The well will be WAG'd to water injection as soon as possible. After the well reaches stabilized water injection (which will likely not occur until after the Alpine plant turnaround that begins in early July), DHD will perform the typical diagnostics including an MITIA and packoff tests. If these tests pass the well will be placed on a 30 -day monitoring period of water injection to confirm integrity for water -only injection. Following this we intend to submit an application for Administrative Approval. Please let me know if you have any questions or disagree with the plan. A well schematic and 90 -day TIO is attached. Each of these wells was found to have a void in the conductor. These voids were filled with cement/sand if needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheets serves as notification that the treatments took place and meets the requirements of form 10-404, Report of Sundry Operations. The cement/sand was installed April 215f thru April 24`h, 2020. The corrosion inhibitor/sealant was pumped on April 22°d thru April 25`h, 2020. The attached spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. We drove to location and took pictures of the repair and I collected some associated paperwork. The repair looked good. All the welds appeared to be v sound. Overall the repair is 19' 9" long. The repair starts 8" below the flutes on the surface casing hanger (the flutes on the surface casing hanger are approximately 2" to 3" above pad grade) and extends down 19' 1". With a pad level of 19' and a tundra level of 12.4' (mean sea level) the repair would extend down approximately 6' below the tundra level. Work is ongoing under Sundry#317-501 to pump a surface casing cement shoe in preparation to repair the surface casing leak. Prior to pumping the cement shoe the OA exhibited signs of possible ice plugging or other restrictions. In order to clear the OA and assure injectivity,we pumped diesel and methanol down the OA. This has seemingly displaced heavier fluids from the OA, as the OA pressure this morning is showing a static pressure of 1050 psi. This is above the DNE specified for the operation of the well in AIO 28.005. The well has been shut-in during these wellwork operations, and will remain shut-in. Our intent is to leave the elevated OA pressure and continue with further wellwork to ensure injectivity in the OA and then carry forward with the cement pumping per Sundry#317-501. Please let me know if you have any questions or disagree with the plan. Thanks, Travis Smith/Rachel Kautz Well Integrity Supervisor,ConocoPhillips Alaska Inc. Colville River Unit Well CD4-291 (PTD 213-110) SUNDRY NOTICE 10-403 REQUEST 10-23-2017 ConocoPhillips Alaska, Inc., (CPAI) intends to pump a cement shoe in the outer annulus (OA) of Alpine CRU well CD4-291. The OA in this well is the annular void between 7" casing and 10- 3/4" casing. At present, the annulus does not contain a cement shoe. The well does have an Administrative Approval to inject water-only due to a known surface casing leak./CPAI is requesting to install a cement shoe to minimize natural pressure inflow into the OA from shallow formations. With approval, the OA cement shoe will require the following general steps: 1. Pump cement design to place 1000 ft linear shoe in the OA. 2. Displace cement with freeze protection fluids. 3. Wait on cement to cure. 4. OA drawdown test. 5. Submit 10-404 to AOGCC. CPAI Well Integrity Supervisor 10/23/2017 WNS INJ • CD4-291 ConocoPhillips to Well Attributes Max Angle&MD TD Alaska,Inc. Alpine Well CD4-291 (PTD 213-110-0) SUNDRY NOTICE 10-403 APPROVAL REQUEST May 23, 2017 This application for Sundry approval for Alpine well CD4-291 is for the inspection and repair of surface casing with a suspected surface casing leak. Subsequent diagnostics indicate that the leak is relatively shallow. ConocoPhillips requests approval to inspect and repair the surface casing. With approval, inspection and repair of the surface casing will require the following general steps: 1. Secure the well as needed with mechanical plug in tubing 2. Remove the floor grate, well house, flow lines, and other equipment as necessary from around the wellhead. 3. Remove gravel around well head and excavate to—20'. 4. Install nitrogen purge equipment for the conductor and OA(if needed) so that hot work may be conducted in a safe manner. 5. Cut windows in the conductor casing and remove cement from the outside of the surface casing for inspection. Remove additional conductor as necessary.Notify AOGCC Inspector for opportunity to inspect. p e -io d o c cv wz 6. CPAI Mechanical Engineers will determine type of patch or repair needed to return the casing to operational integrity. 7. QA/QC will verify the repair for a final integrity check. 8. Perform a low-pressure test(Open OA shoe)to confirm no fluid to surface. 9. Install anti-corrosion coating as specified by CPAI Corrosion Engineer. 10. Replace the conductor and fill the void with cement and top off with sealant. h o o 11. Backfill as needed with gravel around the wellhead. do C v 12. Remove plug(s)and return the well to injection. 13. File 10-404 with the AOGCC post repair.
