The attached temperature plot shows no cooling anomalies along the length of the tubing to a total depth of 9,638’ which correlates to the tubing tail, approximately 13’ below the packer. An obstruction at the tubing tail prevented the tool from exiting into the casing. Within the report you will see a tab ‘RIH 17-19-21 Overlay RKB’ where we have overlaid the past two surveys with the current and this confirms that the current run shows no anomaly. On October 14, 2009, Unocal notified the Commission of an apparent well integrity failure on Granite Point State 18742 MUCI-3 (178-019) on Anna Platform in Cook Inlet. On or about March 24, 2009, applied pressure to the 9-5/8" x tubing annulus was lost. Unocal did not timely report this operational change. Enhanced recovery and disposal operations in the Granite Point Field are governed by Area Injection Order No. 6. An aquifer exemption in place for all strata below the sea floor as described in 40 CFR 147.102 (b) (2) (i). This well has historically operated with 2500 psi applied to the annulus in order to keep the seals engaged at the packer and had last passed a witnessed MIT -IA in mid December 2005. Due to waterflood operation equipment problems on Anna Platform, Uncoal has not been able to filter the necessary volumes of Inlet water and the large scale enhanced recovery injection had not been operational since late 2007. Small scale injection of a treated waste effluent/sea water mixture had continued at about 250 barrels/day with surface pressures initially of up to 1000 psi, but of late (10/09) peaking around 750 psi. Following the initial report, Unocal moved quickly to confirm that fluids were being injected into the completed interval by performing a temperature survey on October 16, 2009. The temperature survey demonstrated that the only significant thermal change in the well was associated with the completion interval. It was also indicated by the pressure plot that the fluid level in the well was about 1200' below surface. Based on the survey information Unocal applied for an Administrative Approval on November 5, 2009 to keep the well in operation agreeing to provide a monthly report of daily injection volume and well pressures and to perform a temperature survey every 2 years. Administrative Approval 6.010 was issued on November 18, 2009 and Unocal has complied with the monthly reporting requirements and conducted a temperature survey in May 2011 which continued to indicate that the injected fluids were entering the completion interval. It is not evident that an integrity failure occurred on MUCI-3 although the applied annulus pressure was removed in March 2009 and small volume, intermittent injection continued. The surveys of October 2009 and May 2011 indicate stable temperature increase with depth until the completion zone where the temperature decrease clearly indicates that fluid is being injected into the intended interval. Although the reporting time lapse is unfortunate, Unocal did move swiftly to verify that the well continued to have integrity in October 2009 and has complied with the Administrative Approval requirements. Due to drilling's concern over the pressure profile of the G -40 well, we will not be using this well for the foreseeable future, so I'll be moving this well to the LTSI reporting starting next month. Looks like the Anna MUCI -3 well, in the small batch injection mode, just doesn't provide a good indication of tubing pressures during injection. Basically, the tubing is being loaded up with the small batch and then it pushes itself away over time until the next small batch needs to be placed into the tubing. Not much to work with for tubing pressure, but when the well reverts back to continuous injection, this pressure will be more meaningful. Otherwise the monitored wells are responding as expected to ups and downs in injection volumes without any other pressure or rate anomalies noted. Bottom line, I don't see any indication that any of these wells are losing control of the injected fluids downhole. Box 196247 ';, .21 ;r 2 2011 Anchorage, AK 99519 -6247 RE: Granite Point State 18742 MUCI -3 (PTD 178 -019) Request for Administrative Approval Dear Mr. Martin: Per Rule 9 of Area Injection Order 6, the Alaska Oil and Gas Conservation Commission (AOGCC or Commission) hereby GRANTS Unocal Alaska (Unocal)'s November 5 2009 request for administrative approval to inject water in Granite Point State 18742 MUCI -3 (MUCI-3). Unocal notified the Commission on October 14, 2009 that MUCI -3 exhibits tubing by casing pressure communication. Although pressure communication to the casing annulus was indicated by provided pressure monitoring information, analysis of the data indicated that the casing remained competent. Approval to continue injection for the purpose of performing well integrity diagnostics was granted and Unocal performed a temperature survey on October 16, 2009. Unocal has elected to perform no corrective action at this time on MUCI -3. Temperature surveys performed as part of the diagnostic testing confirm the injected fluids are exiting the well at the perforations and monitoring has shown that pressures are contained within the wellbore. Accordingly, the Commission believes that the well's condition does not compromise overall well integrity so as to threaten the environment or human safety. The Commission's administrative approval to inject in MUCI -3 is conditioned upon the following: 1. Injection is limited to WATER ONLY; 2. Unocal shall monitor and record tubing, inner annulus, and outer annulus pressures and injection rate daily; • Mr. Hal Martin November 18, 2009 Page 2 of 2 3. Unocal shall submit to the Commission a monthly report of well pressures and injection rates; 4. Unocal shall perform a temperature survey every 2 years in lieu of the mechanical integrity test as outlined in Rule 6 of AlO 6 to demonstrate continued production casing integrity; 5. Unocal shall immediately shut in the well and notify the Commission if there is any change in the well's mechanical condition; and 6. After well shut in due to a change in the well's mechanical condition, Commission approval shall be required to restart injection. 