ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in item 27. Test upper & lower Kelly cocks to 3500 psi, OK. Blowdown & set Kelly back in rathole. Haul 290 Bbls mud to storage @ KCS pad. RIH PU DP. 110K# Up, 55K# Dn. MU head pin & cementing lines. RU Dowell. Set Baker K-1 Cement Retainer @ 7955'. Push & pull 30K# to lock in slips & test retainer. Rotate stinger free & unsting from retainer. Stab in & set 35K# down on retainer, OK. Establish injection rates with rig pump & 12.9 ppg mud: 2000 psi @ 1 BPM, 2450 psi @ 1-1/2 BPM. Unsting& break circulation. Close pipe rams & test 7" casing to 3500 psi, lost 50 psi in 15 minutes, OK. Bleed off pressure. Dowell start batch mixing cement. Dowell circulate 5 Bbls water into DP. Test lines to 3500 psi. Circulate 45 Bbls more water into DP. Stab in & set 35K# down on retainer. Inject & pump 50.6 Bbls of 15.8 ppg cement, 1.5 - 2.6 BPM, 3400 - 2000 psi. Pump 5 Bbls water behind cement. Switch to rig pump & follow water with 45 Bbls mud, 2-1/4 BPM, 2000 psi, leaving 2 Bbls cement in bottom of DP. Unsting from retainer & PU +2'. Reverse out, recovered 5 Bbls water spacer & 1 to 2 Bbls of cement. P & A of old production perforations & wellbore below 7955' complete. CIP @ 2230, 12/14/96. Mix 18 ppg pill. Pump 15 Bbls of 18 ppg pill. Chase pill with 49 Bbls mud & spot 18 ppg pill on bottom as a balanced plug. POH LD 7 joints to 7749'. Reverse out excess pill. Inject 25 Bbls SW to flush out tail & perfs. Continue circulating on choke & bring SW to surface to load well, 3 BPM. Monitor well - SITP = SICP = 1175 psi. Freeze protect flowline with diesel while monitoring well. Condition mud in pits & weight up to 12.6 ppg. Thaw line in cellar. Circulate on choke & load tubing with 100 Bbls 12.6 ppg mud. Inject 25 Bbls mud to flush out tail & perfs. Continue circulating on choke, SW returns down production flowline, until mud returned to surface. Continue circulating thru choke into pits until 12.5+ ppg mud returned. Monitor well. Well dead. Freeze protect flowline with diesel. Blowdown & RD circulating lines. Set BPV. The surface hole was to be a 9-7/8" hole and cased with 7-5/8" casing. The production hole was to be a 6-3/4" hole and cased with a tapered 5-1/2" X 5" casing string. The geological results of drilling offset Well 3H-31 required moving Well 3H-23 up in the drilling order. As a result, the required tools and casing was not available to drill the well as a slim hole. Verbal approval was granted by Blair Wondzell on 2/12/94 to drill Well 3H-23 as a conventional KRU well (12-1/4" surface hole with 9-5/8", 36#, J-55 casing and 8-1/2" production hole with 7", 26#, L-80 casing). No other parameters of the original Permit to Drill for 3H-23 are changed. Please note that the surface casing specified in the permit (7-5/8" 29.7# L-80 LTC) has previously not been run in Kuparuk as surface casing and is therefore not one of the casing strings listed in Conservation Order 173 4.d.1. The 7-5/8" surface casing proposed for 3H-23 has burst and collapse ratings which exceed those for the 9-5/8" 36# J-55 BTC currently being run at Kuparuk. Additionally, the surface tensile strength of the pipe body and connection have design factors of 6.5 and 5.4 respectively. An additional analysis of the bending stresses at the kickoff point indicates pipe body and connection tensile design factors of 4.0 and 3.3 respectively. Drill 9-7/8" hole to 7-5/8" surface casing point (4223 TMD')according to directional plan. Run and cement 7-5/8" casing. Install and test BOPE. Test casing to 2000 psi. Note: A Cement Bond Log will not be run in the 7-5/8" surface casing. Drill out cement and 10' of new hole. Perform leak off test to 12.5 ppg EMW. Drill 6-3/4" hole to total depth (9328') according to directional plan. Run open hole evaluation logs. Run and cement 5-1/2" x 5 .... casing. (If significant hydrocarbon zones are present above the Kuparuk, sufficient cement will be pumped for isolation in accordance with Rule 20 AAC 25.030. Alternatively, if significant hydrocarbon zones occur within 200' TVD of the surface casing shoe, cement will be down squeezed in the annular space between the surface and production casing after the primary cement job is completed.) The nearest well to 3H-23 is 3H-19. As designed, the minimum distance between the two wells would be 10' @ 550' MD. The wells would diverge from that point.
