Per your request, I have reviewed the subject well file for compliance with the plugging regulations in effect when
work was completed.
GB St #2 (180-076) and GB St #2A were drilled in 1 Q 1981. #2A was a redrill. So far as I can determine, the well
was effectively
plugged downhole in compliance with the in effect regulations. The deep plugging actually meets current
regulations. According to the
file, the well was actually suspended 4/13/81, although our records show the well to have been P&Aed on that
date. The surface plug
was set in the production casing and the well head left attached. The wellhead was removed and the well actually
abandoned 4/14/87.
It does not appear that any additional work was done other than removal of the wellhead and welding on of a
plate and the marker post.
Casings were cutoff 3' below original grade. A work summary is included and there are no comments of anything
unusual discovered
during the work. Location clearance was approved in October 1987.
Two items I did note were that 2000' of diesel (195 bbls) was left in the 9-5/8" production casing for freeze
protection and the 9-5/8" x
13-3/8" annulus was freeze protected with arctic pack. In this regard, this well is not dissimilar to several of the
Exxon wells in PTU.
The new regulations regarding suspension, overall plugging and location clearance were effective 4/2/86. The
final work abandonment
work was accomplished about 1 year after the regulations were effective.
In doing the work, I also discovered that there is another well co-located with GB St #2A. It is another redrilled
well, Kup Delta State 51-2
(170-034). This well and the original penetration Kup Delta State 51-1 (170-018) were also effectively plugged
downhole. However, no other ~_
plugs were set in the wellbore. The weighted water based drilling mud was allowed to freeze. The file contains
the record of a meeting and
discussion between the Committee and the Operator where it was determined that additional work would not be
required.

Dear Mr. Kanady:
The Alaska Department of Environmental Conservation (ADEC) has completed review and public notice for
permanent closure approval of the inactive reserve pits at the above referenced production pads and exploration
dril1ing sites located in the Kuparuk River Unit. The closure applications and corrective action plans provided
the required information describing the history and conditions of the reserve pits, results of required water
sampling, and an assessment of the potential risks posed by the dri11ing waste to human health and the
environment. A11 sites were inspected by ADEC after completion of the corrective actions to confirm the work
was completed in accordance with the approved plans. At production sites, aU dri11ing waste was excavated
from the pits for disposal by deep underground injection. The pits were tested for the presence of hydrocarbon
sheening and trace metals before and after waste excavation. At these sites, the pits are closed as open
impoundments. Trace amounts of residual hydrocarbon sheen may remain in some pits. Trace amounts of
residual sheening is not considered a risk. The reserve pits at exploration sites West Sak 9, GwydYf Bay State
2/2A and East Ugnu #1 were capped with the dri11ing waste in place. At West Sak 16, the dri11ing waste was
excavated from the sha110w pit and the pit backfi11ed. ADEC has determined that excavated or capped pits at
these sites do not present a material or probable risk to the public health or the environment.
Public notice requesting comments on closure approval was published September 29 and 30, 2004 in the
Anchorage Daily News and posted on the ADEC web site September 29th 2004. No public comments objecting
to closure approval were received in response to this public notice. ADEC has determined that the reserve pits
at these sites meet the closure requirements of 18 AAC 60.440.

This permanent closure approval is subject to the fol1owing terms and conditions:
1)
In accordance with 18 AAC 60.440(1), the Department wi11 require additional investigation,
assessment, monitoring or remediation if new information regarding conditions at the reserve pit
facilities indicates that further actions are necessary to protect human health or the environment.
2)
The approval granted by this letter is for the inactive dri11ing waste reserve pits only. Closure for the
pad as a whole (if required) must be coordinated between the owner/operator and the appropriate
land owner/manager.
Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18 AAC
15.195- 18 AAC 15.340 or an informal review by the Division Director in accordance with 18 AAC 15.185.
Informal review requests must be delivered to the Division Director, 555 Cordova, Anchorage, Alaska, 99501
within 15 days of the permit decision. Adjudicatory hearing requests must be delivered to the Commissioner
of the Department of Environmental Conservation, 410 Wil10ughby Avenue, Suite 303, Juneau, Alaska 99801,
within 30 days of the permit decision. If a hearing is not requested within 30 days, the right to appeal is
waived.

On Friday, August 14, 1970, Mr. Chuck Champion called and requested
verbal approval to plug back part of the open hole in the subject
well. Well had been drilled to ,TD of 13,605' with no porositY
present. 9 5/8" ca~ing had been set at 11,739. Top of Lisburne
was called at 12,145' with no porositY present.~ He proposed to-set
a 100 sack plug at 12,200' up to ll,921',~and set a bridge plug at
11,700' in the 9 5/8" casing. I verbally approved the program
providing he place a 100' cement plug on top of the bridge plug
at 11,700' in the easing. They plan to test up the hole and will
contact us further regarding additional abandonment procedures.

Well sign made and posted. Remote control valve has been replaced. BOP drills
are being cOnducted regularly. The Kup Delta 51-1 was plugged back to 3350'
and a directional well 51-2 is presently'drilling.

North Slope BOF Inspection
Tuesday, May 19, 1970, Homer Burrell, Tom Marshall, Easy Gilbreth, and
I met with Mr. Jack Fogerson, Operations Manager for Hamilton Brothers'
North Slope operations, The meeting resulted from my inspection report
(dated 5/21/70) on the BOP equipment on Hamilton Brothers Milne Point
and .Ku~ drilling wells on the North Slope. The deficiencies noted
during the inspection of each well were listed on the check sheets used
during the inspections and each item was discussed in detail with
Mr. Fogerson. Mr. Fogerson indicated that all points of contention would
be taken care of as soon as possible and that in fact, one set of 5,000 psi.
BOP equipment had already been ordered for the Kup Delta well.
Mr. Gilbreth informed Mr. Fogerson that 5,000 psi (W.P.) BOP equipment
was required at or before 10,500' and that this ruling would apply to
the Milne Point well. Mr. Fogerson responded that he would order
equipment to comply with this regulation.
Mr. Burrell then requested that our office receive a response by letter
as soon as possible to let us know the corrective measures taken on each
well with regards to the deficiencies noted.. We will await their.
written reply to our request at 'this meeting and make further inquiries
and inspections in the near future if'it seems necessary,

proposed work. '
1. It is proposed to run 13-3/8" casing as described in the application for permit to
drill this well, except 3 loops of 1" tubing to be cemented in the interval 300' to
the surface between the 13-3/8" and 20" casing strings at the time the casing is
run. These tubing loops will be used as refrigerant coils fOr circulating coolant.
(Total 1800' of 1" tubing.)

It is further proposed to band 1" molded polyurethane rigid foam on the outside of
top 300' of the 9-5/8" casing string to form a thermal insulation barrier between the
9-5/8" and 13-3/8" casing strings.
3. It is estimated the bottom of the permafrost in this well is from 265' - 275'.