7. The next temperature survey is due May 20, 2011 and at 2 -year intervals thereafter. As provided in AS 31.05.080, within 20 days after written notice of this decision, or such further time as the Commission grants for good cause shown, a person affected by it may file with the Commission an application for rehearing. A request for rehearing is considered timely if it is received by 4:30 PM on the 23rd day following the date of this letter, or the next working day if the 23rd day falls on a holiday or weekend. In your letter you required the following: [w]ithin 14 days of receipt of this letter, Union must provide the. Commission a written explanation of this event and confirmation that all injection well operations at the Granite Point Field Anna Platform are compliant with AIO 6 requirements. 1. Explanation of the Event. Union begins by acknowledging that it did not timely report the well integrity failure in Well MUCI -3 ( "MUCI -3 "). Some background information is helpful in understanding the recent operation history of the MUCI -3 and the Anna Platform that played a role in Union's oversight. MUCI -3 has been a high rate and intermittent waterflood injector on the Anna Platform with historical injection rate capability of over 2500 barrels of water per day ( "BWPD "). At least since 1999 (and possibly earlier), the injector has had +/- 2500 psi pressure applied to the tubing x casing annulus to keep the tubing seals in the packer. The well has passed four year Standard MITs in 2001 and 2005 operating in this fashion. In late 2007, Anna waterflood was shut down due to surface waterflood system issues and has remained shut -in since that time (repair of the system is currently being evaluated). Even though waterflood has been down, the effluent from the waste system, which is part of our enhancement to waterflood, continued to be injected into MUCI -3 through intermittent pumping at about 250 BPD with maximum injection pressure of approximately 1000 psi. Approximately March 24, 2009, the annular pressure applied to the 9 -5/8" casing was lost. With waterflood down, the loss of annular pressure was overlooked, and relatively low pressure, low volume intermittent effluent injection continued. After the well integrity failure was recognized the matter was reported to the AOGCC on October 14, 2009. Since the discovery of the integrity issue in MUCI -3, and after receiving approval from the AOGCC, Union performed a temperature survey on the well on October 16, 2009 which indicated that there is no temperature anomaly evident above the packer and that all injected fluids are entering the perforated intervals as intended. Union reported the results of the temperature survey to the AOGCC on October 19, 2009. Union has also submitted a request for administrative approval for MUCI -3 under AIO 6. Union would also like to address the Commission's reference to the Steelhead Subsurface Safety Valve Testing matter. This matter is not similar to the Steelhead SSV Testing matter. The Steelhead matter involved a program failure, which had significant safety and resource implications. In this case, the same safety and resource considerations are not at issue particularly in light of waterflood being shut in and the low volumes of effluent injection. Union in no way wishes to minimize this reporting matter, but does not believe the two matters are analogous. Union believes the present matter is an isolated incident of not recognizing an integrity failure. On the issue of training, Union wishes to emphasize to the Commission that it takes its compliance obligations very seriously. In response to this incident, Union has initiated a root cause investigation, and has already reinforced with its workforce the need for compliance with well integrity reporting requirements. Specifically, Union has reinforced with all appropriate personnel the importance of maintaining awareness in monitoring and complying with all waterflood injection regulatory requirements, including the timely reporting of well integrity failures. 