GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in
Alaska.
ITEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
ITEM 4b: TPI (Top of Producing Interval).
ITEM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
ITEM 20: True vertical thickness.
ITEM 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the
cementing tool.

Jack up rig. DSO disconnect ESD from well on DS. Move rig
off well. Clean up cellar area/post pad inspection wI DSO. Fin

Enter CPF3 Spine Rd and head to 1 L-25
Arrive on 1 L Pad. Move over well. Accept rig at 17oohrs.
NU BOPE. grease all manifold, stack and tree valves. Lift and
secure 2' coil reel #15178 wI 15100'.
Prep to test BOP's. Witnessing wavied by Chuck Sheive
Pressure test BOPE 250psi/low 3500psi/high 5min ea.
Pressure test BOPE. 250 psillow 3500psi/high 5min/ea
PJSM review plan forward RU injector and possible hazards.
Hang injector and install pack-off and gooseneck assemblies.
Install internal coil grapple. Stab coil to injector. Prep end of
coil for Slack Management.
Fill coil with fluid - slowly in case of ice plugs. Then pump Slack
Management.
AES build berm for tiger tank
Cut off 90' CT. Install CTC. PT 40k, 4k. Meg line 44 ohms.
Install BHI head and test. AES set tiger tank and fin berm.
Run hardline and PT. Work wI instrument tech to RU TRSSSV
panel. This one is too big. Purchase one from Kuparuk whse

each 0.1' drilled. At 8839.0', 4k str wt wi 3.2k wob

8840.3' seeing good drill-off's
Continue milling fl 8840.6' 2-3k wob 3-400psi mw. good

Exited window. Drill rathole.
POH to pick up crayola mill to dress window.
Tag up at surface. Trap 950 psi below SSSV. Open choke
and whitey valve on lubricator. No flow observed. Hold PJSM
to discuss BHA swap. Recheck for flow. Confirmed that there
is no flow observed.
WEXIT Lay down motor and mill from BHA #2. Mill gauged 2.74" Go,
and 2.73" No-go. Mill had a good indication of center well. All
looks as expected. Pick up WF crayola mill and 1.75 deg
motor to dress window.
RIH with BHA #3 to dress window. Trapped 950 psi on well,
opened SSSV. Found 940 psi below SSSV, did not appear to
be much bleed off during BHA swap.
Log tie in pass over GR charecter at 8767'. Make +6'
correction.
Orient to high side. Try to run down thru window with pumps
on. Hung up at bottom of window at 8841 '. Kick on pumps,
ream down at bottom of window to clean up, holding TF at high
side.
At 8841.5'. Continuing to ream down thru window with TF at 0
deg. Reaming down as per David Heam. Trying to clean up lip
at bottom of the window.
Finally made it thru the spot at 8841' where we had problems.
Run to bottom. Make backreaming passes at 0 deg, 45 and 90
degrees left and right of high side.
Set down at the bottom of the window. Continue to work
window.
Try to dry drift window, and set down at 8842', just past bottom
of the window. Kick pumps on and able to work down thru it.
Continue to ream bottom of window with tool face at high side
as per David Hearn.
WEXIT TOH
WEXIT Discuss with town group the options for salvaging this window
or drilling new window. We believe the cement ramp has been
milled out slightly below the casing window resulting in a small
"lip" at the bottom of the window. Of all the different options we
finalized trying a new window mill with <1.75 deg motor. Small
problem. No new mill on Slope.
Other options discussed:
. Rerun 1.75 deg motor and new crayola mill. Dress window
again best we can and continue with well. This might work for

lateral motors and liners making it through the window.
· PU 2.5 motor and crayola mill. Redress window. No way to
effectively mill on steel lip without hazardous effects on motors
output shaft.
· With D331 drill past this cement ramp and make another
ramp below both with or without cementing first.
· Setting LSK whipstock at bottom of pilothole and making a
lowside exit. This would be a last resort option.
Wait on 1 deg WF motor.
MU BHA #3 with 1 deg motor and RR One Step Window Mill
RIH with EDC open. Tagged SSSV PU then went through
okay.
Log tie-in starting at 8722' correction + 6.5' Close EDC
RIH slow to 8842.5' seen small wt bump. PU to 8832 20.3k up
wt RIH slow to 8839' wo pump then .5' increments looking for
wob increase. PU to 8800' bring on pump 1.53/150/4000
holding 200 on choke. RIH slow to 8836.6'
Time mill .1'/5min from 8838.6'
DPRB WEXIT We appear to be on the lip at the bottom of the window.
Continue time milling to see if we can clean up the lip.
8841.75'. WOB=600 Ibs. 1.57 bpm, 4130 psi. PVT=244 bbls.

