These voids were filled with cement if needed and corrosion inhibitor, engineered to prevent water from entering.the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. The cement was pumped March 21, 2010. The corrosion inhibitor/sealant was pumped April 1 thru Apri117, 2010. The attached spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. Please call Perry Klein or MJ Loveland at 907-659-7043, if you have any questions. Since ~.- ' Perry ConocoPhillips Well Integrity Projects Supervisor • ConocoPhillips Alaska Inc. Supervisor DATE: FROM: John Crisp, SUBJECT: Petroleum Inspector May 17-18, 2001 Safety Valve Tests Kuparuk River Unit 1Y Pad May !...7 & 1.8, .2.0.01: i traveled to Phillips Alaska's Kuparuk River Field to witness Safety Valve System tests. Ernie Barndt was Phillips Rep. in charge of SVS testing. Testing was conducted in a safe & efficient manner. ! witnessed one failure, the LPP failed to trip above 50% of primary separator pressure. The pilot was re-tested & passed the re-test. The AOGCC test report is attached for reference. I conducted 25 well house inspections on 1Y Pad. The majority of the houses were in good shape w/no problems witnessed. Well houses # 30 & 22 were found w/the well house lids resting on the tree cap gauges. The VSM for the flow line on well # 29 looked as if it had shifted up & had the line in a slight bind. I asked Phillips Rep. Pete Nezaticky to inspect this line. Prudhoe Bay Field has had problems w/VSM's shifting up & ! wanted to make sure KRU Rep's were aware of this problem. SUMMARy_: i witnessed SVS testing in Kuparuk River Field. '16 wells, 32 components tested. 1 failure witnessed. 3.t3% failure rate. 2,5 well house inspections conducted. Attachment: SVS KRU 1Y 5-17 & 18-01jc NON-CON,FIDEN ,T. I. Supervisor State of Alaska -.. Alaska Oil and Gas Conservation Commission DATE: August3, 1993 FILE NO: EAXKGWAA.doc FROM: Jerry Herring,~ ~(~1~ SUBJECT: Petroleum Inspector Diverter Test - Parker 245 ARCO, KRU, 1Y-21 Kuparuk River Field PTD #93-102 Friday, July 23, 1993: I traveled to Kuparuk's 1Y pad and witnessed the function test of Parker 245's diverter system and completed a rig inspection. The rig had just moved on hole from 1Y-28. The Diverter Systems Inspection and Rig Inspection Reports are attached for reference. The function test began at 7:00 a.m. and went well. I found the rig to be in excellent condition. All components functioned properly and no problems were found. There were two separate 10" ball valves off of the 20" hydrill diverter. The controls were such that either the starboard 10" ball valve or the port side 10" ball valve would open (not together) when the bag closed depending on prevailing wind conditions. The 10" lines had 3~ta_rget~d tee's each and were vented sufficiently far enough away from ignition sources. I found the layout acceptable for the situation. Summary: A successful Diverter Function Test and Rig Inspection were completed on Parker 245, drilling well 1Y-21 for ARCO, Kuparuk River Field. Since the permit approval, desired target location and subsequently, TD location, have moved approximately 250' south-southwest due to an apparent fault. The former location would intersect the fault and miss the 'A' Sand. A new target location of 2688' FNL, 2646' FEL, Sec. 36, T12N, R9E is planned for well 1Y-21 which should avoid intersecting the fault. The resulting new TD location is 2312' FNL, 2537' FEL, Sec. 36, T12N, R9E. The new location has been approved by Blair Wondzell and is in accordance with Administrative Approval 190.15.