2. Confirmation that All Injection Well Operations On the Anna Platform Are Compliant with AIO 6. Union confirms that all injection well operations at the Granite Point Field Anna Platform are currently compliant with AIO 6 requirements. A workover in 1/80 attempted to apply mechanical injection conformance control to the completion. The last workover in 4/87 was performed to repair tubing -to- casing communication. Until recently, the well injector operated under a standard MIT with +/- 2500 psi applied to the annulus to keep seals in the packer and maintain integrity. Waterflood injection on the Anna Platform has been down since late 2007 due to surface waterflood system issues. However, Well No. MUCI -3 continued on effluent injection at approximately 250 BFPD as part of an enhancement to waterflood. In late 3/09, the +/- 2500 psi applied to the annulus was lost and effluent injection continued until the oversight was discovered in 10/09. The AOGCC was notified by meeting on October 14, 2009. A temperature survey was run on October 16, 2009 and the AOGCC agreed with Union Oil that injected fluids are only entering the completion interval below the packer as intended. The AOGCC has given interim approval to continue effluent injection in to Well No. MUCI -3 while this administrative approval application is being prepared and processed. The well integrity failure appears to have occurred on or about March 24, 2009. As a result, Union's notification is approximately 203 days delinquent. Failure to timely report a well integrity failure is a violation of Commission regulations and Area Injection Order 6 (AIO 6), Rule 7 as amended by Administrative Approval 6.1 dated October 28, 1986. AIO 6, Rule 7 as amended (Well Integrity Failure) requires: Whenever operating pressure observations or pressure tests indicate pressure communication or leakage of any casing, tubing or packer, the operator must notify the Commission on the first working day following the observation, obtain Commission approval of a plan for corrective action, and when an USDW is not endangered, obtain Commission approval to continue injection. Union's failure to timely report this incident conflicts with prior assurances to the Commission, made between August 25, 2005 and July 14, 2006 during the Steelhead Safety Valve Testing Investigation, that Union had identified and remedied the communication issues which were a major causal factor regarding its failure to comply with safety valve testing and reporting requirements. Specifically, Union stated it had undertaken supervisor and operator training regarding the SVS system to avoid repeat violations. Union also represented that training as to Mr. Dale Haines October 30, 2009 Page 2 of 2 other regulatory requirements had been accomplished. This new violation calls those assertions into question. At a minimum, Union needs to reinforce with its personnel the need for compliance with all regulatory requirements, including timely report of well integrity failures. There is an aquifer exemption in effect for Granite Point Field as granted by EPA at 40 CFR 147.102 (b)(2)(i) so contamination of fresh water is not an issue. For the short term, it is acceptable to keep the well in operation however continued operation pending an Administrative Approval is contingent on the temperature survey demonstrating that injection is into the perForations. FYI, attached is a schematic of the MUCI-3 and a TIO plot covering the period in question. The topics we discussed were: 1. The integrity failure of the tubing/packer resulting in tubing-to-casing communication. 2. The plan forward in working toward an Administrative Approval for continued injection on the well. Integr~_ Failure Background; Historically, the MUCI-3 has been a high rate waterflood injector on the Anna Platform, relative to other waterflood injector rates. At least since 1999 (and possibly earlier), the injector has had +/- 2500 psi pressure applied to the 9-5/8" casing to keep the tubing seals in the packer. The well has passed 4-yr Standard MITs in 2001 and 2005 operating in this fashion. In late 2007, the Anna waterflood was shut down due to surface waterflood system issues and has remained shut-in since that time. Recently it was determined that the sand filters will require more extensive repair than was originally anticipated. The decision to repair the waterflood system is currently under evaluation. Even though this part of waterflood was down, the effluent from the waste system, which is part of our enhancement to waterflood, continued to be injected into MUCI-3 in an intermittent pumping fashion at about 250 BPD with a max injection pressure of about 1000 psi. Integrity Failure: Around March 24, 2009, the annular pressure applied to the 9-5/8" casing was lost. Since waterflood was not in operation the loss of annular pressure was inadvertently overlooked because, although waterflood was down, relatively low pressure, low volume intermittent effluent injection continued. Daily calculated effluent volume reporting had been accidentally overlooked as well and reporting these volumes was initiated in early May. Nevertheless the compliance failure associated with the loss of tubing/packer integrity in MUCI-3 and injection continued until recognized recently and then quickly reported to you in our recent meeting. Plan Forwa_rd Operationally, the MUCI-3 is an important injector, not only for waterflood, but also because it is the best waterflood injector to handle the effluent generated from the platform and used to enhance waterflood recovery. The reason for this is because the effluent generated is > 90% seawater, making it compatible with waterflood injection water and the volumes generated are easily injected into the well. Therefore, Chevron would like to work toward an Administrative Approval of this well to continue injection. Using an alternate well such as the produced water injection Well An-5, which continues to operate