GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and
leases in Alaska.
ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water
supply for injection, observation, injection for in-situ combustion.
ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces
on this form and in any attachments.
ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item
16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional
interval to be separately produced, showing the data pertinent to such interval.

Load 3-1/2" DP into pipe shed. Strap & tally pipe.
Service rig.
MU Baker K-1 Cement Retainer & setting tool on DP. RIH PU DP.
PU Kelly. Install drive bushing, lower Kelly cock, & saver sub. Torque up lower & upper Kelly valves. Test upper &
lower Kelly cocks to 3500 psi, OK. Blowdown & set Kelly back in rathole. Haul 290 Bbls mud to storage @ KCS pad.
RIH PU DP. 110K# Up, 55K# Dn.
MU head pin & cementing lines. RU Dowell.
Set Baker K-1 Cement Retainer @ 7955'. Push & pull 30K# to lock in slips & test retainer. Rotate stinger free & unsting
from retainer. Stab in & set 35K# down on retainer, OK. Establish injection rates with rig pump & 12.9 ppg mud:
2000 psi @ 1 BPM, 2450 psi @ 1-1/2 BPM. Unsting& break circulation. Close pipe rams & test 7" casing to 3500 psi,
lost 50 psi in 15 minutes, OK. Bleed off pressure. Dowell start batch mixing cement.
Dowell circulate 5 Bbls water into DP. Test lines to 3500 psi. Circulate 45 Bbls more water into DP. Stab in & set 35K#
down on retainer. Inject & pump 50.6 Bbls of 15.8 ppg cement, 1.5 - 2.6 BPM, 3400 - 2000 psi. Pump 5 Bbls water
behind cement. Switch to rig pump & follow water with 45 Bbls mud, 2-1/4 BPM, 2000 psi, leaving 2 Bbls cement in
bottom of DP. Unsting from retainer & PU +2'. Reverse out, recovered 5 Bbls water spacer & 1 to 2 Bbls of cement. P
& A of old production perforations & wellbore below 7955' complete. CIP @ 2230, 12/14/96.
Mix 18 ppg pill. Pump 15 Bbls of 18 ppg pill. Chase pill with 49 Bbls mud & spot 18 ppg pill on bottom as a balanced
plug. POH LD 7 joints to 7749'. Reverse out excess pill.
Water back 12.9 ppg mud in pits to 9.3 ppg & condition mud for milling fluid.
Note: Called in final report of plugging operations to Doug Amos, AOGCC Field Inspector, this morning.

Circulate 12.6 ppg mud around. Some heavy & light spots Jn returns, but no gas cut or noticeable amount of oil/water at
bottoms up. Circulate until enen 12.6 ppg Jn & out.
Monitor well, very slow flow. No pressure build up. Weight up mud in pits to 12.9 ppg.
Circulate 12.9 ppg mud around until even 12.9 ppg in & out, 3.3 BPM, 600 psi. No gas or oil/wamr cut at bottoms up.
Monitor well - static, no flow, no loss. Mix & pump slug.
POH - stand back 80 stands tubing, LD jewelry. Install new seal rings, drift, & strap.
POH - LD 43 joints tubing & LD jewelry. Recovered 283 joints tubing, SSSV, 5 GLMs, & Seal Assembly.
Unload tubing & jewelry from rig floor & pipe shed. Monitor well, static.
Load 3-1/2" DP into pipe shed. Strap & tally pipe. Install wear bushing. Monitor well - static.

Pull WKM hanger to floor, 70K# Up, 75K# maximum pull. Circulate once around.
Monitor well, stable. LD hanger & space out pups.
POH with 3-1/2" tubing. Stand back tubing, drift, strap, & install new seal rings. LD jewelry, recovered 32 SS control line
bands. Tubing started pulling wet. Monitor well, stable.
Mix & pump slug.
POH, stand back tubing, LD jewelry.
66 stands out, started coming wet. monitor well, slow flow out of tubing & annulus. Hole fill showing 2 Bbl Loss.
Circulate tubing & annulus around, still have slow flow. Gained back 2 Bbls while circulating.
RBIH 66 stands tubing. Still showing slow flow, but mud volume even. no gain, no loss. Shut in well & RU circulating
line.
Unload 280 Bbls of 12.2 ppg mud from Parker 245 into pits. Weight up mud in pits to 12.6 ppg. No pressure build up on
well. Prepare to circulate 12.6 ppg around.

