in the 7" liner.
There was some problem with tight hole at 6470' (7" liner hanger) and the RTTS
had to be worked through, The packer was set at 6479' (4' in the first joint under
the liner hanger) and the 7" liner tested to 3000 psi. The annulus was then tested
and a rate of 3 BPM @2000 psi was observed. The 7" RTTS was POOH and a 9
5/8" EZSV was run and set at 6400'. A volume of 31 Bbls of Neat Class "G"
cement was squeezed away with a final pressure of 1100 psi, The cement was
displaced to just below the EZSV,
After unstinging and reversing out the EZSV was stung into again after several
hours and a pressure test was made up to 2000 psi, The pressure held and did
not bleed off, Once the casing was cleaned out another pressure test was
performed to 2`500 psi which slowly bled off to 1800 psi. The well was then
cleaned out to the landing collar at 9788', Presently we are preparing to drill the
production interval withe 6" bit,
1, Ddil ahead with a 6" bit through the production interval to +11826', run and
cement 4 1/2" production liner in place,
2,, Cleanout 4 1/2," production liner, PU c) 8/4" perforating guns and perforate
designated intervals, eleanout 4 1/2" liner, run 4 1/2" permenant packer with "XN"
nipple.

Prudhoe Bay Field
Tuesday, June 27, 1995: I traveled this date to BPX's PBU well G-30A being drilled
by Doyon rig 14 and witnessed the initial BOPE test.
As the attached AOGCC BOPE Test Report shows there was 1failure. The ball type
floor safety valve leaked. The valve was replaced and tested before I departed the
location.
The location was clean and all excess equipment was stored properly and there
was good access to all areas of the rig. The rig was clean and all equipment
seemed to be in good working order.
Summary:. ! witnessed the BOPE test on Doyon rig 14 - test time - 3 hours - 1
failure.

authorize conducting drilling operations until all other required
permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with
20AAC 25 035. Sufficient notice (approximately 24 hours) must be given
to allow a representative of the Commission to witness a test of BOPE
installed prior to drilling new hole. Notice may be given by contacting

Well Control:
Well control equipment consisting of 5000 psi W.P. pipe rams, blind rams, and
annular preventer will be installed and is capable of handling maximum
potential surface pressures.
Diverter, BOPE and mud fluid system schematics on file with AOGCC.
Hazards and Contingencies
Mud weights should be kept to a minimum while drilling through the horizontal
section (+9.0 ppg) to prevent differential sticking. With 9.0 ppg the hydrostatic
is +4250 psi which well over the formation pressure of +2500 psi.

SUMMARY PROCEDURE
1. Pre-Rig Work: RU slickline. RIH and ensure the sliding sleeve at 9598' is open, if sleeve is
inoperable then perofrate 4 squeeze holes at 9597'.RU cementers and tie directly into tree down
the production tubing, mix and pump 20 bbls of Class "G" and displace with 137 bbls of
seawater (top of cement @ _+9000'), POOH, RU wireline, RIH with jet cutter and tag top of
cement with AOGCC representative present (top of cement must be above 9500'), then jet cut
4 1/2" production tubing at +8698'
2. Move in drilling rig.
3. Puli BPV. Circulate diesel freeze protection cap from well and recover for future use.
4. Install a TWC valve. Remove the production tree.
5. Install BOP's and test to 5000 psi. Remove the TWC valve.
6. Pull and lay down the 4 1/2", 12.6#, L-80, BUTT tubing, RU wireline, RIH ~vith EZSV for
9 5/8" casing and set retainer at 6850'.
PU 9 5/8" section mill, RIH to 6850', spot heavy mud to 6650', pull section mill to 6600'.
Displace well to FLO-PRO/milling fluid. Cut a 50' section in the 9 5/8" casing starting
from about 6600'-6645'MD. POOH
9. RIH with open ended drill pipe to 6750' and lay a cement plug back to 6400', POOH.
10. Run in with a 6 3/4" steerable drilling assembly and a 8 1/2" bit with gamma
ray/directional MWD. Contact AOGCC to witness tag of cement for abandonment purposes.
Dress the cement to 5' below the section top. Kickoff and if necessary displace well to clean
FLO-PRO system. Continue to directionally drill to the 7" casing point at _+9744' at 89.5°
(see attached directional plan).
1 1. Make a conditioning trip with-an 8 1/2" hole-opener for the open hole section Install a 7",
29#, L-80, STL Solid liner with 7" X 9-5/8" packer/hanger. Packer / hanger to be set at
_+645O' MD.
12. Cement the liner in place with the total cement volume based upon 50 % open-hole
enlargement plus the 80' shoe volume, 9-5/8 X 7" lap volume, and 150' excess on top of
the liner with drillpipe in the hole.
13. Re-test BOP's as needed.
14. PU 3 1/2" drill pipe and 4 3/4" steerable drilling assembly with game ray and directional
MWD. RIH and cleanout liner to landing collar, test casing to 3500 psi.
15. Drill out shoe and continue to drill horizontally to 11967' MDTD.
16. Install a 4 1/2",12.6#,L-80,NSCT solid liner with 4 1/2" X 7" hanger/packer. The liner
hanger is to be at +9594'.
17. Cement the liner in place with the total cement volume based upon 50% open-hole
enlargement plus the 80' shoe volume, 4 1/2"X7" lap volume and 150' excess on top of the
liner with 3 1/2" drill pipe in the hole.
Clean out cement to landing collar.
RIH with drillpipe conveyed perforating assembly and perforate the well. POOH LDDP.
Run the 3-1/2" L-80 completion string assembly using as much 3-1/2" used tubing and
jewelry as possible. Space out and circulate with corrosion inhibitor.
Install a BPV and test plug. Remove BOP's.
Install and test the production tree to 5000 psi. Pull tree test plug and BPV.
Set packer.
Test the tubing x casing annulus to 3500 psi for 30 minutes.
Shear RP valve in the upper GLM and freeze protect the well.
Set aBPV. Release rig. RECEIVED
Complete appropriate documentation and hand-over to Production.
