The Alaska Oil and Gas Conservation Commission ("AOGCC") approved the following
referenced permit to drills. The AOGCC has a long-standing policy of releasing
cancelled or expired permits two years from the date the application was approved by the
AOGCC.

This memo wffi remain at the front of the subject well ~e.
Our a0opted conventions for assigning APl numbers, permit numbers and well names did not
specifically address expired or cancelled permits. This omission has caused some inconsistencies
the treatment of these kinds of applications for permit to ddil. Operators have asked us to adopt
formal procedures for this class of permit application in order to prevent future database disparities.
If a permit expires or is cancelled by an operator, the permit number of the subject permit will remair
unchanged. The APl number and in some instances the well name reflect the number of preexistin,(
reddlls ancl or muitilaterais in a well. In order to prevent confusing a cancelled or expired permit with,
an active well or multilateral these case sensitive well identifiers will be changed for expired and
cancelled applications for permits to ddll.
The APl number for this cancelled or expired permit is modified so the eleven and twelfth digits is 9[
The well name for a cancelled or expired permit is modified with an appended xx.
These procedures are an addendum to the APl numbenng methods descnbed in AOGCC staff
memoran0um "Multi-lateral (wellbore segment) Drilling Permit Procedures, revised December, 29,
1995.
AOGCC database has been changed to reflect these changes to this permit.

Enclosed is the approved application for permit to drill the above referenced well.
The permit to drill does not exempt you from obtaining additional permits required by laxv from
other governmental agencies, and does not authorize conducting drilling operations until all other
required permit-ting determinations are made.
The permit is for a new segment from exisitng well DS 18-28, APl No. 029-2256400.
Production should continue to be reported as a function of the original well identifier.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice mav be given
inspector on the North Slope pager at 659-3607.

CT Lateral Leg Drilling and Completion
Intended Procedure
OBJECTIVE:
Set whipstock and mill window in the 5-1/2" horizontal liner, drill
approximately 1100' of formation. Run 2-7/8" slotted liner.
Retrieve whipstock.
INTENDED PROCEDURE:
1. Cleanout 5-1/2" liner and run tie-in GR/CCL log.

MU and run the whipstock assembly. Setting the assembly in the 5-1/2" at -
10600'. Mill 3.8" window in 5-1/2" casing.

MU drilling 3-3/4" BHA on CT and drill - 1100' MD out of the 5-1/2" liner.
Drill 10' of formation and perform Leak off test to 0.5 ppg above the highest
anticipated mud weight ( 8.9 ppg Flo Pro as drilling fluid).
4. MU and run a slotted 2-7/8" FL4S liner in open hole. Retrieve the whipstock.
5. POOH with CT and RD. Put well on production.
