ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation,
injection for in-situ combustion.
ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any
attachments.
ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the
producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data
pertinent to such interval.

Approval is requested for the proposed P&A of PBU well 5-32. The well is planned to be sidetracked to a
new bottom hole location as a horizontal producer targeting Ivishak Zone 1 reserves. The existing 5-32
completion has confirmed leaks in the tubing and produces at a high GOR. Under current conditions, the
well offers no intervention opportunities and provides no benefit to the field. A horizontal sidetrack is
recommended to target Zone lA reserves at risk. A horizontal well will increase standoff to gas and help
optimize production in the area.
As outlined in the scope of operations below, the sidetrack kick-off point is in the 5-1/2" liner above the
existing Zone 1 perforations. Zone 1 perforations will be isolated with a bridge plug set just below planned
kick-off point after de-completing the well. The bridge plug bridge plug will be located ~250' above the
Zone 1 perforations and pressure tested. Final abandonment of the perforations will be provided with the
cement pumped in the 3-1/2" production liner lap (~760') above the bridge plug. See attached diagrams
for details and please call with any questions regarding the planned operations.
The 5-32A sidetrack is currently scheduled for Nabors 2ES beginning around January 18Ih, after the
completion of the 5-02A sidetrack. The pre-rig work for this sidetrack is planned to begin as soon as the
attached Application for Sundry is approved.
Scope
Pre-Rig Work:
As of 12/17/01, the following rig prep has been performed:
1. The tubing x casing annulus failed to pressure test (leaking tubing).
2. The 7 5/8" casing tested to 3000 psi.
3. The SSSV and chemical injection valve at 8895' dummied off.
4. The tubing and casing packoffs both tested to 5000 psi/3000 psi, respectively.
Pre-Rig Work Remaining:
1. Rig up slickline. Replace all live gas lift valves with dummy valves and drift 3 %" tubing down to 'XN'
nipple at 9121' with 2.75" drift. Drift runs are to verify the tubing is clear for upcoming chemical
cuts with E-line, and can be run at discretion of Wells Group.
2. Rig up E-line and perforate at 9040' MD (5' below the top of the uppermost full joint of tubing tailpipe)
to allow gas trapped below the packer to migrate into the tubing.
3. Chemical cut the 3-1/2" 13-Cr tubing at 8980' MD (this is roughly in the middle of the first full joint
immediately above the Iocator seal assembly). Monitor tubing and annulus pressure for indication of a
successful cut and note results on WSR.
4. Displace the well to clean seawater through the tubing cut at 8980' MD and freeze protect the top
2200' MD with diesel.
5. Cycle tubing hanger LDS.
6. Bleed tubing and annuli pressures to zero. Set a BPV in the tubing hanger. Secure well.

7. Remove the well house and flowlines. If necessary, level the pad. Please provide total well
preparation cost on the handover form.
Ri.q Operations:
1. MIRU Nabors 2ES.
2. Pull BPV. Circulate out freeze protection and displace well to seawater. Set TWC.
3. Nipple down tree. Nipple up and test BOPE. Pull TWC.
4. Pull tubing from chemical cut above packer. POOH laying down 4~" completion.
5. RIH with overshot and recover cut stub and Iocator seal assembly.
6. Make-up packer mill with packer retrieving tool. RIH and mill 7-5/8' packer. Catch packer with packer
retrieving tool via mill out extension. POOH with packer and tubing tailpipe.
7. RU E-line and make GR/JB run to top of tubing cut. MU bridge plug. RIH and set at 9657'.
8. Test wellbore to 3500 psi for 30 minutes.
9. RIH with 5-1/2" Baker whipstock assembly. Orient whipstock as desired and set on bridge plug. Mill
window in 5-1/2" casing at 9640' MD. POOH.
10. MU 4-3/4" drilling assembly with MWD/GR only and RIH. Kick-off and drill the 4-3/4" production
interval, building at ~12°/100' to horizontal, targeting the Zone 1 sands. Drill ahead to an estimated
total depth of 12,130' MD (8815' TVD).
11. Run and cement a 3-1/2", 9.3# solid production liner throughout the 4-3/4" wellbore and back into the
5-1/2" and 7-5/8" casing to the liner top at ~8,800'. Displace well to seawater above liner top. POOH.
12. Test the 3-1/2" and 7-5/8" casing from the 3-1/2" landing collar to surface to 3500 psi for 30 minutes.
13. PU Schlumberger memory CNL and RIH on DP. Run log through build section of well. POOH and
download log to identify GOC prior to perforating.
14. PU Halliburton 2" perf guns and RIH on DP. Perforate ~1000' of Zone 1 in the horizontal section
POOH LD DP.
15. Run 3-1/2", 9.3# 13Cr-80 completion tying into the 3-1/2" liner top.
16. Set packer and test the tubing to 3500 psi and annulus to 3000 psi for 30 minutes.
17. Nipple down the BOPE. Nipple up and test the tree to 5000 psi.
18. Freeze protect the well to ~2200' and secure the well.
19. Rig down and move off.
Post-Rig Work:
1. MIRU slickline. Pull the ball and rod / RHC plug from 'X' nipple below the production packer and
install gas lift valves.