Cameron hand got CIW lubricator from Prudhoe. RU lubricator & pulled VR plug. RU lines to circulating manifold.
Pressure test circulating lines & line from choke to production flowline. Thaw line in cellar.
SITP = 2050 psi, SICP = 2200 psi.
Bleed gas head off annulus then tubing, bleed thru choke to production flowline. Load tubing, circulate 100 Bbls Sea
Water ~nto tubing, returns th'ru choke to production flowline. Inject 25 Bbls SW to flush out tail & perfs. Continue
circulating on choke & bring SW to surface to load well, 3 BPM. Monitor well - SITP = SICP = 1175 psi. Freeze protect
flowline with diesel while monitoring well.
Condition mud in pits & weight up to 12.6 ppg. Thaw line in cellar.

PRE-RIG WORK:
1. RU slickline unit. RIH tag fill. RIH and dummy off all GLMs, leave deepest GLM open.
2. Set BPV in tubing. Install VR plug in annulus valve if necessary.
WORKOVER RIG WORK:
1. MIRU Nordic Rig. Lay hard-line to tree. Pull B PV and VR plug.
2. Load pits w/kill weight mud.
3. ND tree. NU and test BOPE.
Circulate well to kill weight mud. Ensure well is dead.

Screw in landing joint. POOH standing back tubing and laying_down all jewelry.
PU DP and RIH with cement retainer. Set retainer at 7965' MD. Pump cement squeeze (-46
bbls) below retainer into A Sand perfs filling 7" casing to bottom A Sand perf / top of fill.
Unsting from retainer and circulate well to milling fluid. Spot an 18 ppg balanced mud pill
from PBTD to the base of proposed section. POOH standing back DP.
6. RIH w/section mill. Cut a 50' sectiOn in the 7" casing from 7765' - 7815'. POOH w/mill.

RIH w/open ended DP. Wash down through section and circulate any metal shavings from
well. Mix and pump balanced cement kick-off plug f/100' below base of section to 200'
above top of section.

Change Approved Program
Kuparuk River Unit Well 3H-23 was permitted to be drilled as a slim hole
Kuparuk River Unit development well (Permit ~94-18, see attached). The surface
hole was to be a 9-7/8" hole and cased with 7-5/8" casing. The production hole
was to be a 6-3/4" hole and cased with a tapered 5-1/2" X 5" casing string. The
geological results of drilling offset Well 3H-31 required moving Well 3H-23 up in
the drilling order. As a result, the required tools and casing was not available to
drill the well as a slim hole. Verbal approval was granted by Blair Wondzell on
2/12/94 to drill Well 3H-23 as a conventional KRU well (12-1/4" surface hole
with 9-5/8", 36#, J-55 casing and 8-1/2" production hole with 7", 26#, L-80
casing). No other parameters of the original Permit to Drill for 3H-23 are
changed.

Enclosed is the approved application for permit to drill the above
referenced well.
The permit to drill does not exempt you from obtaining additional
permits required by law from other governmental agencies, and does not
authorize conducting drilling operations until all other required
permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with
20 AAC 25.035. Sufficient notice (approximately 24 hours) of the BOPE
test performed before drilling below the surface casing shoe must be
given so that a representative of the Commission may witness the test.
Notice may be given by contacting the Commission petroleum field
inspector on the North Slope pager at 659-3607.

Attached is an application for Permit to Drill KRU'3H-23.
Please note that the surface casing specified in the permit
(7-5/8" 29.7# L-80 LTC) has previously not been run in
Kuparuk as surface casing and is therefore not one of the
casing strings listed in Conservation Order 173 4.d.1.
The 7-5/8" surface casing proposed for 3H-23 has burst and
collapse ratings which exceed those for the 9-5/8" 36# J-55
BTC currently being run at Kuparuk. Additionally, the
surface tensile strength of the pipe body and connection have
design factors of 6.5 and 5.4 respectively. An additional
analysis of the bending stresses at the kickoff point
indicates pipe body and connection tensile design factors of
4.0 and 3.3 respectively.
Also attached for your information is a wellbore diagram of
casing and hole sizes being planned for 3H-23. If you have
any questions concerning the well design, please contact at

Move in and rig up Parker #245.
Install diverter system.
Drill 9-7/8" hole to 7-5/8" surface casing point (4223 TMD')according to
directional plan.
Run and cement 7-5/8" casing.
Install and test BOPE. Test casing to 2000 psi. Note: A Cement Bond Log will not
be run in the 7-5/8" surface casing.
Drill out cement and 10' of new hole. Perform leak off test to 12.5 ppg EMW.
Drill 6-3/4" hole to total depth (9328') according to directional plan.
Run open hole evaluation logs.
Run and cement 5-1/2" x 5 .... casing. (If significant hydrocarbon zones are present
above the Kuparuk, sufficient cement will be pumped for isolation in accordance with
Rule 20 AAC 25.030. Alternatively, if significant hydrocarbon zones occur within
200' TVD of the surface casing shoe, cement will be down squeezed in the annular
space between the surface and production casing after the primary cement job is
completed.) Pressure test to 3500 psi.
ND BOPE. NU tubinghead & full opening valve for cased hole logging.
Secure well and release rig.
Run cased hole cement evaluation. ND full opening valve & NU tree assembly.
Move in and rig up Nordic #1 or Coiled Tubing Unit.
Install and test BOPE.
Perforate and run completion assembly.
ND BOPE and install production tree.
Shut in and secure well.
Clean location and release rig.

