Here is an update on the well's performance since the last correspondence.
After the request below was submitted, the well was WAG'd to gas on 12/19/15. Originally the well did not take any gas
injection and didn't really start taking gas until 01/18/16 (see attached sheet including injection volumes). Once the well
started taking gas the IA pressure seemed to stay down until a sudden pressure spike on 02/16/16. The IA pressure was
bled down that evening and did not have a sudden pressure build up during the diagnostic monitor immediately
following the bleed.
It is ConocoPhillips intention to start the 60 day injection test on gas when the well started taking gas (01/18/16) rather
than when the well was WAG'd to gas (12/19/15) and continue monitoring the pressures to see if the pressure spike
repeats. This would end the gas test on 03/18/16. If the pressure spike repeats then the well will be WAG'd back to
water at that time for 30 days in preparation for an AA to continue injection.
Please let me know if you disagree with this plan. Attached is a recent 90 day TIO plot and the injection volume data.

The initial test period on water is ending. Once the well was WAG'd to water the injection temperature and rate were
more stable which also showed a more stable IA pressure. Passing tubing packoff tests and a diagnostic MITIA were
performed on 12/10/15. Currently the well shows no signs of communication while on water injection.
It is ConocoPhillips intention to WAG the well back to gas injection in attempts to confirm if the communication exists or
if it is on gas only. The test period will be for 60 days. The first 30 days will include some extended bleeds on the IA if
the anomaly repeats to try and get the IA pressure to stabilize. Once stabilization is achieved then the last 30 days
would be to confirm no signs of communication. If at any time communication is confirmed,the well will be WAG'd
back to water injection and an AA will be submitted for continued injection. The well will remain on water injection until
it can be WAG'd to gas injection. Please let me know if you disagree.
Attached are a new TIO plot and a copy of the diagnostic MITIA that was performed.

Kuparuk injector 1Q-14 (PTD 185-045) was reported with an elevated IA pressure. Initially looking at the injection
volumes coupled with the temperature swings, the IA pressure swings were suspected to be thermally related. With the
unstable injection continuing, it is difficult to confirm whether the IA pressure swings are thermally related or
communication while on gas injection. The well will be WAG'd to water and initial testing will include tubing and inner
casing packoff tests, an MITIA and an IA DD test if the MITIA passes. Further diagnostics will be performed as needed.
It is ConocoPhillips intention to WAG the well to water for 30 days. Pending the outcome of the test on water, a request
may be submitted for another test on gas injection or an AA application for continued injection on water only will be
submitted. Please let me know if you disagree.
Attached are a wellbore schematic and a 90 day TIO plot.

Dear Mr. Maunder:
Enclosed please find a spreadsheet with a list of wells from the Kuparuk field (KRU). Each of
these wells was found to have a void in the conductor. These voids were filled with cement if
needed and corrosion inhibitor, engineered to prevent water from entering the annular space. As
per previous agreement with the AOGCC, this letter and spreadsheet serves as notification that
the treatments took place and meets the requirements of form 10-404, Report of Sundry
Operations.
The cement was pumped November 20, 2008 and the corrosion inhibitor/sealant was pumped
December 2-3, 2008. The attached spreadsheet presents the well name, top of cement depth prior
to filling, and volumes used on each conductor.

May 17 ,,200!' I traveled to PhilliPs Alaska's Kuparuk River Field to witness
Safety Valve System tests.
Ernie Bamdt was Phillips Rep. in charge of SVS testing. Testing was conducted
in a safe & efficient manner.~l witnessed one failure, the SSV on 1Q-05 would
not pass the DP test. The valve was serviced & would not pass the re-test. Well
1Q-05 is shut in until repaired & re-tested. The AOGCC test report is attached
for 'reference.
I inspected 20 well houses on lQ Pad w/no problems witnessed.

15 wells. 30 components tested. 1 failure witnessed. 3.33% failure rate.

November 13, 2000: I traveled to Phillips AlaSka's KRU lQ Pad to witness
Safety Valve System tests.
Kuparuk's problem wells group now has a down hole diagnostics crew for wells
work, this includes SVS testing. The gentlemen I worked with were not
comfortable with testing procedures for MI wells in Kuparuk. Test procedures
were discussed & everyone agreed that improvements in procedures could be
implemented, we were able to test one producer & two MI wells after my arrival
/
on lQ Pad. DHD crew waS scheduled for safety training after lunch. When DHD//
crew's training is completed & test procedures are in place it will be a good
improvement for State testing in Kuparuk.
SUMMARY: I Witnessed SVS testing in KRU on lQ pad.
3 wells, 6 cOmponents tested. No failures witnessed.