as well on a daily basis, is not preferred because of the various incompatibility issues generated from mixing produced water and seawater such as H2S, BaSO4, bacteria, corrosion, etc. The use of other 10/20/2009 ~ ~ Page 4 of 4 waterflood injection wells on the platform is not recommended because of the generally low injection rates observed in these wells. The plan that we agreed to pursue in our meeting is to run a temp survey ASAP and have the analysis demonstrate to the AOGCC that no abnormal cooling is taking place above the packer. If the completion intervals are reachable, the temp survey should also show the expected cooling is occurring from injected fluids going into approved intervals. In the unlikely event that the temperature survey strongly indicates injection may be occurring at some point above the packer, the well will be shut-in immediately. If the temp survey results are as expected, Chevron would like to continue injecting into MUCI-3 while preparing the necessary documentation for an Admin Approval request. It appears that they are holding a back pressure on both these annuli. Talking to the operators, they have been doing this from day 1 based on something the Amoco engineers designed from the start. We missed these annulus back pressure wells when we were digging around after the G-16 issue arose. The last MIT tests for both wells used the bleed off and annulus monitoring technique as both annuli were above the minimum required test pressure. Looking at the TIO plots for both these wells, these pressures seem to be in the 'normal' range for the annuli. The MUCI-3 well is their swing well. It is on injection continuously, but it handles the excess from the pumps that the other wells won't take. One of the two water injection pumps quit in mid-November, so when the inspector was on the platform only one pump was in use. This means that almost no water was going into MUCI-3 and, per the operators - it almost goes on vacuumm at low rates, the tubing pressure would drop off to almost nothing. Their sewage system is designed to go only to the MUCI-3 well and was sized for a max of 15-20 bpd, usually 1-2 bpd. This well normally takes around 1750 bpd of water, but with only one pump running it is down to around 10-20 bpd, which is very hard to meter at that level. Our files are very skinny on these wells too. I do have MITs on both wells from 2/8/97 and I sure can dig downstairs to see what else is down there. Hope this helps. r -. The annulus pressure on each well was then reduced 500 psi and monitored for 30 minutes with no changes noted. Both wells passed the 30 minute MIT. The upper pipe rams, inside choke and inside kill valve were tested first to 3000 psi OK. The outside choke and outside kill valve were tested next to 3000 psi and tested CK. ~he check valve on kill line also tested to 3000 psi OK. The lower pipe rams an~ 11 valves on manifold ~are tested separately to 3000 psi CK. One valve on manifold to 3000 psi and held test OK. There were two manually operated chokes on mare[fold and ~ positive choke. One of the manual chokes was seized up 1500 psi and tested OK. The upper and lower kelly w~re tested to 3000 psi and held tests of 3000 psi OK. The master and ~ controls operated satisfactory. There were five nitrogen badcdp bottles with 1800, 1800, 2250, 2250, and 2250 each OK. On this job the accumu/ator was perfonmance tested ~¢ith all pc~er except accumulatz~ bottles shut off. Anr~lm? .preven*_e-?, 1500 psi left on master gauge with no leaks, indicating BOP closing, unit and all seals in BOPs ~are OK. The master gauge read 3000 psi to start. In ~: I witnes~ the BOP tests on Platform Anna on M:U.C.I. }%D. 3. ~ were c~pleted, the BOPs and all cc~pcnents ~ere ~n satisfactory coalition with o~e exception. The subject well is to be drilled as a joint Mobil-Union-Chakachatna (MUC) lease line water injector. The well is to be directionally drilled from Leg 3, Slot 3, Granite Point Platform Anna to a primary target on top of the Middle Kenai C-7 sand zone at 3000' FEL on the south line of Section 12, T10N, R12W, S.M. Secondary targets include the tops of the C-5 and C-8 zones as detailed in the attached application. Total depth will be achieved following complete penetration of the C-8 zone. This well is required to improve waterflood sweep efficiency and oil recovery from t_he competitive properties involved. Also attached for your reference are copies of the previous correspondence on this well, ending with'Administrative Approval No. 76.6 granted on January 10, 1978. The well will not be produced as a producing well prior to use as an injector.. The target location at 10,500' + MD, which is the approximate mid-point of the Middle Kenai-producing zone, is 3000' from the east line and on the south iine of Section' 12-T10N-R12W, SM as shown on the enclosed plat. The purpose of this third lease line injection well, part of a field-wide'injection program conducted by cooperative agreement between all interested parties, is to improve waterflood efficiency and oil recovery from each competitive prOperty, thereby preventing waste of recoverable hydrocarbons not otherwise recoverable through the use of the present two line injectors, yet preserving the correlative rights of the affected parties. It is contemplated that up to 3,000 BWPD will be injected a~ MUC No. 1-3 at a maximum well.head pressure of 5500 psia. O. K.