Drilling fluid practices will be in accordance with the appropriate regulations stated in 20
AAC 25.033.
Maximum anticipated surface pressure is calculated using a surface casing leak-off of 13.5
ppg EMW and a gas gradient of 0.11 psi/ft. This shows that formation breakdown would
occur before a surface pressure of 1871 psi could be reached. Therefore, ARCO Alaska,
Inc. will test the BOP equipment to 3000 psi.
The nearest well to 3H-23 is 3H-19. As designed, the minimum distance between the two
wells would be 10' @ 550' MD. The wells would diverge from that point.
Incidental fluids developed from drilling operations will be hauled to the nearest permitted
disposal well or will be pumped down the surface/production casing annulus of the last well
drilled.
*Spud mud should be adjusted to a FV between 50-80 sec/qt to drill gravel beds.

2_ ASSURE THAT ACCUMULATOR PRESSURE IS 3000 PSI WITH
1500 PSI DOWNSTREAM OF THE REGULATOR.
3. OBSERVE TIME, THEN CLOSE ALL UNITS SIMULTANEOUSLY AND
RECORD THE TIME AND PRESSURE REMAINING AFTER ALL UNITS
ARE CLOSED WITH CHARGING PUMP OFF.
4. RECORD ON THE IADC REPORT. THE ACCEPTABLE LOWER LIMIT
IS 45 SECONDS CLOSING TIME AND 1200 PSI REMAINING
PRESSURE.
BOP STACK TEST
1. FILL BOP STACK AND MANIFOLD WITH WATER.
2_ CHECK THAT ALL HOLD-DOWN SCREWS ARE FULLY RETRACTED.
PREDETERMINE DEPTH THAT TEST PLUG WILL SEAT AND SEAT
IN WELLHEAD. RUN IN LOCK-DOWN SCREWS IN HEAD.
3. CLOSE ANNULAR PREVENTER AND CHOKES AND BYPASS VALVES
ON THE MANIFOLD. ALL OTHERS OPEN.
4. TEST ALL COMPONENTS TO 250 PSI AND HOLD FOR 10 MINUTES.
INCREASE PRESSURE TO 3000 PSI AND HOLD FOR 10 MINUTES.
BLEED TO ZERO PSI.
5. OPEN ANNULAR PREVENTOR AND MANUAL KILL AND CHOKE LINE
VALVES.
6. CLOSE TOP PIPE RAMS AND HCR VALVES ON KILL AND CHOKE LINES.
7. TEST TO 250 PSI AND 3000 PSI AS IN STEP 4.
CONTINUE TESTING ALL VALVES, LINES, AND CHOKES WITH A 250 PSI
LOW AND 3000 PSI HIGH. TEST AS IN STEP 4. DO NOT PRESSURE TEST
ANY CHOKE THAT IS NOT A FULL CLOSING POSITIVE SEAL CHOKE.
8. OPEN TOP PIPE RAMS AND CLOSE BOTTOM PIPE RAMS. TEST BOTTOM
RAMS TO 250 PSI AND 3000 PSI.
9. OPEN PIPE RAMS. BACKOFF RUNNING JT AND PULL OUT OF HOLE.
CLOSE BLIND RAMS AND TEST TO 3000 PSI FOR 10 MINUTES. BLEED TO
ZERO PSI.
10. OPEN BLIND RAMS AND RECOVER TEST PLUG. MAKE SURE MANIFOLD
AND LINES ARE FULL OF ARCTIC DIESEL AND ALL VALVES ARE SET IN THE
DRILLING POSITION.
11. TEST STANDPIPE VALVES TO 3000 PSI.
12- TEST KELLY COCKS AND INSIDE BOP TO 3000 PSI WITH KOOMEY PUMP.
13. RECORD TEST INFORMATION ON BLOWOUT PREVENTER TEST FORM.
SIGN AND SEND TO DRILLING SUPERVISOR.
14. PERFORM COMPLETE BOPE TEST ONCE A WEEK AND FUNCTIONALLY
OPERATE BOPE DAILY.