As per Conservation Order No. 173, Rule 5, paragraph (a) (2), ARCO Alaska requests
Commission approval to replace the failed surface controlled, subsurface safety valve
in Injection Well 1Q-14 with a downhole, spring-controlled, injection valve. This
injection valve will act as a one-way check valve, allowing downward injection of water
and gas while preventing-upward flow of these fluids. The injection valve is capable of
preventing uncontrolled flow by automatically closing if such a flow should occur.

On October 25, 1985 Kuparuk well 1Q-14 was converted from a producer

Per our conversation on September 4, 1985, ARCO Alaska,
Inc. proposes to convert 110 production wells to water
injection to implement fullfield waterflood of the
Kuparuk River Unit.
As discussed a single "blanket" sundry notice (form
10-403) has been completed for all well proposed to be
converted to water injection. Individual subsequent
sundry notices will be filed when the actual well
conversions occur.
As stated in form 10-403, well conversion to water
injection will occur in two primary stages. The first
stage will convert approximately four wells per
drillsite to achieve a nine-spot pattern. First stage
conversions are expected to be completed by
approximately 10/15/85, prior to waterflood startup.
The second stage of conversions will convert
approximately four additional wells per drillsite to
achieve a line drive pattern. Second stage conversions
are expected to be completed mid-December to January
1986.

On July 1, 1985, the "A" sands of Kuparuk well 1Q-14 were
hydraulically fractured and propped to improve production.
treatment consisted of 345 BBLS of gelled diesel as pad and
fluid and 1000 LBS of 100 mesh and 10,420 LBS of 20/40 sand
proppant. 60 BBLS of gelled diesel was used as a flush.

to AP, CO's' Kup. ~'Q,14 Fell to standby for the BOPE test.
Friday,...May 17, .1985: The BOPE test commenced at 11:45 pm and was
concluded at- 1-15 am on May 18, 1985. There were no failures and I
filled out an AOGCC BOPE inspection report which is attached.
In summary, I witnessed the successful BOPE test at ARCO's Kup.

Enclosed is the approved application for permit to drill the
above referenced well.
If coring is conducted, a brief description of each core and a
one cubic inch size chip from each foot of recovered core is
required. Samples of drill cuttings and a mud log are not
required. A continuous directional survey is required as per
20 AAC 25.050(b)(5). If available, a tape containing the
digitized log information shall be submitted-on all logs for
copying except experimental logs, velocity surveys and dipmeter
sur~;eys.
Many rivers in Alaska and their drainage systems have been
classified as important for the spawning or migration of
anadromous fish. Operations in these areas are subject to
AS 16.05.870 and the regulations promulgated thereunder (Title 5,
Alaska Administrative Code). Prior to commencing operations you
may be contacted by the Habitat Coordinator's Office, Department
of Fish and Game.
Pollution of any waters of the State is prohibited by AS 46,
Chapter 3, Article 7 and the regulations promulgated thereunder
(Title 18, Alaska Administrative Code, Chapter 70) and by the
Federal Water Pollution Control Act, as amended. Prior to
co~encing operations vou may be contacted by a representative of
the Department of Environmental Conservation.

To aid us in scheduling field work, please notify this office 24
hours prior to commencing installation of the blowout prevention
equipment so that a representative of the Commission may be
present to witness testing of the equipment before the surface
casing shoe is drilled. ~There a diverter system is required,
please also notify this office 24 hours prior to commencing
equipment installation so that the Commission may witness testing
before drilling below the shoe of the conductor pipe.
In the event of suspension or abandonment, please give this
office adequate advance notification so that we may have a
witness present.

ARCO Alaska, Inc., proposes to drill a Kuparuk River Field development well to approximately 8989' MD (6469'
TVD). Selected intervals will be logged. A 20", 2000 psi annular diverter will be installed on the 16"
conductor while drilling the surface hole. The 13-5/8", 5000 psi, RSRRA BOPE stack will be instal]ed on the
9-5~8~'$~ surface pipe. It will be tested upon installation and weekly thereafter. Test pressure will be 3000 psig i
~gd8~:(~-,~o~u psi for annular). Expected maximum surface pressure is 2841 psi. Pressure calculations are based on
virgin reservoir pressure. Surface pressure is calculated with an unexpanded gas bubble at surface, includingd~r.,~l~d~

the surface. There are no closer wellbores anywhere downhole. Reserve pit fluids will be disposed of by
pumping them down the 7" x 9-5/8" annulus of this well. The 7" x 9-5/8" annulus will be left with a