Operations Summary
1. MIRU drilling rig. NU diverter and function test same.
2. Spud well and drill a 13 1/2" vertical hole to 3500'tvd/md.

Set 10 3/4" casing and cement same. Test casing to 2500 psi
for 30 minutes. NipPle up BOPs and test same to 5000 psi.
.
Pick up 9 7/8" bit and build BHA and drill 10' of new hole and
perform leakoff test. Continue drilling 9 7/8" "straight" hole to
3800'tvd/md (KOP point).
.
At 3800'tvd/md (KOP), build angle at 2.5 degrees/100'md in the
N29' 1" E direction to 38 degrees at 5234'tvd/5349'md (EOC).
Hold angle at 38 degrees to 8375'tvd/9374'md (7 5/8" casing
set point).
6. Set 7 5/8" casing and cement same.
7. PU 6 3/4" BHA and test casing to 3000 psi. Drill 10' of 6 3/4"
hole and perform formation integrity test to12.5 ppg EMW.
8. Continue drilling 6 3/4" hole in the N 29' 1" E direction to
8972'tvd/10141'md while maintaining a 38 degree angle.
9. Run wireline logs as programmed.

assembly. NU tree and test to 3000 psi. Release completion rig.
15. Rig up wireline unit and perforate well.

Operations Summary-continued
NOTES:
a. Excess non-hazardous fluids developed from well
operations and as necessary to control fluid level in
reserve pit will be pumped down the 10 3/4" x 7 5/8"
annulus. The annulus will be left with a non-freezing
fluid during any extended period. The annulus will be
sealed with 300 sxs of cement followed by
Arctic Pack.
b. Wells within 200' of the proposed well are'

productive zones are anticipated above 4000'tvd.
f. No hazardous levels of H2S are anticipated in this

1. CHECK AND FILL ACCUMULATOR RESERVIOR TO THE PROPER
LEVEL WITH HYDRAULIC FLUID.
2. ASSURE THAT THE ACCUMULATOR PRESSURE IS 3000 PSI WITH
1500 PSI DOWNSTREAM OF THE REGULATOR.
3. OBSERVE TIME, THEN CLOSE ALL UNITS SIMULTANEOUSLY AND
RECORD THE TIME AND PRESSURE REMAINING AFTER ALL UNITS
ARE CLOSED WITH THE CHARGING PUMP OFF.
4. RECORD ON THE IADC REPORT. THE ACCEPTABLE LOWER LIMIT
IS 45 SECONDS CLOSING TIME AND 1200 PSI REMAINING PRES-
SURE.
13-5/8" STACK TEST
1. FILL BOP STACK AND MANIFOLD WITH A NON-FREEZING FLUID.
2. CHECK THAT ALL HOLD DOWN SCREWS ARE FULLY RETRACTED.
MARK THE RUNNING JOINT WITH PREDETERMINED PLUG SEATING
DEPTH. RUN THE TEST PLUG ON DRILL PIPE AND SEAT IN WELL-
HEAD.
3. CLOSE ANNULAR PREVENTER AND UPSTREAM VALVES.
4. FUNCTION TEST CHOKES.
5. IN EACH TEST, FIRST HOLD 250 PSI FOR 3 MINUTES PRIOR TO THE
HIGH PPRERSSURE TEST.
6. INCREASE PRESSURE ON ANNULAR TO 3000 PSI AND HOLD FOR 3
MINUTES. BLEED PRESSURE TO ZERO.
7. OPEN ANNULAR PREVENTER AND CLOSE TOP PIPE RAMS.
INCREASE PRESSURE TO 5000 PSI AND HOLD FOR 3 MINUTES.
BLEED PRESSURE TO ZERO.
8. CLOSE VALVES DIRECTLY UPSTREAM OF THE CHOKES AND OPEN
CHOKES. INCREASE PRESSURE TO 5000 PSI AND HOLD FOR 3
MINUTES. BLEED PRESSURE TO ZERO.
9. OPEN TOP SET OF PIPE RAMS. CLOSE BOTFOM PIPE RAMS.
INCREASE PRESSURE TO 5000 PSI AND HOLD FOR 3 MINUTES.
BLEED PRESSURE TO ZERO.
10. TEST REMAINING MANIFOLD VALVES (IF ANY) WITH 5000 PSI
UPSTREAM OF THE VALVE AND ZERO PRESSURE DOWNSTREAM.
11. BLEED PRESSURE TO ZERO. OPEN BOTTOM PIPE RAMS.
12. BACK OFF RUNNING JOINT AND POH. CLOSE BLIND RAMS AND
TEST TO 5000 PSI FOR 3 MINUTES.

AND LINES WITH NON-FREEZING FLUID. SET ALL VALVES IN THE
DRILLING POSITION.

16. RECORD BOP TEST INFORMATION ON THE BOP TEST FORM SIGN
