Cancelled or Expired Permit Action
EXAMPLE: Point McIntyre P2-36AXX API# 029-22801-95
This memo will remain at the front of the subject well file.
Our adopted conventions for assigning AP! numbers, permit numbers and well names did not
specifically address expired or cancelled permits. This omission has caused some inconsistencies in
the treatment of these kinds of applications for permit to ddil. Operators have asked us to adopt
formal procedures for this class of permit application in order to prevent future database disparities.
If a permit expires or is cancelled by an operator, the permit number of the subject permit will remain
unchanged. The AP! number and in some instances the well name reflect the number of preexisting
reddils and or muitilaterais in a well. In order to prevent confusing a cancelled or expired permit with
an active well or multilateral these case sensitive well identifiers will be changed for expired and
cancelled applications for permits to ddli.
The APl number for this cancelled or expired permit is modified so the eleven and twelfth digits is 95.
The well name for a cancelled or expired permit is modified with an appended xx.
These procedures are an addendum to the APl numbering methods described in AOGCC staff
memorandum "Multi-lateral (weiibore segment) Ddiling Permit Procedures, revised December 29,
1995.
AOGCC database has been changed to reflect these changes to this permit.

The permit to drill does not indemnify you from the probable need
to obtain additional permits required by law from other
governmental agencies prior to commencing operations at the well
site.
To aid us in scheduling field work, please notify this office 24
hours prior to con~nencing installation of the blowout prevention
equipment so that a representative of the Commission may be
present to witness testing of the equipment before the surface
casing shoe is drilled. ~here a diverter system is required,
please also notify this office 24 hours prior to commencing
equipment installation so that the Commission may witness testing
before drilling below the shoe of the conductor pipe...

Notes:
Drilling fluid practices will be in accordance with the
appropriate regulations stated in 20 AAC 25.033.
Maximum anticipated surface pressure is calculated using a
surface casing leak-off of 13.5 ppg EMW (Kuparuk average) and
a gas gradient of .11 psi/ft. This shows that formation
breakdown would occur before a surface pressure of 3000 psi
could be reached. Therefore, ARCO Alaska, Inc. will test our
BOP equipment to 3000 psi.
After 200' of departure, there are no well bores within 200'
of this proposed well.
In accordance with Area Injection Order No. 2, excess
non-hazardous fluids developed from well operations and those
fluids necessary to control the fluid level in reserve pits
will be pumped down the surface/production casing annulus.
The annulus will be left with a non-freezing fluid during any
extended shutdown in disposal operations. The annulus will be
sealed with 175 sx of cement followed ~by arctic pack upon
completion of the fluid disposal, i~.~, ~'~~ .........

GENERAL DRILLING PROCEDUR~
KUPARb~ RIVER FIELD
DEVELOPMENT WELLS
1. Move in rig.
2. Install Tankco diverter system.
3. Drill 12¼" hole to 9-5/8" surface casing point according to directional
plan.
4. Run and cement 9-5/8" casing.
5. Install and test blow out preventer. Test casing to 2000 psig.
6. Drill out cement and 10' new hole. Perform leakoff test.
7. Drill 8½" hole to logging point according to directional plan.
8. Run open hole logs.
9. Continue drilling 8½" hole to provide 100' between plug back total depth
and bottom of Kuparuk sands.
10. Run and cement 7" casing. Pressure test to 3500 psig.
I1. Downsqueeze Arctic Pack and cement in 7" x 9-5/8" annulus.
12. Nipple down blow out preventer and install temporary ~.~s tree.
13. Secure well and release rig.
14. Run cased hole logs.
15. Move in workover rig. Nipple up blow out preventer.
16. Pick up 7" casing scraper and tubing, trip in hole to plug back total
depth. Circulate hole with clean brine and trip out of hole standing
tubing back.
17. Perforate and run completion assembly, set and test packer.
18. Nipple down blow out preventer and install Xmas tree.
19. Change over to diesel,
20. Flow well to tanks. Shut in. -
21. Secure well and release rig.
22. Fracture stimulate.

Attached is the proposed right-of way grant you applied for on December 1, 1986.
The grant will not go into effect until you have accepted, by your signature, its
terms and conditions. This acceptance constitutes an agreement between you and
the United States that you will comply with the grant terms and conditions and
the applicable laws and regulations.
Two original copies of the proposed grant 'and a courtesy copy for your records
are enclosed. If you do not agree or can't comply with the grant or its
stipulations, please state your objections in writing.
If you approve of the proposed grant, you should sign both original copies and
return them to this office. We will sign and date them and return a copy for
your files.
The rental has been estimated to be $100.00 per year pending receipt of a fair
market value appraisal. When an appraisal is received, you will be notified by
decision of the appraised value by regulation (43 CFR 2803.1-2(b)).
A fee of $20.00 is due within sixty (60) days of issuance of the grant. This fee
is required by regulation for costs incurred by the United States in monitoring
the construction, operation, maintenance, and termination of the right-of-way and
for protections and rehabilitation of the laud (43 CFR 2803.1-1(b)(2)).
You are allowed thirty (30) days from receipt of the proposed grant to take one
of the above actions, failure of which may result in rejection of the application.

1. The right-of-way shall be issued for a period of thirty (30) years
subject to renewal.

9. Notify us within five (5) days of initiating production from each of the

10. Any potential valuable minerals encountered in the Federal acreage must
be adequately protected. Adequate protection shall be deemed satisfied
by cementing behind casing of any of the following for~ations encountered
under Federal acreage: UG~U, ~EST SAK, K-5, K-10, K-13, or fresh water
bearin~ sands, and by providing the Authorized Officer, upon request,
with well log and production data from wells drilled through the
right-of-way. The ~ureau of Land Management (BLM) agrees to maintain
confidentially of information submitted pursuant to this right-of-way
stipulation to the maximum extent permitted under Federal law.
11. In the event abandonment of any hole is desired, notify us in advance of
commencing the abandonment operations.
12. Teraination. Prior to teraination of the right-of-way, the Holder shall
contact the Authorized Officer to arrange a preterainatiou conference.
This conference will be held to review the termin&tlon provisions of the
grant.
