The Alaska Oil and Gas Conservation Commission ("AOGCC") approved the following
referenced permit to drills. The AOGCC has a long-standing policy of releasing
cancelled or expired permits two years from the date the application was approved by the
AOGCC.

Cancelled or Expired Permit Action

This memo will remain at the front of the subject well file.
Our adopted conventions for assigning APl numbers, permit numbers and well names did not
specifically address expired or cancelled permits. This omission has caused some inconsistencies
the treatment of these kinds of applications for permit to ddil. Operators have asked us to adopt
formal procedures for this class of permit application in order to prevent future database disparities.
If a permit expires or is cancelled by an operator, the permit number of the subject permit will remair
unchanged. The APl number and in some instances the well name reflect the number of preexistin(,
reddils and or multilaterais in a well. In order to prevent confusing a cancelled or expired permit with
an active well or multilateral these case sensitive well identifiers will be changed for expired and
cancelled applications for permits to ddll.
The APl number for this cancelled or expired permit is modified so the eleven and twelfth digits is 9r,.
The well name for a cancelled or expired permit is modified with an appended xx.
These procedures are an addendum to the AP! numbering methods descnbed in AOGCC staff
memorandum "Multi-lateral (weilbore segment) Ddiling Permit Procedures, revised December 29,
1995.
AOGCC database has been changed to reflect these changes to this permit.

R- 16L1 Sidetrack
Summary of Operations:
A through tubing horizontal lateral is being added to well R-16 to improve production from the Sag
River interval.
This sidetrack will be conducted in two phases.
Phase 1' Cut Window: Planned for 6/9/97. - The existing Ivishak perforations will be protected with sand and FIo-Pro.
- A retrievable through tubing whipstock will be set on ELine at approximately 11,295' MD.
- Utilizing a conventional coil tubing unit, a window will be cut in the 7" liner and 10 ft of
formation will be drilled.
Phase 2: Drill Directional Hole: Planned for 6/14/97.
- Directional drilling coil tubing equipment will be rigged up and utilized to drill the directional
hole as per attached plan to planned TD of 13,889' MD (8960' TVDss).
Mud Program: - Phase 1' Seawater
- Phase 2: FIo-Pro (8.8 - 8.9 ppg)
Casing Program:
- 2 7/8", 6.16#, L-80, Slotted liner will be run from TD to approx. 11,275' MD, or just outside the
window. The whipstock will then be retrieved to permit access to the lower perforations.
Well Control: - BOP diagram is attached.
- Pipe rams, blind rams and the CT pack off will be pressure tested to 400 psi and to 4000 psi.
- The annular preventer will be tested to 400 psi and 2000 psi.
Directional - See attachments.
- Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging
- A Gamma Ray Icg will be run over all of the open hole section.

Well R-16L1 Prep for CT Sidetrack
Well R-16 is scheduled for a through tubing sidetrack (lateral) to be
drilled with coiled tubing. The following summarizes the procedure
required to prepare R-16 for sidetracking and is submitted as an
attachment to Form 10-401.
1. Perform tubing integrity test.
2. Drift tubing and liner with a 3.7" OD, 20 ft long drift.
3. Run Kinley caliper over the proposed whipstock interval to confirm
liner condition.
4. Underream whipstock interval to insure clean conditions for setting
whipstock.
5. Protect existing perforations by laying sand from PBTD to cover all
perforations below the Sag R.
6. Rig up e-line unit and set retrievable whipstock within the existing
Sag R. perforated interval. The whipstock will be retrieved after
setting a slotted liner in the new Sag R. lateral.
7. Mill window in 7" liner.
8. RU drilling coil and drill lateral. Complete with slotted liner and
retrieve whipstock.
