Hi Tom,WB-06 failed a MIT IA pressure test yesterday the following information was recorded during the test:T/l/O=2350/2350/150. SI well.
MITIA FAILED to 3000 psi. LLRT .35 GPM at 3000 psi. (High IAP). IA FL @ surface. Bled IA from 2350 psi to 600 psi in 1 hour (Gas and
Fluid). T/I/O=1450/600/150. PT hose and manifold to 3500 psi. Pressured IA to 3000 psi with 5.5 bbls of methanol. First mins IA lost 250
psi. Re-pressured to 3000 psi with 10.35 gallons. IA lost 150 psi in first 15 rains and 110 psi in second 15mins for a total of 260 psi in 30
mins. Pressured IA back to 3000 psi with 17.38 gallons. IA lost 80 psi in first 15 rains and 150 psi in second 15 mins for a total of 190 psi in
30 rains. Bump pressure back up to 3000 psi with 10.42 gallons. IA lost 100 psi in 15 mins and 100 psi in 15 mins for a total of 200 psi in 30
mins. Re-pressured IA to 3000 psi with 7.53 gallons. IA lost 100 psi in first 15 mins and 100 psi in second 15 mins for a total of 200 psi in
30 rains. Re-pressured with 6.77 gallons. Bled IA down to 600 psi.
Pumped a total of 42.1 gallons for average LLRT-IC of .35 GPM at 3000 psi. Total test time 120 minutes. Pumped a total of 6.5 bbls down
IA. Put will BOI. Final WHP's=1370/800/150.The results of the test indicate we have slow tubing by inner annulus communication. We
request to be allowed to keep the well on injection while a formal state sundry application is submitted to your office for review and
approval. Please let me know if further information is required. Kevin.

The Alaska Oil and Gas Conservation Commission ("AOGCC") approved the following
referenced permit to drills. The AOGCC has a long-standing policy of releasing
cancelled or expired permits two years from the date the application was approved by the
AOGCC.

This memo will remain at the front of the subject well file.
Our adopted conventions for assigning APl numbers, permit numbers and well names did not
specifically address expired or cancelled permits. This omission has caused some inconsistencies in
the treatment of these kinds of applications for permit to drill. Operators have asked us to adopt formal
procedures for this class of permit application in order to prevent future database disparities.
If a permit expires or is cancelled by an operator, the permit number of the subject permit will remain
unchanged. The APl number and in some instances the well name reflect the number of preexisting
redrills and or multilaterals in a well. In order to prevent confusing a cancelled or expired permit with
an active well or multilateral these case sensitive well identifiers will be changed for expired and
cancelled applications for permits to drill.
The APl number for this cancelled or expired permit is modified so the eleven and twelfth digits is -95.
The well number for a cancelled or expired permit is modified with an appended XX.
CANCELED PERMITS ARE RELEASABLE 2 years from the date on the letter received from the
operator requesting the permit canceled.. (Memorandum January 23, 1997 by Dave Johnston,
subject: Canceled and Expired Permits)
EXPIRED PERMITS ARE RELEASABLE 2 years from the date on the application was issued by
the commission, unless the operator ask for extended confidentiality.
AOGCC database has been changed to reflect these changes to this permit.

other governmental agencies, and does not authorize conducting drilling operations until all other
required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximatcly 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installcd prior to drilling new holc. Notice max' be given
by contacting the Commission petroleum field inspector on the North Slopc pager at 659-3607.

CT Sidetrack and Completion
Intended Procedure
Objective:
Plugback existing wellbore. Directionally drill two horizontal wells from the
existing wellbore. Complete with 2-7/8" cemented liner in the lower leg and
with 2-7/8" pre-drilled liner in the upper leg. Perforate the lower leg and
produce both legs.
Intended Procedure:
· Cement squeeze and plugback the existing completion to -- 11490' md.
· Set a whiPstock at - 11480' md. Mill window in 4-1/2" casing at
-11478' md. Drill 10' of formation with 8.9 ppg Flopro fluid and
perform Leak off test to 9.4 ppg.
· Directionally drill 3.75" x 2000' horizontal well( leg 1 - DS12-34A) to
access Zone 4 reserves. Build angle to -96° while drilling and 85° at TD.
· Run and cement 2-7/8" liner to TD. Leave 8.9 ppg fluid in liner.
· Set Whipstock ontop of 2-7/8" linerS- 11365' md. Mill window in 4-1/2"
casing at - 11355' md. Drill 10' of formation with 8.9 ppg Flopro fluid and
Perform Leak off test to 9.4 ppg.
· Directionally drill 3.75" x 2000' horizontal well ( leg 2 - DS12-34AL1) to
access Sag reserves. Build angle to -96° while drilling and 90° at TD.
· Run 2-7/8" pre-drilled liner to TD. Leave top of liner outside 4-1/2" casing at -
11375' md. Retrieve the whipstock set at - 11365'
· Perforate selected intervals in leg 1.
· Place both legs on production.
