LIST OF ATTACHMENTS 32. i hereby certify that the foregoing is true and correct to the best of my knowledge INSTRUCTIONS General: This form is designed for submitting a complete and correct well completion, report and log on all types of lands and leases in Alaska. Item 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. Item 16 and 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. Item 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.). Item 23: Attached supplemental records for this well should show the details of any multiple stage cement- ing and the location of the cementing tool. Item 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water In- jection, Gas Injection, Shut-in, Other-explain. Item 28: If no cores taken, indicate "none". Went on vacuum when we shut down. Pumped 35 bbls cement and displaced with fresh water and seawater to 8500'. Good agreement with rate indicator, displacement tank, and vac truck level indicator. Displaced at 10 BPM/250 psi. SD, and the well went on vacuum. Hesitated 30 min. Pumped an additional 5 bbls. Caught after 4.75 bbls and pressured to 250 psi and holding. Looked good. Cement top estimated at approx 8800 - 8900'. NOTE TO PEs: Next time run dual suction lines or 3' suction off vac truck. 10 BPM is pushing it. The best estimate for the start of rig work on the well is 5^UG94. Contact the Prudhoe Bay Drilling Representative on Doyon Rig #14 for the most accurate moving schedule update at 659-4510. If there are any questions or if more information is needed contact J. E. Robertson at 564-5510. To prepare for the sidetrack, the following actions will need to be taken prior to the rig move: , . , . , . . Notify an AOGCC representative prior to commencing plug-back operations. RU slickline unit. WLIH and open 4-1/2" OTIS 'XA' sleeve at +_9641' md. WLOH and RD slickline. NOTE: See attached tubing talley to verify depths. RU BJ cementers. Tie into tree. Pump 160 bbl seawater and establish injection rate. Follow seawater with 35 bbl Class 'G' P&A cement. Follow cement with 129 bbl seawater to squeeze/abandon all perforations. This should leave calculated TOC at ___8500' md. With AOGCC representative on location to witness operation, run in with slickline and confirm that top of cement is at least 100' above the 4-1/2" sliding sleeve at 9641' MD. RU E-line. WLIH and sever tubing at +_8335' md (middle of first full joint above the PBR). WLOH RD E-line. Circulate well over to seawater. Freeze protect well with diesel to 2000'. Bleed any pressure from the tubing, inner annulus and outer annulus to zero. Install a back-pressure valve. Remove the well house and flowlines. Level the pad and dig out the cellar. Prepare the pad for arrival of the rig. RECEIVED JUL 2 1 1994 Alaska 0ii & Gas Cons. Commission Anchorage Page 1 J-09A SUNDRY APPROVAL APPLI(,;ATION OBJECTIVE.. The objective of this sidetrack is permit effective recovery of ZONE 1 reserves as well as obtain additional ad-rec and acceleration from ZONE 2. The J-09A sidetrack is not part of a development plan. However, it is one of several horizontal wells planned for the J-F-Q area. Successful wells from this group will lead to new sidetracks in the area. SCOPE: The scope of work is as follows: Plug and abandon the original perforations. Pull the existing completion. Set bridge plug. Mill a section in the 9-5/8" casing and lay a cement plug below and across the section. Kick-off and drill a new 8-1/2" hole to +_50 inside the top of the Sadlerochit. Set/cement the 7-5/8", 29.7#, L-80, STL liner and change out to Xanvis mud. Continue to drill/build angle to 90° at +_10042' md. From there drill +_1738' of horizontal section. Run a 5- 1/2", 17#, L-80 STL solid liner and cement same. Complete with 4- 1/2" tubing. Timing. This project is presently scheduled for Doyon Rig #14 on or about 5AUG94. The redrill should take approximately 25 days. RECEIVED JUL 2 1 1994 Alaska 0il & Gas Cons. Commission Anchorage Page 2 I TREE: 6" CIW 5M ....i ,,~,~, KB. All wells _had shut in autcmmtically on low pressure pilot when GC-1 was shut down for modification. Low pressure pilots at the wells are set at 590 psi. Low pressure flowline pilot locate~t at the control house is set at 625 psi. High pressure pilots are'set at 2600 psi. Since we could not b~ into the flowline, a high pressure hose vms c~nnected to the top of the tree and pressure bled into a tank in the back of a pickup. The wells w~re extremely gassy and most of the time expended in tesf_ing was in bleeding the gas caps dc~n. Testing on all wells was initiated by reading t/he pressure above the SSV, bleed/rig it to zero and monitoring for any buildup for five minutes. SSV was then opened, a tub~ pressure reoorded and pressure, above the SSSV bled to an easily read differential on the gauge and monitored for five minutes. Annulas pressure between the tubing and 9 5/8" was recorded during this time.
