Item 1a: Classification of Service wells: Gas injection, water injection, Water-Alternating-Gas Injection, salt water disposal, water supply for injection, observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is a completion. Item 4b: TPI (Top of Producing Interval). Item 8: the Kelly Bushing elevation in feet abour mean low low water. Use same as reference for depth measurements given in other spaces on this form and in any attachments. Item 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00). Item 20: True vertical thickness. Item 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. Item 23: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the producing intervals for only the interval reported in item 26. (Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval). Item 26: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-in, Other (explain). Item 27: If no cores taken, indicate "none". I've added a postable drilling hazards section to the permit- it was in the well plan already but didn't make it over. I'm working with our directional planner to make the Anti-Collision report include full offset data for 18-01 & 18-18. I looked into the cement XS and found that a 6.125" OH was used instead of the 7" OH we are going to drill- this brings our cement requirements to 1102 cu-ft and 942 sks. Looks like the plan to use the hole opener came after the cement cales. Liner Cap 0.1276ft^3/ft x 200' = 25.5 ft^3 Liner Lap 0.0498ft^3/ft x (4400'-4250) = 7.5 ft^3 Annular Volume 0.1 023ft^3/ft x (8375'-4400') x 2.6(160% XS) = 1057.3 ft^3 Shoe Volume 0.1336ft^3/ft x 80' = 12 ft^3 Total = 1102.3 ft^3 I've also attached my cement volume cals for the P&A. I took over this well from Kelly McClure who recently resigned with this being the first permit I've put together. Any advice on format and content would be very helpful. Let me know if you need me to submit another signed permit for these changes or if I can email the corrected version over. Also if you want the cement cales included in the pennit. «1 B-15A Cement Cales. doc» Thanks, Jake 1!...:,',,"r"A' ¡,..rNE-r: \:-~B q éi 2005 (~,,..~ lJ ~ ~ '. Item 1' Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. Item 16 and 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. Item 21' Indicate whether from ground level (GL) or other elevation (DF, KB, etc.). Item 23: Attached supplemental records for this well should show the details of any multiple stage cement- ing and the location of the cementing tool. Item 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water In- jection, Gas Injection, Shut-in, Other-explain. Item 28: If no cores taken, indicate "none". Test - Parker 245 ARCO, KRU, 1 B-15 Kuparuk River Field PTD # 94-51 Sunday, April 10, 1994: I traveled to ARCO's Kurparuk lB pad to witnessed an initial B.O.P.E. test on Parker 245. The B.O.P.E. Test Report is attached. The 9-5~8" casing was installed to 4191 feet. The test began at noon and went smoothly with 1 failure. The Lower IBOP had a small leak, was greased and retested good. The equipment and crew performed well. Summary: I witnessed a successful B.O.P.E. test on Parker 245 drilling well 1B-15 at Kuparuk. The test lasted for 2.5 hours and there was I failure. Maximum anticipated surface pressure is calculated using a surface casing leak-off of 13.5 ppg EMW and a gas gradient of 0.11 psi/ft. This shows that formation breakdown would occur before a sudace pressure of 2476 psi could be reached. Therefore, ARCO Alaska, Inc. will test the BOP equipment to 3000 psi. The nearest well to 1 B-15 is 1B-05. As designed, the minimum distance between the two wells would be 16.3' @ 533' MD. ~ Incidental fluids developed from drilling operations will be hauled to the nearest permitted disposal well or will be pumped down the surface/production casing annulus of the last well drilled. That annulus will be left with a non-freezing fluid during any extended shut down (> 4hr) in pumping operations. The annulus will be sealed with 225 sx of cement followed by arctic pack upon completion of fluid pumping. *Spud mud should be adjusted to a FV between 50-80 sec/qt to drill gravel beds. RECEIVED !