no metal coming back on the ditch magnets. This could
indicate that we are not milling metal, but are skinning the
inside of the lip. Discuss with David Hearn. Call town and
discuss with town engineer. POOH to pick up 1.75 deg motor
with same mill in the hopes that we can get the mill up a little
higher and hook the lip.
PJSM re: BHA swap. Flow check well during Safety meeting.
No flow observed.
Laydown 1 deg motor. BHA #4. Pickup 1.75 deg motor and
same used 1 step window mill. BHA #5. Scribe same.
RIH with BHA #5. Hanging up at tubing collars while RIH.
Having to roll the pumps to get by each collar.
Continue RIH. Having to roll the pumps at every tubing collar
to get in the hole.
Log tie in pass at 8750'. Make a +8' correction.
RIH dry at 8844.5'. PU, kick pumps on to 1.54 bpm, 3904 psi
free spin. RBIH, begin milling from 8839.0' at 0.75 fph. Not
seeing much motor work or WOB on this run.
At 8843.95' start seeing 100-2oopsi motorwOrk and 1.5k wob.
Continue at .75fph Milled to 8844.5'. Opened EDC and CBU
50% metal 30% scale 18% cmt 2% sd
Log tie-in no correction.
Discussed with town plan forward. Will drill another 5' trying to
confirm if we're in or out of the 7' casing.
Start the 5' of milling/drilling? From 8844.5' setdown to apply
5-6k wob 1.52/1.46/3850 4oo-600psi motorwOrk' Establish
drilling rate. Work up from 6'/hr to 20'/hr. Drill to 8851.5' PU of

WEXIT btm 20k to 8842'. RIH lightly tag 8851'.
WEXIT Circulate samples. Grab several samples prior to BU. 1st was
90% cmt. Each succeeding sample showed an increase in
sand content. Sample f/ 8851 showed 25% sd. Other samples
taken after BU showed a max of 30% sd.
From the sample results we believe we must be outside the
casing and into the old rathole. May have reamed the rathole. If
we were inside the casing we would not expect to see any
sand.

LD 1-Step mill and WE motor. PU BHI 2.8deg motor and build
section BHA.
RIH w/ BHA #6 following pressure schedule with open EDC
and highside TF. BHA going smooth throughout TIH slowing
down at mandrel depths.
Log tie-in starting at 8740'. +6' correction.
RIH highside TF with open EDC. Setdown at 8841.5' PU 19k
RIH again slow taking wt at 8841.5' PU orient 40L RIH slipped
on through window to 8844.9' PU above window and orient

Description of Operations
Looks like we have 2 wellbores. One that is lower in INC with a
TD at 8873'. One that has 6 degrees higher INC, and landed at
8937'. There appear to have been 2 pilot holes from the
various milling and cleanout runs done previously. One pilot
hole was drilled with the original 2.5 deg motor, and one pilot
hole drilled with the 1.75 degree motor on the last cleanout run.
The deepest leg is the one from the 2.5 degree pilot hole.
The shallowest is from the 1.75 degree pilot hole.

lL-25 is located on the southern edge of lL Pad on the southeastern edge of the Kuparuk field. lL-25 is a long
term shut-in well. Originally completed in 1991 as an A Sand producer, lL-25 production was poor. The C
Sand was then perfed and fraced; however, no incremental production was gained from the C Sand. lL-25 was
converted to an AlC injector in 1995. The well initially injected 2000 BWPD, then apparently quickly pressured
up, with injection continually declining until injection ceased after 18 months. In 1996 lL-25 was shut in. lL-
25 has been selected for a through tubing sidetrack with the SLB/Nordic 2 Coiled Tubing Drilling rig on or
around November 20,2005. Four laterals are proposed in the sidetrack. lL-25A, will exit the 7" liner from a
cement ramp and target C sands. The existing perforations will be isolated by a cement plug and the liner
cement job. The following describes work planned and is submitted as an attachment to Form 10-403. A
ssþematic diagram of the proposed sidetrack is attached for reference.

Kuparuk Well 1L-25
ARCO Alaska requests approval to convert Well 1L-25 from an oil producer to a water
injector. This well will be operated as a Class II UIC well in compliance with Area
Injection Order No. 2. Water will be injected for the purpose of enhanced recovery into
the Kuparuk interval between the depths of 5975' and 6181' TVD. This interval
correlates with the strata found in ARCO's West Sak River State Well No. 1 between
the depths of 6474' and 6880' as authorized in Rule 1 of this injection order. A
Mechanical Integrity Test will be performed before injection is initiated and every four
years thereafter, in compliance with Rule 6 of this order. This well's injection volumes,
average injection pressure, and average annulus pressure will be reported to the
AOGCC monthly.

