PWI well MPB-27 (PTD# 2042210) is currently shut in and will not be brought online for its
scheduled 4-year MIT-IA due this month. The well is now classified as NOT OPERABLE
and will remain shut in. A witnessed MIT-IA will be scheduled once the well is returned to
injection.

Subject: Corrosion Inhibitor Treatments of MPB -Pad 1( Y 1i
Dear Mr. Maunder,
Enclosed please find multiple copies of a spreadsheet with a list of wells from MPB -Pad
that were treated with corrosion inhibitor in the surface casing by conductor annulus.
The corrosion inhibitor is engineered to prevent water from entering the annular space
and causing external corrosion that could result in a surface casing leak to atmosphere.
The attached spreadsheet represents the well name, API and PTD numbers, top of
cement depth prior to filling and volumes of corrosion inhibitor used in each conductor.
As per previous agreement with the AOGCC, this letter and spreadsheet serve as
notification that the treatments took place and meet the requirements of form 10 -404,
Report of Sundry Operations.

accepted @ 04:00 hrs, 07/01/2005.
PJSM. Rig up lines to tree & tiger tank. Take on heated
seawater. Install secondary annulus valve. Pull BPV. SITP = 0
psi, SICP = 0 psi, OIA = 210 psi.
PJSM. Pressure test lines to 3500 psi, OK. PJSM on well kill &
over view of rig operations.
Line up to pump heaqted seawater down tubing, taking returns
thru choke to tiger tank. Attempt to pump down tubing,
pressuring up immediately. Double check all valves, OK.
Attempt to pump down IIA, pressuring up also, as with tubing.
DPRB DECOMP PJSM. Continue attempting to establish circulation while
rigging up Hot Oil. Rig up Hot Oil, test lines to 5000 psi, OK.
Attempt to circulate hot diesel, (90 deg), down tubing, 5000 psi
max pressure. Rig pump lined up on itA, pressuring up to 3500
psi max pressure. Alternate pumping on one & bleeding down
the other wI various combinations there of, all to no avail. Ice
andlor thick oil blockage suspected.
PJSM. P/U lubricator, set BPV & rig down Hot Oil.
PJSM. Rig down rig components, scope down & layover
derrick. Move rig components off to the side, in preparation for
rigging up coil tubing unit.
CTU Unit #4 spotted R/U, tested I Tag obstruction at 976' and
jet slushy ice to 1350' I Circulate out well with 350 bbls of
seawater RDMO with Coil
Well prep Set BPV
Move rig over location
PJSM. Nipple up secondary inner annulus valve. Rig up
surface lines & test @ 250 1 3500 psi, OK. Pick up lubricator &
pull BPV. SITP = 0 psi, SICP = 0 psi, OIA = 200 psi.
PJSM regarding well killing operations & overview of workover
ops. Line up & curculate seawater @ 5 bpm I 700 psi, taking
oily seawater returns thru choke & to tiger tank. W /115 bbls
pumped, shut well in & bullhead 10 bbls into formation @ 2
bpm 11450 psi. Resume circulating seawater around @ 5 bpm
11400,25 bbls. Bleed all pressure & shut in well.
Monitor SITP. Zero pressure build up in 45 minutes.
Open well & resume pumping seawater around to clean
seawater returns. Moniotr well, well dead.
PJSM. Set two way check valve & test, OK. NID wellhead &
HCR valve.
PJSM. N/U DSA, test spool & 13518" x 5M BOPE. N/U
secondary annulus valve.
PJSM. Conduct full test on BOPE. Witnessing of test waived
by AOGCC State Rep Chuck Scheve. Test witnessed by
Nabors Tool Pusher Gene Sweatt and BP Company Man Paul
Haynes. No failures. UD test joint. P/U lubricator & pull two

Dear Sirs:
Attached is a sundry application fonn 403 requesting approval to convert well MPB-27
from pre-production to water injection. The well was drilled and completed as a pre-
produced injector in November of 2004. There was only very limited production made
from the Schrader OA sand in an effort to clean up the completion prior to injection.
The proposed work scope will include:
· MIRV, ND tree, NU and test BOPE
· Circulate out freeze protection fluid and circulate/kill well with seawater
· POOH with existing 4-112" tubing completion
· E-line perforate the Schrader NC and ND sands with 45' total perfs
o 4.5-in HSD Powerjet, 5-spf, 4505 HMX
o NC-sand: 7,298-ft to 7,313-ft MD (15-ft)
o ND-sand: 7,322-ft to 7,352-ft MD (30-ft)
o Reference log: MWD log 19 to 29Nov04
o If not yet obtained on pre-rig work, run in PDS mode.
o Short joint: 7,115 to 7, 125-ft MD
o RA Tag: 7,115-ft
· Cleanout the seal bore extension below the liner hanger
· Set RBP and stonn packer baniers, swap out tubing head for dual tubing head,
remove packers
· Run 2-3/8" dual completion
o Set upper dual packer at 7, 160-ft MD.
o Sting bottom completion seal nipple into PBR below 7" liner hanger
· Set BPVs

Rig up lines to pressure test tubing and annulus.
Pressure test tubing to 2000 psi for 30 minutes (lost 20 psi in
30 minutes). Leave 2000 psi on tubing and pressured annulus
to 3500 psi. Lost 200 psi in 10 minutes and pressure on tubing
increased to 3000 psi. Pressured up annulus to 3500 psi again.
Pressure equalized with pressure in tubing at 3250 psi. Held
pressure test for 30 minutes and recorded on chart. Appear to
have small leak from annulus to tubing. Bled off tubing
pressure and sheared DCK-2 valve with 2500 psi differential
pressure. Bled off all pressure on well.
Rigged up Hot Oil Truck and tested lines to 3000 psi. Pumped
88 bbls of diesel down annulus while taking returns up tubing.
Pumped 3 bpm at 2000 psi.
Hooked up tubinglannulus equalizing lines and let well
equalize. Suck lines clear. Rigged down Hot Oil Truck and
rigged down lines from tree.
Rigged up lubricator and install BPV in tubing hanger. RD
lubricator.
Secure well (checked all valves on tree and wellhead).
Recorded pressures (50 psi on tubing and lA, 100 psi on outer
annulus). Released rig at 1400 hrs 12-01-2004.

Drilling Hazards & Contingencies:
· Hydrates have not been reported on B-Pad however nearby pads have seen hydrates to 3000'.
· Tight hole and stuck pipe due to clays and poor hole cleaning have occurred on B Pad wells.
· The Schrader Bluff sands underlying B Pad are expected to be slightly depleted.
Lost Circulation
· We do not expect losses while drilling this well if there is good hole cleaning.
· Losses are possible in the OA lateral while drilling and running the liner. If seepage and/or
losses occur, follow the LOST CIRCULATION DECISION MATRIX in the well plan. Break
circulation slowly in all instances and get up to recommended circulating rate slowly prior to
cementing. Condition the mud before POOH to run casing/liner and watch surge pressures.
Expected Pressures
· The Schrader Bluff sands;?:cre xpected to be normally pressured: 8.7 ppg.
· Due to production from M8 OA, reservoir pressure could be depleted to ~ 7.0 ppg EMW.

9. Run a 4.5" slotted liner with HGR, ZXP & PBR.
10. Displace the well with kill weight brine. lDDP.
11. Run a GR 1 SCMT 1 CCl on E-Line. V
12. Run a 4-1/2" temporary, jet pump completion and sting into the PBR.
13. ND BOP/NU Tree.
14. Freeze Protect.
