Each of these wells was found to have a void in the conductor. These voids were filled with corrosion inhibitor, engineered to prevent water from entering the annular space. As per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that the treatments took place and meets the requirements of form 10 -404, Report of Sundry Operations. The cement was pumped September 8 2010 and the corrosion inhibitor /sealant was pumped December 21 22 23 24 , 2010. The attached spreadsheet presents the well name, top of cement depth prior to filling, and volumes used on each conductor. Ernie Barndt was Phillips Rep. in charge of SVS testing. Testing was conducted in a safe & efficient manner. ! witnessed one failure, the LPP failed to trip above 50% of pdmary separator pressure. The pilot was re-tested & passed the re-test. The AOGCC test report is attached for reference. I conducted 25 well house inspections on 1Y Pad. The majodty of the houses were in good shape w/no problems witnessed. Well houses # 30 & 22 were found w/the well house lids resting on the tree cap gauges. The VSM for the flow line on well # 29 looked as if it had shifted up & had the line in a slight bind. I asked Phillips Rep. Pete Nezaticky to inspect this line. ARCO Alaska, Inc., proposes to convert well 1Y-3 to a selective single completion. The procedure will be as follows: 1. The well will be killed with brine. Weight to be determined after running pressure bomb. 2. The current completion assembly will be pulled. :3. A retrievable packer will be run to verify casing integrity. 4. The well will then be completed as a selective single. A 13-5/8", 5000 psi, SRRA BOP stack will be used on well work. BOP equipment will be tested weekly and operationally tested each trip. A non-freezing fluid will be left in all uncemented annuli within the permafrost int~mval. A 5000 psi WP wireline lubricator will be used and tested on all wireline work. The subsurface safety valve will be installed and tested upon conclusion of the job. Estimated Start: September 4, 1984 RECEIVED .AUG 2 8 1984.
