Cancelled or Expired Permit Action

This memo will remain at the front of the subject well file.
Our adopted conventions for assigning APl numbers, permit numbers and well names did not
specifically address expired or cancelled permits. This omission has caused some inconsistencies
the treatment of these kinds of applications for permit to ddll. Operators have asked us to adopt
formal procedures for this class of permit application in order to prevent future database disparities.
If a permit expires or is cancelled by an operator, the permit number of the subject permit will remain
unchanged. The APl number and in some instances the well name reflect the number of preexisti~
redrills and or muitilaterais in a well. In order to prevent confusing a cancelled or expired permit with
an active well or multilateral these case sensitive well identifiers will be changed for expired and
cancelled applications for permits to ddil.
The AP! number for this cancelled or expired permit is modified so the eleven and twelfth digits is 95
The well name for a cancelled or expired permit is modified with an appended xx.
These procedures are an addendum to the APl numbering methods descnbed in AOGCC staff
,memorandum "Multi-lateral (weiibore segment) Ddiling Permit Procedures, revised December 29,
1995.
AOGCC database has been changed to reflect these changes to this permit.

Dear Mr Strickland:
Enclosed is the approved application for permit to re-drill the above referenced well.
The permit to re-drill does not exempt you from obtaining additional permits required by law
from other governmental agencies, and does not authorize conducting re-drilling operations
until all other required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25.035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling a new hole. Notice may be given
by contacting the Commission's petroleum field inspector on the North Slope pager at 659-3607.

NK 21A
Pool 7 is currently approaching the target pay zone on NK 21. Because of
uncertain geology in the area, it is possible that the pay encountered will not
economically justify completing the well. Approval of this contigent sidetrack
plan is requested so that the sidetrack may commence without delay of the rig
operations.
The proposed new bottom hole location and directional plan is based on the
best information available at the time of submitting this request. New
information received from the Niakuk 21 logs may alter desired the NK 21A
bottomhole location, however. I will be in touch as NK 21 progresses to discuss
concerns and changes.

Contingent Operations Summary
Abandon the 9-7/8" hole in the reservoir section by running in hole with open-
ended drillpipe and laying a balanced cement plug from TD to at least 100'
above the top of the Kuparuk formation.
Trip out of hole and lay a second 500' cement plug from 8100' MD to 7600' MD
('l-VDss: 6680' to 6340'). Trip out of hole and wait on cement to set up.
Run in hole with 9-7/8" steerable drilling assembly and tag the top of cement.
Kickoff and drill as per attached directional plan for NK 21A.
Set 7 5/8" casing and cement same. Test casing to 4000psi for 30 minutes.
Run 4-1/2" carbon steel tubing. Displace to diesel.
Freeze protect the annulus.
Release rig.
Run cased hole cement bond log.
