Each of these wells was found to have a void in the conductor. These voids were filled with cement if needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. The corrosion inhibitor/sealant was pumped August 2-17, 2008. The attached spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. Supervisor DATE: May 17-18, 2001 FROM: John Crisp, SUBJECT: Petroleum Inspector Safety Valve Tests Kuparuk River Unit 1Y Pad May 17 & 18. 2001: I traveled to Phillips Alaska's Kuparuk River Field to witness Safety Valve System tests. Ernie Barndt was Phillips Rep. in charge of SVS testing. Testing was conducted in a safe & efficient manner. ! witnessed one failure, the LPP failed to trip above 50% of primary separator pressure. The pilot was re-tested & passed the re-test. The AOGCC test report is attached for reference. I conducted 25 well house inspections on 1Y Pad. The majority of the houses were in good shape w/no problems witnessed. Well houses # 30 & 22 were found w/the well house lids resting on the tree cap gauges. The VSM for the flow line on well # 29 looked as if it had shifted up & had the line in a slight bind. I asked Phillips Rep. Pete Nezaticky to inspect this line. Prudhoe Bay Field has had problems w/VSM's shifting up & i wanted to make sure KRU Rep's were aware of this problem. SUMMARY: I witnessed SVS testing in Kuparuk River Field. 16 wells, 32 components tested. 1 failure witnessed. 3.13% failure rate. 25 well house inspections conducted. Each interval will be perforated twice at 6 SPF --30 degrees either side of the Iow side of the hole. A standard pressure control lubricator will be used for this operation. RIH with perf guns loaded 6 SPF and configured shot orientation to ~30 degrees to the right of the Iow side of the hole looking down. The following guns are acceptable: Atlas: 2-1/8" Silver Jet Halliburton: 2-1/8" Dyna-Strip Schlumberger: 2-1/8" Enerjet Tie into short joints immediately below perforation interval. Open the well to flow, Check tie in and perforate lowest interval with gun orientation -..30 degrees to the right of the Iow side of the hole looking down. Flow well for a minimum of 10 minutes. POOH. Note shots not fired and repeat as appropriate. With the well flowing, perforate the same interval with gun orientation ~30 degrees to the left of the Iow side of the hole looking down.