Drilling fluid practices will be in accordance with the appropriate regulations stated in 20
AAC 25.033.
Maximum anticipated surface pressure is calculated using a surface casing leak-off of 13.5
ppg EMW and a gas gradient of 0.11 psi/ft. This shows that formation breakdown would
occur before a surface pressure of 2130 psi could be reached. Therefore, ARCO Alaska,
Inc. will test the BOP equipment to 3,000 psi.
The nearest well to 1 L-25 is 1L-24. As designed, the minimum distance between the two
wells would be 29' @ 440' MD. The wells would diverge from that point.
Excess non-hazardous fluids developed from well operations and those fluids necessary to
control the fluid level in reserve pits will be pumped down the surface/production casing
annulus of the last well drilled. That annulus will be left with a non-freezing fluid during any
extended shut down (> 4hr) in pumping operations. The annulus will be sealed with 175 sx
of cement followed by arctic pack upon completion of fluid pumping.
*Spud mud should be adjusted to a FV between 100-120 sec/qt to drill gravel beds.

Drill out cement and 10' of new hole. Perform leak off test to 12.5 ppg EMW.
Drill 8-1/2" hole to logging point (9,467') according to directional plan.
Run open hole evaluation logs.
Run and cement 7" casing. Pressure test to 3,500 psi.
ND BOPE and install lower production tree assembly.
Secure well and release drilling rig.
Run cased hole cement evaluation and gyro logs.
Move in and rig up Nordic Rig 1.

1. CHECK AND FILL ACCUMULATOR RESERVOIR TO PROPER LEVEL
WITH HYDRAUUC FLUID.
2. ASSURE THAT ACCUMULATOR PRESSURE IS 3000 PSI WITH
1500 PSI DOWNSTREAM OF THE REGULATOR.
3. OBSERVE TIME, THEN CLOSE ALL UNITS SIMULTANEOUSLY AND
RECORD THE TIME AND PRESSURE REMAINING AFTER ALL UNITS
ARE CLOSED WITH CHARGING PUMP OFF.
4. RECORD ON THE IADC REPORT. THE ACCEPTABLE LOWER LIMIT
IS 45 SECONDS CLOSING TIME AND 1200 PSI REMAINING
PRESSURE.
BOP STACK TEST
1. FILL BOP STACKAND MANIFOLD WITH ARCTIC DIESEL.
2. CHECK THAT ALL HOLD-DOWN SCREWS ARE FULLY RETRACTED.
PREDETERMINE DEPTH THAT TEST PLUG WILL SEAT AND SEAT
IN WELLHEAD. RUN IN LOCK-DOWN SCREWS IN HEAD.
3. CLOSE ANNULAR PREVENTER AND CHOKES AND BYPASS VALVES
ON THE MANIFOLD. ALL OTHERS OPEN.
4. TESTALL COMPONENTS TO 250 PSI AND HOLD FOR 10 MINUTES.
INCREASE PRESSURE TO 3000 PSI AND HOLD FOR 10 MINUTES.
BLEED TO ZERO PSI.
5. OPEN ANNULAR PREVENTORAND MANUAL KILLAND CHOKE LINE VALVES.
CLOSE TOP PIPE RAMS AND HCR VALVES ON KILL AND CHOKE LINES.
6. TEST TO 250 PSI AND 3000 PSI AS IN STEP 4.
7. CONTINUE TESTING AL VALVES, LINES, AND CHOKES WITH A 250 PSI LOW
AND 3000 PSI HIGH. TEST AS IN STEP 4. DO NOT PRESSURE TEST ANY
CHOKE THAT IS NOT A FULL CLOSING POSITIVE SEAL CHOKE.
8. OPEN TOP PIPE RAMS AND CLOSE BOTTOM PIPE RAMS. TEST BO'I-rOM
RAMS TO 250 PSI AND 3000 PSI.
9. OPEN PIPE RAMS. BACKOFF RUNNING JTAND PULL OUTOF HOLE. CLOSE
BLIND RAMS AND TEST TO 3000 PSI FOR 10 MINUTES. BLEED TO ZERO PSI.
10 OPEN BLIND RAMS AND RECOVER TEST PLUG. MAKE SURE MANIFOLD AND
LINES ARE FULL OF ARCTIC DIESEL AND ALL VALVES ARE SET IN THE
DRILLING POSITION.
11. TEST STANDPIPE VALVES TO 3000 PSI.
12. TEST KELLY COCKS AND INSIDE BOP TO 3000 PSI WITH KOOMEY PUMP.
13. RECORD TEST INFORMATION ON BLOWOUT PREVENTER TEST FORM. SIGN
