Well History:

Following pressure tests have been performed to verify well integrity:
- CMITxIA passed to 3,000 psi (8/15/12)
- MIT-OA passed to 2,000 psi (8/15/12)
- PPPOT-T passed 5,000 psi (8/10/12)
- PPPOT-IC failed 2,000 psi (8/10/12)
The parent well was drilled in 1983. B-26A was a coil sidetrack in December 1999 that was
abandoned due to lost returns while drilling. B-26B was drilled as a coil sidetrack in May,
2000. Production declined steadily to 400 bopd and 250 bwpd by October 2000.
In 2005 B-26B was shut in due to high GOR. A PPROF in 2007 shows no opportunity for a
gas shut off.
A plug was set in the tubing tail in 2010 to secure the well for a tree change out. The
MIT-T failed when the WI-IP quickly climbed to 1400 psi and then suddenly dropped
to 0 psi. A video log showed a hole in the tubing between the sliding sleeve and the
SBR as well as collapsed tubing in the tail of the string(at 10,453' MD)
Reason for Sidetrack:
Due to the mechanical condition of the tubing a RWO to fix the integrity problems Oft
would be very expensive and is not justified based on the reserves in the area. The ,) ifr' 1
best use for this well is as a r_c_q_u_sidetrack to a new location. - '
Additional Requests: Not to squeeze perfs in the 2-7/8” liner due to a tubing collapse above
the PX plug. This will fail to satisfy state requirements for 100 ft of cement plug above perfs. �-
The plan is to circulate 1000' TVD plug in the 5-1/2"tubing and IA through the lowermost
GLM.
Pre-Rig Work:
1. Drift 5.5"tubing to 10,300 MD.
2. CMIT-TxIA 3,000 psi.
3. Pull lowermost#1 GLM at 10,259' MD.
4. Circulate-97 Bbls of abandonment cement into tubing and IA.
5. Tag TOC and Pressure cement to 3,000 psi.
6. RU e-line and jet cut 5-1/2"tubing above TOC 3800' MD.
7. Circulate well with 8.5 ppg seawater.
8. CMIT-TxIA to 3,000 psi. MIT-OA to 2,000 psi.
9. Freeze protect tubing and IA from 2,200' w/diesel.
9. Freeze protect tubing and IA from 2,200' w/diesel.
10. Set and test TWC one week before rig moves over the well (Record the test)
11. Function LDS. PPPOT-T to 3,500 psi, PPPOT-IC to 3500 psi.
12. Remove wellhouse, level pad if needed.
Rig Work: (scheduled to begin on April, 2014)
1. MI/RU Doyon 25.
2. Test TWC from below to 3500 psi and above to 3500 psi for 5 minutes.
3. Nipple down tree.Nipple up BOPE(2-7/8"x 5-1/2"VBR's in upper and lower ram
cavity). Test BOPE to 250 psi low/3500 psp i high. Pull TWC.
4. RU and pull the 4-1/2"tubing from the pre-rig jet cut at—3800' MD.
5. PU 4"drill pipe and 9-5/8"clean-out assembly to include a full-gage whipstock mill and
clean-out well to the top of the tubing stub—3800' MD, tag tubing stub.
6. RU E-line, RIH with GR/CCL and 9-5/8" EZSV for 47#casing. Set the EZSV at—3525'
MD (competent sand/confirm with GR)as required to avoid cutting a casing collar while
milling the window. Rig down E-line.
7. Pressure test the EZSV to 3500 psi for 30 minutes and chart.
8. PU and RIH with 9-5/8" Baker bottom-trip anchor whipstock assembly and 8.5"window
milling assembly(to include full gauge string mills). RIH to the top of the EZSV. Orient
and set the whipstock. Shear off the whipstock.
9. Change-out the 9-5/8" pack-offs if required.
10. Displace the well to 9.5 ppg LSND based milling fluid prior to 9-5/8"window milling
operations and other path-forward 8-1/2"drilling operations.

Enclosed please find multiple copies of a spreadsheet with a list of wells from GPB Pad
B that were treated with corrosion inhibitor in the surface casing by conductor annulus.
The corrosion inhibitor is engineered to prevent water from entering the annular space
and causing external corrosion that could result in a surface casing leak to atmosphere.
The attached spreadsheet represents the well name, API and PTD numbers, top of
cement depth prior to filling and volumes of corrosion inhibitor used in each conductor.
As per previous agreement with the AOGCC, this letter and spreadsheet serve as
notification that the treatments took place and meet the requirements of form 10-404,
Report of Sundry Operations.

Thursday, January 18, 2001' I traveled to BPXs B Pad and witnessed the semi
annual safety valve system testing.
As the attached AOGCC Safety Valve System Test Report shows I witnessed the
testing of 17 wells and 34 components with 2 failures. The Pilot on well B-18 tripped
below the required set point and .the pilot on well B-21 was frozen. Both failures
were repaired and sucessfully retested before I departed location.
The new style pilot insulation box fabricated using pipeline insulation installed on
most wells on this pad should be installed on the remaining SVS pilots to eliminate
the freeze up problems evident on this pad.
I also inspected the well houses on this pad while witnessing SVS testing. All
the well houses inspected were clean and the equipment appeared to be in good
condition.

Sunday, May 14, 2000' I witnessed the weekly BOPE Test on Nordic rig #1
which is performing a coil tubing redrill on BP× well B-26b in the Prudhoe Bay
field.
The test went well with all BOP equipment functioning properly and holding their
respective pressure tests with the exception of the outside valves on the choke
manifold which leaked slightly. These were greased and retested "OK". A leak
also developed at the base flange of the BOP's. This was tightened and tested
"OK".
The test time was excessive due to a leaking swab valve that allowed the test
pressures to bleed off during all tests. The grease crew was called out to "grease
lock" this valve which cured this problem. This is a recurring problem I have
noticed during tests of this type and I discussed some possible options with Rick
Whitlow (BPX Co. rep.) that may help.

