June 19,2015 JUN 2 2 2 G' Commissioner AOG'` State of Alaska Alaska Oil&Gas Conservation Commission 333 West 7th Avenue Suite 100 Anchorage,Alaska 99501 Dear Commissioner: ConocoPhillips Alaska, Inc. hereby submits an Application for Sundry Approval to workover Kuparuk Injector 1Y- 12(PTD# 183-026). 1Y-12 was originally drilled and completed in 1983 as a comingled"A&C"single packer completion in the Kuparuk. It was pre-produced until 1985 and then converted over to a producer. In 1988 a rig workover was conducted to install a 5K psi wellhead to allow for gas injection. In 1997 a cement squeeze of the C sand perforations was attempted, however, after two years the cement squeeze partially failed and finally broke down completely in 2010. The proposed workover scope for 1Y-12 will be to pull the existing 3-1/2" single completion, mill out the cement donut covering the C sand, and run a new selective completion. If you have any questions or require any further information, please contact me at 263-4641. These voids were filled with corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10 -404, Report of Sundry Operations.
