General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and
leases in Alaska.
Item 1a: Classification of Service wells: Gas injection, water injection, Water-Alternating-Gas Injection, salt water disposal, water
supply for injection, observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
Item 4b: TPI (Top of Producing Interval).
Item 8: the Kelly Bushing elevation in feet abour mean low low water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
Item 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).

permit application for the sidetrack, 3S-17 A, will be submitted by 4/29/03.
Attached are the plug back procedure and a well bore schematic. ConocoPhillips hereby requests a
waiver of the tagging requirement on the lower abandonment plug. The plug will be set from the bottom
of the hole and an additional 100' of cement will be placed above the Kuparuk "C" sand to ensure
isolation.

35-17 Plug Back Procedure
1. Contact inspector to witness job.
2. PU +/- 947' cement stinger w/ slotted flow diverting joint on bottom.
3. RIH with stinger and 5" DP to TD.
4. Circulate and condition mud to obtain consistent mud weight in and out.
Pluq # 1
5. Spot balanced cement plug to cover the Kuparuk "c" sand interval. (see cement
calculations) Reciprocate pipe until cement reaches the end of the plugback string.
Spot plug at 8-10 BPM.
6. Pull stinger and DP out of plug no faster than 1 stand / 2 minutes. Pull out of hole to
+/- 8300' MD.
7. Circulate up spacers and contaminated mud/cement the long way at maximum rate.
8. POOH to 7100' MD and spot a weighted (1 ppg over mud weight) high viscosity pill
from 7100' to 6800' MD.
Plug # 2
9. POOH to 6800' MD / circulate and condition mud to spot sidetrack plug. Reduce the
Yp to 15-20. Balance mud weight in and out.
10. Spot balanced cement plug from 6800' MD to 6200' (see cement calculations).
Reciprocate pipe until cement reaches the end of the plugback string. Spot plug at 8-
10 BPM.

I have looked at the proposal for plugging the lower portion of 3S-17 in order to sidetrack
to a new BHL. You have noted that there was only 6' of pay at the original BHL.
The procedure you propose is acceptable. Since you plan to set the deep plug on bottom, we
will waive the need to tag the plug. However if operational information (lost returns or
pipe drying up early) would lead one to doubt that the plug was placed as designed, it
would then be prudent to then tag the deep plug. Since you are sidetracking, the upper
plug will be tagged in the drillout process.
Thanks for providing this information and dispensing with a late evening phone call.
Good luck with the operations. Please call the NS Inspector or myself with any questions.
I will look forward to the paperwork today and tomorrow.

35-17 Plug Back Procedure
Scott Lowry spoke to Chuck Scheave with AOGCC @ 21 :304/27/03 and obtained verbal
approval of the following plug back procedure. Scott Lowry will speak with Tom Maunder on
4/28/03 to obtain a waiver on tagging the abandonment plug.
1. Contact inspector to witness job.
2. PU +/- 947' cement stinger w/ slotted flow diverting joint on bottom.
3. RIH with stinger and 5" DP to TD.
4. Circulate and condition mud to obtain consistent mud weight in and out.
Plug # 1
5. Spot balanced cement plug to cover the Kuparuk "c" sand interval. (see cement
calculations) Reciprocate pipe until cement reaches the end of the plugback string.
Spot plug at 8-10 BPM.
6. Pull stinger and DP out of plug no faster than 1 stand / 2 minutes. Pull out of hole to
+/- 8300' MD.
7. Circulate up spacers and contaminated mud/cement the long way at maximum rate.
8. POOH to 7100' MD and spot a weighted (1 ppg over mud weight) high viscosity pill
from 7100' to 6800' MD.
PluQ # 2
9. POOH to 6800' MD / circulate and condition mud to spot sidetrack plug. Reduce the
Yp to 15-20. Balance mud weight in and out.
10. Spot balanced cement plug from 6800' MD to 6200' (see cement calculations).
Reciprocate pipe until cement reaches the end of the plugback string. Spot plug at 8-
10 BPM.
11. Pull stinger and DP out of plug to 6100' MD no faster than 1 stand /3 minutes while
pulling out of cement.
12. Circulate up spacers and contaminated mud/cement the long way at maximum rate.

2. Install diverter system.
3. Drill 12-1/4" hole to 9-5/8" surface casing point according to directional plan. No LWD logging ,
equipment required. /
4. Run and cement 9-5/8" 40#, L-80 BTC casing string to surface. Employ lightweight permafrost
cement lead slurry and LiteCRETE tail slurry. Pump adequate excess to ensure cement reaches
the surface. If cement does not circulate to surface, perform top job. Install and test BOPE to
5000 psi. Test casing to 2500 psi. /"
5. Drill out cement and 20' of new hole. Perform LOT.
6. Drill 8-1/2" hole to total depth according to directional plan. Run LWD logging equipment with /
GR, resistivity, and density-neutron. Run 7" 26# L-80 BTCM casing to surface and cement.
Note: If significant hydrocarbon zones are present above the Kuparuk C sand, sufficient cement /"
will be pumped for isolation in accordance with 20 AAC 25.030(d)(3)(C). A cement bond log will
be performed prior to perforating the well.
7. Run 3-1/2" 9.3#, L-80 8rd EUE 8rd-Mod completion assembly wI SAB-3 permanent packer and
PBR. Set packer 350' above top planned perforation, shear out of PBR, P/U tubing hanger, and
land tubing. Pressure test tubing to 3500 psi. Pressure test 3 V2" x 7" annulus to 3500 psi.
Burst shear valve for freeze protect.
8. Set TWC and ND BOPE. Install and pressure test production tree.
9. Freeze protect. Secure well. Rig down and move out.
10. Handover well to Operations Personnel.
Note: If the 7" 26# L-80 BTCM casing must be top-set above the Kuparuk C sand, 6 1/8" hole will be Š
drilled to total depth according to the directional plan and a 3 V2" 9.3# L-80 SLHT liner will be ran and
cemented across the productive interval. The cement bond log will be done after moving the rig off the
well.

Waste fluids generated during the drilling process will be disposed of down a permitted annulus on 35
pad or hauled to a KRU Class II disposal well. All cuttings will be disposed of either down a permitted
annulus on 35 pad or hauled to the Prudhoe Bay Field for temporary storage and eventual processing for
injection down an approved disposal well.
At the end of drilling operations, an application may be submitted to ¡permit 35-17 for annular pumping of .........---
fluids occurring as a result of future drilling operations on 35 pad.