- 10917 ff. to right and above old wellbore. Added 5 ft to CTD to correct depth to RA marker at 10,880

Hole was clean. Drilled build section to 11260 ft. Tripped to window. Tied-in with RA marker, [+4 ft
CTD]. POOH for less bend motor. Change out Sperry MWD and orientor, motor and bit. RIH with BHA
#3 with 1.3 degree motor. Work to get thru shale 11220-240'. Drill from 11260-290' with full returns and
finish left turn. Drill horizontal from 11290-447' turning right to intersect next fault and head to polygon.
Differentially stuck BHA while drilling. WO dead crude.

Wait on crude to free differentially stuck BHA at 11447'. Pulled free before pumping crude. Short trip to
window. Tied-in with RA tag, added 8 ft to CTD. RIH to TD. Slow drilling. Drilled to 11487 ft. Stacking
wt. Wiper trip to tailpipe. RIH to TD. Slow drilling from 11487-11600 ft. Began losing 0.5 bpm at
11580'. Wiper to window. Losses increased while RIH. Drill ahead making slow hole to 11675' with
losses stabilizing at 1.2 bpm. Wiper to window. Tie-in, add 7'. Mix M-I FormasetTM LCM pill while RIH.

displace with 2% KCI water to latch collar. Jet TOL at 10496' and POOH jetting. Recovered estimated
5 bbls of cement at surface.

Cement out of BOP stack. Performed weekly BOPE pressure / function test. Witnessed by AOGCC.
Grease locked tree twice to get pressure test. M/U Baker 2.28" mill and 2 1/8" motor BHA. RIH with
60jts of 1 ~,4" CSH workstring. RIH with BHA on CT. Drill a few cement stringers from 12170-210'.
Cleanout to PBD at 12253'. POOH leaving fresh FIo Pro mud in liner. Stand back 30 workstring
doubles. LD motor and mill. PT wellbore at 2200 psi for 15 minutes. Lost 30 psi. MU SWS 2" perf
guns. RIH on 30 workstring doubles. RIH on CT to PBD. Drop ball and perforate 12182-188' and
12192-202'. No fluid loss. POOH with perf guns.

POOH with perforating guns. Safety Meeting. Lay down 59 jts. CSH workstring. Safety Meeting. Lay
out SWS perforating guns. All shots fired.
Perforated Interval: 12,182' - 12,188', 12,192' - 12,202' FT BKB, Total of 16 Ft at 6 SPF.
RIH with nozzle on CT and freeze protect the 5 1/2" tubing to 2,000 ft with MEOH. POOH. Closed Swab
and Master valves. Blow CT down with N2. Bleed down CT pressure. Clean rig pits. Pull CT stub to
reel and secure it. N/D BOP stack. N/U tree cap and pressure test with 4,000 psi. Release Nordic #1
Rig at 16:00 Hrs, 05/15/00. Total time on well was 9 days, 6 hours. Moved rig off well, changed reels of
CT and installed 3" mudline in rig prior to rig move to F-1 lA.

Enclosed is the approved application for permit to redrill the above referenced well.
The permit to redrill does not exempt you from obtaining additional permits required by law
from other governmental agencies, and does not authorize conducting drilling operations until all
other required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

B-26a Sidetrack
Summary of operations:
B-26 is currently shut in and on "operation shutdown" status due to an attempted sidetrack in Dec. 1999 that
encountered severe lost circulation resulting in a plug back to the window. Plug back cement was left some
220 ft above the existing window and tested to 1500 psi. Upon re-entry, cement will be cleaned out and the
existing window will be utilized for the B-26a sidetrack. The BHL of B-26a has moved 517 feet from the BHL
permitted for the original B-26a sidetrack, Permit Cf 199-102.
Phase 1: Drift tubing, Service and test tree valves: Planned for April 21,2000. · Top of existing whipstock at 10,870' with top of cement at 10,650'.
· 5 1/2" x 4 ~,,~" tubing will be drifted with a dummy drilling assembly.
· Tree valves will be serviced and pressure tested.
Phase 2: Clean out cement, drill and complete sidetrack: Planned for April 26, 2000.
Drilling coil will be used to clean out cement above the whipstock and through the window then drill
and complete the directional hole as per attached plan.
Mud Program: · Phase 1' Seawater
· Phase 2: Seawater and FIo-Pro (8.6 - 8.7 ppg)
Disposal:
· No annular injection on this well.

All drilling and completion fluids and all other Class II wastes will go to Grind & Inject.
All Class I wastes will go to Pad 3 for disposal.
Casing Program:
· 2 7/8", 6.16#, L-80, ST-L liner will be run from TD to approx. 10,485' MD and cemented with approx.
20 bbls. cement to bring cement 200 ft above the window. The liner will be pressure tested to 2200 psi
and perforated with coiled tubing conveyed guns.

Pipe rams, blind rams and the CT pack off will be pressure tested to 400 psi and to 4000 psi.
The annular preventer will be tested to 400 psi and 2000 psi.

· Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging
· An MWD Gamma Ray log and a Phase Resistivity log will be run over all of the open hole section. A
Memory CNL log will be run inside the liner.
Hazards
Several faults with lost circ. potential will be crossed. Res. pressure is normal.
Reservoir Pressure
Res. press, is approx. 3344 psi @ 8800ss. Max. surface pressure with gas (0.12 psi/fi) to surface is 2288
psi.
