Our adopted conventions for assigning APl numbers, permit numbers and well names did not specifically address expired or cancelled permits. This omisSion has caused some inconsistencies in the treatment of these kinds of applications for permit to ddll. Operators have asked us to adopt formal procedures for this class of permit application in order to prevent future database disparities. If a permit expires or is cancelled by an operator, the permit number of the subject permit will remain unchanged. The APl number and in some instances the well name reflect the number of preexisting redrills and or multilaterals in a well. In order to prevent confusing a cancelled or expired permit wit h an active well or multilateral these case sensitive well identifiers will be changed for expired and cancelled applications for permits to drill. The APl number for this cancelled or expired permit is modified so the eleven and twelfth digits is -95. The well number for a cancelled or expired permit is modified with an appended XX. CANCELED PERMITS ARE RELEASABLE 2 years from the date on the letter received from the operator requesting the permit canceled.. (Memorandum January 23, 1997 by Dave Johnston, subject: Canceled and Expired Permits) EXPIRED PERMITS ARE RELEASABLE 2 years from the date on the application was issued by the commission, unless the operator ask for extended confidentiality. As the attached AOGCC BOPE Test Report shows the test lasted 4 hours and there were no failures. The testing went very well considering this is a new rig with new crews. The rig was clean and all equipment appeared to be properly rigged up. The location was clean and all extra equipment was properly stored. Hazards M-Pad is not classified as a H2S drillsite. Stuck pipe while drilling the Kingak has been reported in offset wells, however, the Kingak will not be penetrated. Lost circulation has been reported in the Sag River, Shublik and Ivishak production intervals. Three 10'-15' faults will be crossed in the polygon. Only minor losses are expected, however, potential for higher losses is possible and will need to be monitored closely. Shallow gas (Ugnu, W. Sak) will not be penetrated. Put River (gas) will not be penetrated. Disposal Cuttings should be hauled to the ball mill at CC-2A. Fluid Handling: Haul all drilling and completion fluids and other class II wastes to CC-2A for injection. Haul all Class I wastes to Pad 3 for disposal. In addition, haul all cuttings from drilling operations to CC-2A for disposal. M-12Ai Summary of Operations Pre-Rig Operations: The objective of the pre-rig work is to ascertain the condition of the 9 5/8' casing presently in the hole; prepare well to be P&A'd by rig; chemical cut existing tubing in the first pup joint above production packer; and test the 9 5/8" pack offs. 1. Notify AOGCC of initiation prior to commencing any P&A operations. 2. NU on Well. 3. Bullhead Seawater down tubing to kill well and establish injectivity. 4. Perform CMIT on 9 5/8" casing and 7"x5 Y2" tbg to 3500 psi. IA/OA. 5. RIH with CTU, spot cement (84 sks; 12 bbl plus 5 bbls for squeeze.) from 9930'MD to place TOC at -9600' MD. 6.
