ITEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is a completion. ITEM 4b: TPI (Top of Producing Interval). ITEM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other spaces on this form and in any attachments. ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00). ITEM 20: True vertical thickness. ITEM 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. ITEM 23: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the producing intervals for only the interval reported in item 26. (Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval). ITEM 26: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In. or Other (explain). ITEM 27: If no cores taken, indicate "None". This conventional well is currently shut-in due to multiple tubing leaks allowing communication between the tubing and inner annulus. The holes were verified by a Kinely caliper. The wellbore is providing no production or value to the field as it currently sits. Prior to being shut in the well was last tested on 3/18/01 at 190 bopd, 80% WC, 1700 GOR. Z-O4 was drilled and completed in early 1990. No RWOs or frac jobs have been performed since it was drilled. A coiled tubing deployed packer cement squeeze was performed in 1995 to repair a leaking packer; the packer, tailpipe, and some of the tubing above the packer are cemented in place. The well was a good producer for several years following an acid job in 1994, averaging 1000 bopd, 65% WC, 7000 GOR. It was shut in following the pad explosion in 19G8 and returned to production briefly in 2001 to evaluate RWO potential. The two 2001 well tests indicated the watercut had increased and oil rate decreased to 100-200 bopd. The watercut increase mechanism was never diagnosed. Well Z-O4 currently has tubing by inner annulus communication via several corrosion holes in the tubing indicated on the caliper log. The 9 5/8" casing has been pressure tested via a combo tubing by inner annulus pressure test on 10/03/04 to 3000 psi. The outer annulus was successfully pressure tested at 3000 psi on 7/26/04. The proposed Z-04A horizontal is a Zone 4 pattern improvement play located in the southwestern quadrant of Z-Pad, in the central (gascapped) part of the Highland State Fault Block. The sidetrack will exit the current Z-O4 well bore in the Kuparuk formation by milling a 8-1/2" window in the 9-5/8" casing. The 8-1/2" well bore drops inclination and trends to the West setting 7" casing at the top of the Sag River formation. A 6-1/8" production interval will build to horizontal landing in the 42P and then turn to the Northwest. After crossing a minor fault, the well turns to the West and runs parallel to an East-West trending fault South of Z-09i for 2950 feet. The well toes down towards the 41 P at the end to confirm the oil-water contact. A solid 4-1/2" liner will be cemented in place throughout the production interval. The Z-04A sidetrack is currently scheduled for Nabors 7ES beginning around 11/15/04, after completing the Z-13A sidtrack well. The pre-rig work for this sidetrack will be priority scheduled immediately after the Application for Sundry is approved. Pre-Ria Ooerations: Z-04 Application for Sundry 1.
