This form and the required attachments provide a complete and concise record for each well drilled in Alaska. Submit a current well schematic
diagram with each 10-407. Submit 10-407 and attachments in PDF format to aogcc.permitting@alaska.gov. All laboratory analytical reports from
a well must be submitted to the AOGCC, no matter when the analyses are conducted per 20 AAC 25.071.
Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each
segregated pool is a completion.
Pursuant to 20 AAC 25.070, attach to this form: well schematic diagram, summary of daily well operations, directional or inclination survey, and
other tests as required including, but not limited to: core analysis, paleontological report, production or well test results.
Report measured depth and true vertical thickness of permafrost. Provide MD and TVD for the top and base of permafrost in Box 29.
Attached supplemental records should show the details of any multiple stage cementing and the location of the cementing tool.
If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the
producing intervals for only the interval reported in item 26. (Submit a separate form for each additional interval to be separately produced,
showing the data pertinent to such interval).
Provide a listing of intervals cored and the corresponding formations, and a brief description in this box. Pursuant to 20 AAC 25.071, submit
detailed descriptions, core chips, photographs, and all subsequent laboratory analytical results, including, but not limited to: porosity,
permeability, fluid saturation, fluid composition, fluid fluorescence, vitrinite reflectance, geochemical, or paleontology.

BHA #8. Found internals of Strider agitator on top of mud motor. Replaced

Enclosed is the approved application for the permit to drill the above referenced well.
The permit is for a new wellbore segment of existing well Permit Number 222-106, API Number 50-
103-20137-60-00. Production from or injection into this wellbore must be reported under the original
API Number stated above.
Per Statute AS 31.05.030(d)(2)(B) and Regulation 20 AAC 25.071, composite curves for all well
logs run must be submitted to the AOGCC within 90 days after completion, suspension, or
abandonment of this well, or within 90 days of acquisition of the data, whichever occurs first.
This permit to drill does not exempt you from obtaining additional permits or an approval required by
law from other governmental agencies and does not authorize conducting drilling operations until all
other required permits and approvals have been issued. In addition, the AOGCC reserves the right to
withdraw the permit in the event it was erroneously issued.
Operations must be conducted in accordance with AS 31.05 and Title 20, Chapter 25 of the Alaska
Administrative Code unless the AOGCC specifically authorizes a variance. Failure to comply with an
applicable provision of AS 31.05, Title 20, Chapter 25 of the Alaska Administrative Code, or an
AOGCC order, or the terms and conditions of this permit may result in the revocation or suspension
of the permit.

I’m finished with my portion of the review for these two permit to drill applications.
FYI: James’s email with the additional requested info for 3G-13AL1 AOR is included (below and
attached). I noticed that James did not specify a footage for the top of cement in nearby well 3G-20
(PTD 1901230). Curious about this, I pulled and scanned the cement evaluation logs, which are
stored on the server in folder G:\AOGCC\Wells\z190\1901230_KRU_G-20. I’d pick the top of good-
quality, consistent cement at about 9615’ MD, which is about 70’ above the shallowest C-sand perf
at 9686’ MD in that well.

Here is the L1-01 AOR map and the ¼ mile offset well it is within range of:
KRU 3G-20 & L1 (Injector)
•The L1 sidetrack is being drilled currently to the North-East of the toe
•Completed with 7”, 26#, J-55 casing on 9/12/1990.
•C-sand perf’d 9686’- 9700’ and A-sand perf’d 9720’ -9820’. The tubing tail is at 9601’ MD
with the packer at 9575’ MD.
•The cement bond log taken from 7,092’ to TD shows cement up to and above the
perforations. Well was put on injection in 1991 and put on WAG injection service May 2007
with state's knowledge. The well has a history of passing state-witnessed MIT-IA's and no
reporting of abnormal 9-5/8" casing pressure. The proposed kick-off point for the CTD
sidetrack is in the A-sand perforations of the 7” casing and should not compromise existing
conformance.

Yes, James provided the last of the requested information on Thursday. I finished my portion of the
review for the 3G-13A application last week, and I will finish reviewing the application for the lateral
today.

I quickly surveyed CPAI’s Permit to Drill applications for these two proposed injection wellbores, and
I don’t see Area of Review (AOR) maps or tables that provide casing and cementing information and
demonstrate zonal isolation for all other wells that are open to the pool within that AOR. Please
provide this information so that review of these applications can continue.
